My wife is at Good Shepherd Community-Sauk Rapids and it has been a very good experience. People are very friendly and very caring. They visit with you like one of their own. They want to help you as much as they can. It's a very nice place and it's very clean. It's freshly painted. It has nice carpet on the floor. It's well-kept and really in good shape.

- Senior Housing - offering market rate and income-based apartments for adults 55 or 62 and better. Choose from one and two bedroom apartments with many features and amenities to meet your needs.
- Assisted Living - provides a helping hand when you need it. Our services are delivered to tenants in their own apartments and individualized according to need including bathing, dressing, medication set ups, reminders and/or administration, 24 hour on-site staff supervision, emergency response system, up to three meals a day, housekeeping and laundry.
- Memory Cottages - Each of four memory cottages provides a warm, secure home environment for nine tenants living with memory loss. Services include RN Case Management, RN on-call 24 hours a day, 24-hour on-site staff supervision by Care Managers, three meals plus snacks daily, weekly housekeeping and laundry, and life enrichment programming.
- Home Health Care Agency - provides a variety of health care, skilled nursing and therapy services to clients in their home. Our Registered Nurses, Therapists (PT, OT, and ST) and Home Health Aides serve a 30 mile radius from Sauk Rapids.
- Nursing Home - each of our eight household settings create a warm living space with 16 to 20 private suites. Residents gather in their household's common dining and living areas to enjoy meals and activities together. We provide skilled care for short-term or long-term care, in-patient or out-patient physical, occupational, or speech therapy, respite and hospice care.
- ACE Rehab (Advanced Care for Everyone) - this sub-acute short-term household offers specialized recovery care and rehabilitation following an acute illness or injury.

Meals are processed, without flavor, low nutrition and arrive every week with very little variety. You'd have to love bland mashed potatoes and overcooked entries to think well of the food. The nursing home prohibits bird feeders and there are no aquariums, birds or pets of any kind in the nursing home. There is not even a virtual aquarium to watch. Despite having a director and support staff devoted to resident wellness, the activities are minimal and they're typically just passive, uninventive/unimaginative and the same every week, every month. Without volunteer Lutheran church members leading sessions of prayer and Bible study, the low # of activities would be even less than it is. We were surprised that bingo occurs only once a week on Fridays for one hour. Presumably it's because that's the way it's always been done (a common response to inquiries). We see no attempt to provide ongoing mental stimulation. Residents typically remain alone in their rooms -- that is apart from the depressing meal times where they may sit in silence for a long time before/after the food arrives. The CNAs vary in quality, though inconsistency in staffing is a sure bet. Toileting assistance can take 20 to 30 minutes to arrive. My loved one is reluctant to push the call button, knowing it fails to bring him timely aide. Family council meetings occur during normal business hours, making it prohibitive for almost all working adults to attend. Administration works near the main building entrance where it is is notably comfortably climate-controlled, remodeled and far apart from the poor living conditions of the actual nursing home residents. This facility could be so much better if its leaders were open to change and empathetic to the often forgotten, vulnerable residents who have to live their final days/years under their roof. I recommend every executive spend at least one weekend evening (5-10 pm) and one full Sunday (11-7) quietly observing and interacting with nursing home residents. Spend hours staring out the window with them. Eat at least one of their meals with them, too.

My mother has been at Good Shepherd Community - Sauk Rapids for a week now. This place is awesome. I like it. Their activities are awesome. They have things that they do every day. One day, they had a wacky hair day. They have what they call a happy hour and they go to the event center where they have a band playing, and they serve soda and cookies. They have a movie at least once a month. They have manicures there. They have a beauty salon. They just have so much to do. And the staff is just awesome. They are very, very good. They encourage everybody there to go to the little dining room and my mom met some people she was really happy about. They talk to each other. They try to make sure that they keep conversations, and they push everybody to make friends and not be isolated. My mother is an extremely picky eater, but she says the food is very good. This place is huge. It's one community to the next, and my mother likes it. There are different wings to the different levels of care. She has a regular room with a bathroom, a bed, and a TV, and she has a window to look out. All the rooms have windows to look out so it's not dark. I think it's a very good place.

Minimal activities and family member spends a lot of time alone. One staff person yells at the residents and is very impatient. Junk food is provided for snacks and meals are all processed food which residents don’t like. Staff are not welcoming to family members.

Staff is very interactive with the residence. There is a lot of activities for those who are able. And they are very compassionate and caring for the residence I particularly like on Monday when they polish the nails up any and all residents whether it be male or female!

I received top-notch quality care at Good Shepherd ACE subacute rehab. All of their staff went out of their way to give me the best care could possibly get. They were friendly, caring and made my stay a joy to be there! The therapy staff was outstanding! I highly recommend them.

