South Mountain Healthcare and Rehabilitation Center has been great. They've been amazing. The staff has been very helpful in this journey, and they've made her stay very pleasant. She does like the food there. The facility is very nice and comfortable. They are very well taken care of, I would say. She's not in a position where she can use those facilities at this time. The staff has been great from the admissions to the social worker, to the physician assistant, the physician, and the nurses, they're all great.

South Mountain Healthcare and Rehabilitation Center has been great. They've been amazing. The staff has been very helpful in this journey, and they've made her stay very pleasant. This facility should be totally revamped and sterilized. My mom was transported there on a Friday night and we finally got her out on Monday. The facility - provided a private room and was suppose to be recently redone. I have pictures...of dirty tub and sink...(called for someone to clean) rust on commode...feces around toilet...blood on the wall where the call button is. My mom was put in diapers when this should never have been done. She had come from a week stay at hsp where she went to the bathroom on her own. the toilet overflowed and they put towels on top of the mess for 2 hours...and when it was cleaned up finally...no disinfectent. No doctor on Monday as promised...I could go on and on...but I am sending pictures and documents to Medicare and to US NEWS who gave good reviews which we based our decision on. God help the poor people who no one can speak for. Might look ok but take a closer look ...a petri dish of bacteria and germs. No one should be in an unclean facility. thank you for listening. Marian

This is the worst rehabilitation center ever the food is horrible the staff is horrible no one wants to do there job my friend is on a restricted diet and they give the wrong food

Patient on a low residue diet. Within the first 48 hours patient receives the wrong food with every meal. Per the dieticians guidelines outlined in her chart we bring this to the attention of over 6 people on 2 more occasions of wrong food within the same day today and yet now at dinner the mishap continues. Note, literally, we attached a note to the patients room placard stating such, however the dinner meal delivery still was incorrect. Patient did have menu in hand at lunch and was able to inform staff of dietary restrictions for dinner. Patient is suffering from lack of food hence cannot gain energy to build strength. Day one facility fails to have the proper gauze bandages in stock to dress the wound. Day 2 facility fails to have headache medicine on hand requiring family to drive home round trip in late evening for delivery of said item. (Don't they have if not a pharmacy at least the chart to indicate meds generally taken PRN?) Day 3 above. Rehab staff does not coordinate with wound care staff hence double book patient for care causing patient to have to miss one of the prescribed events. Professional staff doesn't coordinate with wound care staff hence patient is double booked again and misses family consultation meeting. Otherwise, staff generally nice and acknowledging. Seems like once they iron out a handful of kinks patient will be on the way to regaining confidence in the facility and hence gaining on recovery. 1 star for now. Time will tell... so will we ;-) you earn your stars...

South Mountain Rehab is nice, and I think the staff is nice and very concerned. They have patience with the family. They are nice and do the rehabilitation for their client. They try to have fun with them. So, I find them good. I wish my mother could stay there. The rooms are nice. It is two patients to a room. The facility is very nice too. They have different activities for them. They have people from the outside come in. They have different amusement forms. They sing. They have gospel and churches. They also do things outside, but maybe they can do a little more outside activities.

The facilities were very nice and updated. The outside was also nice, but more parking is definitely needed. The staff were generally nice, but some were not as knowledgable as they should have been about my grandma's condition. The amenities were nice, there was even a hair salon on the 2nd floor where residents can get their hair done. The eating area was also updated and bright and cheery. Overall, a very nice facility!

My grandmother was a resident of this nursing home for over ten years. She had Multiple Sclerosis and gradually lost the ability to move most of her body so she really needed the best of care. My mother and I would usually go to visit her two nights a week and sometimes on the weekends. The nursing home was always clean and quiet. During birthday parties the family would pile up in her room and we would sometimes go into the lounge with no problems. The staff was always friendly. We kept my grandmother at that home for so long because we liked the service she received but no nursing home is completely without its issues. I came to the nursing home unexpectedly once during daytime hours and my grandmother lay in her bed with a bowl of spaghetti all over the place. My grandmother could barely feed herelf so that didn't make much sense to me. Someone should have been assisting her. Also my grandmother developed diabetes later on in life and she developed a nasty open sore at the base of her back. My grandfather was upset because he felt that the nursing home waited too long to tell us about it. I give this nursing home a three because the spaghetti incident stays with me and I feel that more could have been done for my grandmother's bed sore - but overall the nursing home environment and staff were pleasant.
