The care is terrible at this facility There’s no record of when things are done Don’t ever go to this facility if you need more then general assistance We were promised help with feeding As our mother had fractured her arm This rarely if ever happened Her clothes were rarely changed Getting someone to show up after pushing the button took between 30 and 45 minutes If you ask someone in the hall there only answer is push the button Even after multiple requests they refused to provide a water glass that our mother could drink from 24 oz styrofoam glasses are to heavy with one weak arm Pt and Ot were slim to nonexistent Head nurse was rarely available and certainly not overly helpful Staff new 3 day before with reminders not to feed her before a procedure and they fed her anyway resulting in my 97 year old mother waiting an extra 2 hrs at the hospital The staff doesn’t seem to have any idea why each other are doing No communication is going on to prevent mishaps

My mother was a current resident at this facility after a fall and neck fracture, she lives in an assisted living and was rehabing here. if you love your vulnerable adult, DO NOT SEND THEM HERE PLEASE!!! THE ONLY THING OK THERE WAS THE FOOD On 12/22 we were greeted right away by Ben the nursing director and a couple other staff, and I felt really good about leaving her there. The staff seemed attentive and caring. 12/23 I went to visit mom and she didn't have her neck brace on, it had been off since I left there Friday. She even had physical therapy without the brace on. Nobody knew or seemed to care where the brace was or that she was wearing it. I found it at the bottom of the dresser. (she has this AI disorder and has moments of disorientation, which was a big reason why she was there). I let the nursing staff know and chalked it up to a new patient error. 12/25 I went there for Christmas. We had a nice lunch (the chef is great and the staff in the kitchen all wonderful), I gave her a shower and changed her clothes. I noticed she hadn't had a clothing change but again, new pt-I would let it go and just shower and change her. 12/29 we had a meeting in regards to her progress and the next steps. In the meeting was a caseworker, a floor manager, a nurse from her current assisted living, myself (on the phone-not physically there) and my mom. I think the floor supervisor was the one going over the results from what occupational therapy saw in their evaluation. She needed supervision using the restroom (my mom claimed no one has actually been there when she goes and said that at the meeting- they explained its a technicality which I didn't really understand but let go) they went on to talk about her dressing...she was able to get on her top, help with her pants and her brace still. she is able to eat when food is presented to her, brush her teeth with all the things there..ect. When my mom has a thought she likes to let us know right away, even if it's not the most appropriate time..she chose that moment to ask the staff if they liked the sweatshirt i got her for Christmas. I asked her if that was the boys school spirit shirt I put her in on Monday (5 days ago) after her shower. She said "yes". I said have you changed out of that since monday? She said "No". At that point I had a lot of concerns. Why is nobody noticing she's in the same clothes? Why is nobody helping her change? And how can the OT sign off that she can do and not do certain things when it's clearly not been shown? No Laundry had been done for her, she had not had that brace or her clothes on or off for 5 DAYS! and people were claiming..they were actually documenting...that she was capable of things I knew she wasn't yet. The meeting was at 1. The staff assured me this would be investigated. 3:30 I called my mom and learned she was, in fact, STILL sitting in the same clothes.... I spoke to grace who had just learned of the case, and she passed the info along to Ben. Ben called me right away and said he would assure she would get a shower and change and from here on out this would not be an issue. She did get a nice shower and fresh clothes. I reminded him she needs reminding and help as she has this brain disorder. He repeatedly assured me she was in good hands. 12/30 I went there. She was changed as of her shower the evening before and clean. 1/1/24- HAPPY NEW YEAR! My sons and I went for lunch and low and behold she was in the same clothes as her shower friday night! It's now monday. nobody checked on her yet again! I spoke to the supervisor who was at a loss as to why and really apologetic. 1/2/2024- my mom was told she's getting discharged ASAP. She is nervous as she doesnt know how to put on the brace. Tuesday evening she asks someone to help her put on the brace and they literally told her to "get back in her room" She is a very timid lady so she slinked back in her room (no brace on) and waited a half hour to 45 minutes for someone to walk by and beg to help her. 1/3/2024 (today) I got a call at 830am they want her out tomorrow. She doesn't know how to put on the brace, has been changed a total of 3 times (2 of them being me) in the week and a half she was there and now we are being pushed out?

We chose Covenant Living of Golden Valley. We liked how happy everyone was, the involvement of people in the community, and the interactions that people had with each other. The residents were just interactive, happy, and welcoming. It just depends which room you get. Some have laundry, some don't, and it depends on the size of the room. But one thing about them, there's a buy-in option, but you can redecorate your room how you want it before you move in.

I do not write reviews often but I feel the moral agency to warn others of this “faith based facility”. If you care about your loved one (and your own stress levels) AVOID THIS FACILITY AT ALL COSTS. The staff Director (Ben), and social worker seemingly went out of their way to do everything in the most incorrect and inappropriate way possible. From my dad sitting in feces for over an hour and a half to documented call light response times of over an hour (The social workers response was verbatim “I cannot control that and we were down a staff member and my staff deserves to have a day off, so I’m not sure what to tell you” extremely disrespectful and frankly incompetent social workers (Carrie). Getting ahold of anyone at the facility was nearly impossible the director simply refused to answer or return calls after a certain point and Carrie, in writing stated that she simply would not be returning my calls. We spent over 500.00 out of pocket for transportation to and from doctors appointments which is just one example of something the facility was supposed to take care of and simply did not. On top of the terrible care and “physical therapy” he received in his time here, they gave notice of a discharge on a Friday afternoon that would be happening on Monday morning for a man that is paralyzed, giving absolutely no time or assistance with getting him the equipment he would need to return home. His neurology, cardiology and GP were absolutely SHOCKED at this decision, especially since that same Friday his neurologist sent orders to the facility stating that he still needed TCU care and PT 2 times a day. The social worker filled out the paperwork incomplete so many times that the medical supply company had to schedule a meeting with her and the director to show instruct her how to correctly fill out the orders. What’s really terrifying about this is that this happened to a patient that had active family support, my heart hurts for those going through this “facility” by themselves. I can recount over 3 times that I walked through the hallway and an elderly woman called to ME and asked if I could please get someone to come help her because they hadn’t answered her call light in over and hour and when they did they said they would come back and never did. Please check out the health department violations of this facility—THEY ARE ALL ACCURATE. If the hospital is trying to discharge you or your loved one here because they have a bed open—DO NOT TAKE IT. I have documentation of all of this and will be reporting all of this to the health department and board of social work to hopefully prevent this from happening to other patients who are already in trying times in their lives.

I would avoid this place like the plague. Terrible care. Communication with staff very difficult. Majority have a language barrier. Have not witnessed so many unknowledgeable and unmotivated people masquerading as healthcare professionals in my lifetime. Again, pick another place to go.
