About Great Oaks Rehabilitation and Healthcare Center in Byhalia, Mississippi

We are a 60-bed facility located in northern Mississippi and only 10 miles from the Tennessee state line. We focus on a home-like environment while also providing up to date medical and rehab services. Our center provides a wide range of skilled nursing and rehabilitation services for both short stay recovery, as well as ongoing supportive services for individuals requiring long-term care. Our focus is on improving the quality of life for those we serve through our orthopedic, neurological, cardiac, pulmonary, wound care services as well as interventions for other medically complex diagnoses, admitting 24 hours a day, 7 days a week.

I am a friend or relative of a current/past resident

I didn't like anything about Great Oaks Rehabilitation and Healthcare Center. I took my husband out after one day. They need to improve their nurses and their cleanliness. They never change his diapers like they said they would. The food was like sandwiches that just weren't good. The bread was hard.

My grandmother came here during a tough time in her life. We were all worried after her fall. We saw Great Oaks was a 5 star facility and decided to put our heart there. From the door everyone made my granny and our family feel like MVP’s. The Administrator personally introduced himself and talked about his Granny. The nurses were so thorough. Her first meal was great, the place was clean, decorated for the holidays. I cannot say enough about this place getting my granny back to us. She loved her time there and they have customers for life from the Hampton/Jones family.

My mother was there for 13 days. They fed her the same tasteless watered down chicken noodle soup for lunch and dinner for 9 days straight and once for breakfast because we told them she needed soft foods. I had to take her meals so she wouldn’t starve. The dining hall his over crowded. Most people barely touch their food and are probably malnutritioned. They gave her the first shower on day 13 which is disgusting. They made her sit in the wheelchair for 4 hours straight on day 3 of being there even though she was crying in pain From surgery from a broken hip. They are under staffed apparently because the next day her 4 hours were up and she was told there were 15 people in front of her. After she used the bedpan they just sat it in the bathroom so I cleaned it up to get rid of the smell. Please don’t take your loved ones here. They make promises to fix things and it doesn’t get fixed. She had no remote for her TV because the TV’s are so outdated and they said we could bring one from home. Ridiculous.

Great Oaks Rehabilitation and healthcare is a beautiful nursing home with a great reputation for quality. The nurses staff care and are always willing to assist. Everyone on the team is willing to go out lf their way to make your experience better. I would definitely recommend this facility to others

This facility is great. It is such a great atmosphere surrounded by love, kindness, and most definitely happiness throughout the building.

Great Facility! My mom was there for 3 months. Staff is great. Therapy Department is amazing. My mom definitely enjoyed her stay.

I visited this facility

This is a hidden jewel in Byhalia MS. The staff is so caring and friendly. They just received an excellent State Survey. Gloria the Dietary Manager is sweet and the food is always yummy and hot. I am glad I came here. The rooms are nice and clean. the therapy gym is large and they have an ADL suite to help get residents prepared for discharge back home

One Star equals terrible. One star is generous! My Mother was at Great Oaks Rehab for three weeks. She was taken out Against Medical Advice and immediately admitted to a hospital for IV antibiotics for massive infection in hip wound and treatment for Congestive Heart Failure that was ignored by the “professionals” at Great Oaks Rehab Byhalia, MS. Do not take your love one to this facility. I wish I hadn’t.

Do not take anyone to this place for any reason. Even after stressing my mother was a high fall risk did not matter to them, obviously. Fell first night she was there. No one let me know, said they would be having X-rays done. Never happened all day. When I arrived she had been taken by ambulance to hospital and no one thought I needed to know. Give meds with no gloves with 2" fake nails. Do not give meds as scheduled by doctor from hospital cause it does not work for them. My mother was walking when she arrived there for physical therapy. Never walked there. Wheel chairs only means of transportation from place to place. Is that physical therapy? Her roommate waited over 2 hours for a change in soiled diapers.

My mother in law was there for one week and we took her out. Three days later she was admitted back to the hospital where she had pneumonia, blood clots in both legs and was completely raw in her adult diaper area. Oh I could say even more especially about her nurses. This is an awful place, please reconsider putting your loved ones in Great Oaks !!

I am/was a resident of this facility

Due to a very serious incident where i iinquired severe frostbite injuries that required wound treatment and intense physical therapy. I was treated for five weeks in which time physical therapy worked with me with compassion and sincere concern for my recovery. The Cnas were very caring and helpful with all my needs. The majority of the nurses helped me through some very painful and trying days and nights and very attentive. The wound nurses helped heal me feet and legs which were in terriable condition. I visited this facility

Some nurses are great and attentive. One shift was anything but attentive. We had to rush our loved one to the hospital because of I attention to oxygen levels. The facility is nice but more attention to patient detail is needed
