rooms are spacious .very clean, the facility has no odor . the food is good many options on the menu to pick from. therapy was able to get me on track to be able to take my friend home . appreciated the experience . if needed we would return

My mother went to LCC Plymouth after she fell and broke her leg. We should have known what to expect within her first few minutes at the facility. Because she needed to be on oxygen, they were going to connect her in her room--one of the "skilled" staff members picked up the nose canula off of the floor and would have given it to her if I hadn't noted that it was dirty. She regularly had her pills simply left on her tray. We would be told not to shut her door and then we'd show up to visit, find her alone in her room with the door shut. One time she was on the verge of falling off the edge of the bed and they would never have known because they'd shut the door. She was finally moved from the facility because she ended up in septic shock and had to go to the local ER. The only reason the ambulance came was because my sister called them herself after she spoke to nursing staff three times to express concern that my mother was throwing up, was feverish, was in pain. etc. Please don't send your loved one to this facility.

We're very impressed with Life Care Center of Plymouth. The person who assisted me was very, very nice and very accommodating and showed us whatever we wanted to see. She just gave us a total overview and she was very helpful. The facility was very clean. The dining area was very accommodating for the people, whether they had wheelchairs or walkers. I didn't see any apartments. It was just a floor plan that we observed. It was straight and everything. People could be able to go here and go there. There were no obstructions at all.

My sister went to Life Care Center of Plymouth and the experience was good. The appearance of the place was bright, she had a nice room and a spacious area, and she had a roommate. They had a really nice ice cream parlor and she loved that. She also liked the beauty center and nail salon, and they had a coffee social hour. The services there were good, but there were a couple of things that occurred. On the fourth day she was there, her test was positive for COVID, and she never had COVID. So all of the things she was doing there that she enjoyed suddenly had to be stopped, and she had to be isolated for the rest of the time until she became negative. When she finally became negative, it was time for her to go home. The staff was good, but once she got COVID, the staff had to dress up and wear stuff and deliver her food. Other people also had to be quarantined, so everything kinda shut down at that point, which was kinda difficult,but they handled it well. It was paid for by Medicare.

My mother is a resident and we couldn't ask for a better place or care than she is receiving from Life Care/ Plymouth! The staff is extraordinary and they treat our mother as if she were a loved relative. The nurses are professionally knowledgeable, accessible, and caring. The activities department offers varied activities which my Mom looks forward to. The director has also ordered audio books I've asked for. The social worker has called with ideas for innovative interventions and has included us in their implementation. Thanks to everyone at Life Care who has cared for my Mom, Martha!

I cannot say enough about the staff at Lifecare in Plymouth. My mother was there for rehab after suffering a fall. Despite the pandemic, the staff was very communicative, arranged FaceTime calls, and often the doctor would call me directly with updates. Their kindness and professionalism was greatly appreciated during this difficult time.

This facility has amazing personnel. My father was on the 2nd floor rehab center for 4 weeks following major surgery. Everyone was attentive to his needs from regular showers and daily laundry to medicines, OT, PT and nursing. Everyone knew him and would acknowledge him and us when they saw us elsewhere in the building. The PT/OT staff worked him hard, but knew his limits and let him rest when needed. They had great personalities that helped encourage him. He advanced quickly under their care. The CNA staff were generally cheerful and helpful. They were excellent at keeping him showered. The nursing staff worked long hours, even worked overtime... yet ALWAYS had time to immediately acknowledge me and answer my myriad of questions. They were respectful of my dad and me. The administrative staff was quick to learn who we were and spoke to us daily. We had one problem, which they took care of immediately. The downstairs long-term care provides activities that my Dad enjoyed: bingo, musicians, singers, etc. Dad's case worker was very efficient at doing all the paperwork required to transfer him to a different Rehab when we were ready to take him home to another state. The only downside to this facility is the building itself. (Hence the reason for only 4 stars.) Every room is for 2 people, which is okay, but can make it difficult to get sleep if your roommate has problems. The upstairs study/dining room is being used for storage and is very dated in decor. I found it depressing, so we found the downstairs sitting room perfect for our visits. The snack room (where we got free drinks :)!!) is too small and hard to keep clean and neat. The PT gym is too small. Fortunately, they were able to use the outside walkways in the nice weather. Still, I think Dad enjoyed the small gym because he liked hearing all the therapists talking and laughing with the patients. I knew they had done an excellent job taking care of Dad, but when we got Dad moved to another rehab center at home, I really discovered how great they were. The facility at home was new and large and beautiful. The care, however, is severely lacking. Life Care Center of Plymouth has certainly earned my respect.

We're happy with Life Care Center of Plymouth. The care is good, and they are well organized. The staff is very caring. It is located in a nice area with courtyards for the residents. They have clean, standard rooms with two people in each room. They have activities, like exercises classes, trivia, bingo, and cookouts.

My father-in-law is in Life Care Center for rehab. The first floor is for rehab and the second floor for residents. His room is OK, and the staff is very good and very helpful.

My dad needs assistance so he is in Lifecare center of Plymouth. It's a clean and nice place. The food is good. When I visited, the staff was there when he rang the bell or buzzer. They have therapy too. He likes it there.

The Building was ok on the outside, typical brick old building. The inside was in the need of some repairs. The staff my grandmother had working on/ with her where the only reason we kept her there. They were very attentive with not only my grandmothers needs but the needs of her family. They would call with updates with her current medical condition, and when she passed away we recieved phone calls and cards checking in to see how the rest of us were doing with her passing.
