Sage Creek Post-Acute is a very nice and new building. It's very clean, they're cleaning all the time, and the layout has about 60 individual rooms. The therapy area and gym were OK, and the nurses and aides were really nice. They also have a nice path with seating places, but it was too hot to take advantage of that while we were there. I had an issue with the staffing on the weekends. It could have been a little bit better in terms of numbers, but overall they did a good job, and the ones who worked with my husband were really kind. They either fed him in his room, or they had an area where he could go out and eat at a table for lunch. I could also eat or buy their lunch. The food was pretty good for that type of community, or I could just bring my own. Our bill was covered by Medicare as well.

My mother was at this facility for the longest week of my life. The way they deal with Alzheimer’s patients is to ignore them. There were days that she refused her meds and they were fine with it. I would go there and she would have no water in her room. Her pajamas mysteriously disappeared and she slept in her clothes. I had to hire an outside caregiver to be another set of eyes. The place also smelled so bad and the nurses seemed like they were bothered if you asked for assistance. Do not send your loved ones there!!!!

Sage Creek is a joke the facility is horrible. The nursing staff is ridiculous they don't give out the medication on time you order your medication is supposed to be out at a certain time and they don't put it out order your medication for 9:00 a.m. you don't get it until 2:00 a.m. the next morning that's unacceptable I would not take my sick puppy to that place if y'all know like I know don't go there don't send your family members over there of all places they said oh we have this going on and we have that going on they don't do nothing for those people over there nothing please take my advice don't send your family members to that facility take them home if you have to not that place that that that's not the place to go thank you

My wife is currently there the care is subpar the staff is rude I tried desperately to get information on my wife was given the complete runaround when all I wanted to know were the results of an ultrasound avoid this place at all cost

What happened to me should not happen to anyone I was in the hospital at Saint Rosa at night at 9 o’clock Sage Creek said they would take me in with my insurance I came over at 9:30 it was dropped off into quarantine when they realized I had those shots I didn’t have to be in quarantine and move it to another room two days later I found the service marginal at best the food just OK very little of it you always were hungry afterwards I saw them put a woman in the dining area they were sit there for three hours with her safety belt on hunched over no one to talk to no one came by just say her there After 20 days they tell me that my insurance doesn’t cover it and submitted me a $36,000 bill they brought me here on the false pretenses they didn’t check the data and then submitted me with a bill it’s not gonna happen worst place ever

My mom was at Sage Creek Post-Acute for rehab. The place was clean, as far as we know. My dad had stayed there before as well, but because of COVID we were never able to actually go in the building and see, and we were not able see her at all. The way they transferred her from there to the other community was really bad. They did not send her medications with her. We were promised a wheelchair; they did not send that. There was really bad communication. We had called several times to try to get a hold of her case worker and never got a callback. Both me and my sister were calling, so we weren't real happy with them. They said that she would get physical therapy, that she was able to do far more than she actually could do, but only when she arrived at the other community. She had basically not been attended to like they were saying. The building looked fine from what we could see, I had been there before when my dad was in there and it looked fine, but we were only limited on what we could see. It was a very nice facility.

My mother had to come here because St Rose Acute couldn't keep her at their facility past 14 days. She was improving and thriving so well from a hip replacement. My mother was here for 10 days and died July 3rd in this facility. I wouldn't place any of my relatives here. The only helpful person was Wendy. Everyone else was to busy in meetings. Because of COVD-19 I could not visit her and had entrust the health care workers to take care of her. The Facility missed her Doctors Appointment and displaced the blame amongst the staff. I also received a 25,000 dollar bill for the month of July , my mother was only in the establishment for 3 Days in July ; but the bill was sent for the entire month, Why? My family never received a sympathy card or phone call from this facility upon my mother's passing. This is just a short summary of the facts involving my mother
