My mother-in-law stayed at the Southlake Village Rehabilitation & Care Center. They have physical therapy capabilities within the facility. The staff members are available and competent. The place was clean. The food is very good. They have a salon, an exercise room, a physical therapy room, and a small dining room with snacks. It was paid for by Medicare.

My sister was at Southlake Village Rehabilitation & Care Center and she was not happy there. There were some trust issues between her and the social worker and I don't think the social worker was doing anything wrong. It just didn't work out for both of them. Her room was clean. The common areas were very nice. I know she had some complaints about the food though. Apparently, there's also terrible cell phone reception there.

They are amazing. The staff is kind and helpful. A daily newsletter is delivered to the room with games and a list of daily activities. Residents can choose the food and snacks they want. Families are updated on the resident’s health and well-being.

My grandmother lived at Southlake Village for two years. She had excellent care by caregivers who truly loved and cared for her. She enjoyed the activities and meals and made friends.

I don’t even know where to start. All’s I can really say is if you have a family member needing rehab, please look else where and make sure you really look into places before making a decision. My dad was at Southlake for 3 weeks and let me just put it this way, I HAVE NOTHING GOOD TO SAY. He had hair in his breakfast one morning, didn’t get therapy like they said before his admission, the whirlpool was threatened to be taken away from him several times after they were letting him do it daily (due to lazy staff) when he was in the whirlpool the nurses sit on their phones. He had a bandage from a port removal that was suppose to be removed the next day of his admission and wasn’t removed until he did it on his own after an appointment 11 days later (that was a source for infection)—how was this going to be explained to our family if something happened? The nurses are rude and act as if they dread being there and are VERY SHORT. The list goes on. I feel very incredibly sorry for those patients who can’t speak up for themselves or remember to tell their families. Please consider who you hire and make sure you are giving these patients the care they deserve. I’m not a happy daughter and can’t stress these issues enough! I don’t feel like saying anything to the “higher ups” as they just stick up for their employees anyways!

Was admitted due to immobility after accident. Had pre existing dementia living at home well. After mobility returned to normal they would not allow him to leave due to mind and were basically causing worse dementia by holding him in the facility against his and families will. Took many days of complaints to get any action taken to get him out

My mom is in Southlake Village. The place is great. Her apartment is very nice. It is nicely furnished. I have not eaten there, but my mom seems to like the food a lot. It is set up with wings, where there are, I do not know, 15 rooms per wing, and they have a common dining room per wing. You do not have to walk very far. It is a nice setup. They have several things going on for the holidays. They have Halloween and the big dinner for Thanksgiving for everybody. It is more of a skilled-nursing place. It has an assisted living place in that one.
