My mom is at Desoto Healthcare Center. They're very good and help my mom with everything because she had a stroke July 1st which affected her right side and she is not able to do anything. My mom is in rehab, and they're really working with her. They keep me well informed about her care and her medications.

If I could not give a star I absolutely would. My loved one was in the facility for 3 1/2 years. Upon first glance things it seemed to be a loving environment with a caring staff, but I soon realized that was further from the truth. If you're concerned about the welfare of your loved one. I would not place them in Desoto Health Care. It isn't a safe place, anyone can walk off the street day or night and walk right through the front door; there maybe someone at the desk in the daytime but you can just certainly walk straight in. I have visited my loved one as late a 12:00 am and was able to walk straight through the front doors and walk back in, not mention passing cCPN'S on the parking lot smoking. On several occasions I've seen residents with Dementia walking behind the nursing station as well as going into other residents rooms with not one employed person monitoring the halls. There is no real leadership at this facility, it's a Pass The Buck kind of place. Medically my grandmother received the worst care there. From her laying in her own fecal and urine for 24 hours, which caused her to have bed sores and has received several Urinary Tract Infections. The family spoke to upper management on numerous occasions about our concerns which fell upon deaf ears not even a follow up. My heart was broken so many times with empty promises with the safeguard and healthcare for my grandmother. This place is INCOMPETENT!!!!

There is a constant horrific offensive odor in that place, to make it worse they try to cover it up with floral spray that makes it even more nauseating. You honestly need to wear a gas mask While visiting at this place. They seriously need exhaust fans throughout the building.

I have never seen a facility with such neglect for a human that they are getting paid to take care of. This facility is horrible and I do not recommend this facility to anyone to care for their love one. My love one cannot walk, bed ridden, and cannot feed herself. They allow her to lay in pain, in her own poop, and wet. Trying to get assistance to help was unreal. How can one that is human that would allow this to happen to another. Please please choose another facility.

Very neglectful facility. They did not help our 84 year old loved one eat after a stroke, nor provide her with fluids. She was dehydrated after 3 days and had to be transported to the hospital. They neglected to place a sign on the door that she required thickener in her fluids to keep from choking or aspirating and did not rinse or even wash her mouth out after she attempted to try and feed herself. Trying to reach someone on staff and get answers as to why she was not answering her phone was almost impossible. They have no phone resin on duty and you continue to get a message on the main line that they are closed. She was not bathed, hands or face even washed off and teeth not provided with anything to brush her teeth. She went in for rehab therapy and came out worse than when she arrived.

The ladies and gentlemen at this facility are very kind and gentle and very attentive with my father. They are very responsive to any questions regarding my fathers health. I love the staff. The facility is clean. I only have one complaint and that is that the administrative staff is not responsive at all. They do not return calls and do not seem to have any sense of urgency in helping me to resolve issues that I have. Very disappointing.

Very disappointed in the REHAB side of this facility. They are NOT equipped for handling someone with medical issues as well as needing rehab. LOTS of EXCUSES. Our loved one ALMOST died on the REHAB floor because they chose NOT to follow drs orders from the hospital. I WOULD NOT RECOMMEND FOR REHAB and/or SKILLED NURSING>