Good Shepherd Community in Sauk Rapids MN is a wonderful place to live. It offers good care, also have lots of social activities, never a dull moment.

My mom is still on the list to get to Good Shepherd Community. It looked very clean, it smelled fresh. It has three different levels of living. There's nursing home, and then the assisted living, and then elderly apartments. They're connected to the nursing home. It's an apartment and they can have someone come in and check on them, and there are nurses available if they need to for medication, but then you pay extra for that. I like the location. My mom and dad raised the family just a couple of blocks away, so it's like her neighborhood. I talked to the receptionist, she's very friendly. There were things for them to do. They could walk over to the nursing home and visit over there if they wanted to. A bus comes occasionally and they go shopping. They have people come in and play music. There's bingo, playing cards and reading to people who can't read.

We had a chance to visit Good Shepherd Community and it's very nice. The staff are very helpful. I liked that everything there is available.

My wife and I visited Good Shepherd Community. The apartments were so small, you couldn't even get a walker into the kitchen area. The staff was very nice. The living room is like a small bedroom. It was clean but the rooms are way too small.

The people that are living in Good Shepherd don't communicate at all. My brother has dementia, but he is much more social person, so for him it won't work. But for other people it would. It's very clean and the staff is good and everything.

I was fairly impressed with the facility it seemed that the staff were qualified and caring. The only downfall was that this was a nursing home and my friend who was a resident there was only 22 at the time she was there. The rehab unit of the facility was very goal focused and it truly seemed like the staff were caring about meeting and reaching goals to achieve discharge.

In my experience the Good Shepherd community was a nice place. The facilities were clean very clean. I have visited other nursing homes in the past and have seen some very unhappy patients. Depressed, some just sitting in the hallways begging to be taken out of there. The residents here seemed content and well taken care of. The staff seemed nice and genuinely compassionate. The "campus" as they call it is fairly large in my opinion, and located on the outskirts of town. Nice calm neighborhood. I do believe they have separate facilities on campus for varying degrees of patient health, such as memory units, etc. I like the fact that they did not keep all the patients together, as I can only imagine how stressful having let's say, a dementia patient as a neighbor would be. That being said, I believe having specified units for these patients provides them with the best care possible also. They are cared for by staff who are specialized in the needs of such a patients. I didn't get a full tour on my visit, these are just some of the things I had picked up on. Leaving me with the impression that although this is not the newest facility I've seen it is definitely a good one. Feels somewhat homey, I guess as homey as a medical facility could. And as said the staff seem genuine, and I noticed a few patient/staff interactions that lead me to believe there's actual compassionate human to human interaction. Not just worker to work.

I thought it was a great experience while I was there and my grandmother says the same. The staff members were unbelievably caring and thoughtful when dealing with my grandma. The facility was top of the line and seemeed to have all of the accompaniments needed in order for her to live a good daily life.

My grandparents were searching for a place for my great grandma. She was having a harder time getting around, and many places were looked over before deciding on Good Shepherd Community in Sauk Rapids, MN.The facilities were excellent and the community was very active. They have daily scheduled events, specialized programs based on care and needs, and when I visit - It sort of "feels" like it really is her home.Everything was spacious and laid out in a manner that made mobility a heck of a lot easier. That was one of the biggest concerns, due to her having to use a walker, and the facilities had MORE than enough room to get around in.They also offer spiritual care, weekly or biweekly housekeeping (included in the rent) as well as excellent community areas to socialize and enjoy the day.They are a slightly hefty monthly bill, that would be the only negative. However, when it comes to the services provided and what it takes monetarily to 'assist' someone of an advancing age, regardless of where, the bill is always going to be large.4/5 on the facilities :)

Good Shepherd Memory Cottage staff was a godsend to our family. Our mother who had Parkinsons and Alzheimers was a resident at the Good Shepherd Assisted Living for a few years until her health and memory required more care and supervision. The staff at Good Shepherd Cottages were absolutely wonderful and provided excellent care. The staff works very hard to make all the residents feel loved and comfortable. When my mother was there it helped to relieve alot of the stress I had been feeling. They helped educate me and helped me understand what was happening. Sadly my mother passed away on Feb 23, 2012, but she was able to stay at the cottages (her home) until the very end. Our family can never thank them enough for the wonderful care they provided for our mother and continue to provide for other residents.

We found Good Shepherd Community in Sauk Rapids MN for my Mother who needs daily support care. We toured the place and I found it nice. Though she hasn't moved to Good Shepherd yet because we're still processing her papers. I will definitely recommend Good Shepherd Community to others.
