In June 2022 my father came to this facility for rehabilitation. To say we were reluctant is an understatement, we had experienced him being in other facilities and it went terribly wrong. When my sister and I walked into this facility we were so wonderfully surprised. Our father was up and dressed, eating a nice meal, he was clean, and very happy and had made several friends. The relief and the weight lifted that day was inexplicable. In just a few short weeks we were faced with the painful decision of having to consider long term care. The financial manager went above and beyond to make the transition as easy as possible, helping with forms, and being available at any given time to answer questions. The staff and the nurses took the time to joke around with my father and make him laugh while they cared for him. They had a 50s music event which had him dancing in his chair and singing while smiling the entire time. I would recommend this facility to anyone faced with the difficult decision of having to place a loved one in long term care. It absolutely was the best decision for our family.

Rooms are very small. Staff did not react quickly to any concerns. I had the feeling we were in inconvenience to them. this came from many people there. The menu that is posted is probably 50% accurate. They have two choices for most meals but are never asked what you would like. They just bring whatever they want. One time I noticed that one time the menu said sliced pears, we received 2 (TWO) slices of peaches. another time was pork loin with Au gratin potatoes. we got a dead burger on a plain dry bun. Whoopee! think twice about going there.

I was in Sterling Care Forest Hill before for long-term care. It's a good facility, there are good people over there, they make sure that everything is good with you. I have my own room, so it's nice, it's on the first floor. It's one room and one bath. The staff was excellent. The food is excellent. They have a weight room. They have a pool. They have an exercise room. It's really good over there. They had checkers and bingo.

my mother was a patient at Forest Hill Health and rehab for three week. While my mom was a resident there i was in contact with a social worker named Becca. She contacted me everyday to keep me filled in on my moms progress and concerns. Becca made me feel that my moms health and welfare was a priority to her and the therapists helping my mom. I would like to bring this to the attention of Becca's supervisor and let u know that she has gone above and beyond to make this experience less stressful. Thank you Becca and to my moms therapist.

When not given a bedpan in time, pateints are left with wet or dirty diapers for hours resulting in very sore bottoms. Immobile patients are left in wheelchairs or sitting on the edge of their bed for hours because no one answers the call for assistance getting into bed. Rough handling of patients in pain is also a major concern. The overall lack of compassion of some employees needs to be addressed. The PT are the only bright spots of this facility. My loved will not complain for fear of worse treatment. How sad to be immobile, in pain and feel this way.

My mother was at Forest Hill for about two weeks after discharge from acute care. We were both very happy with her care there overall. The social workers were very conscientious about always responding to every one of my calls and messages. My intake call was very thorough and professional, and we covered all of my questions. My mom loved her PT sessions. The senior nursing and therapy staff were also very helpful and followed through on everything that we discussed during our meeting. My mother's stay was during a time of very high COVID cases nationwide, and the procedures there were very strict to prevent exposures. While Mom was there, no cases were reported. There were times when the shifts were understaffed, and that is very challenging for everyone but Mom always felt that the staff who were on duty were doing there best with the situation they were given. The food was not a high point, and some laundry went missing, but I guess these things happen Overall, I would recommend this facility.

Incredibly poor at returning phone calls. Not unusual to call 8 to 12 times over the course of several weeks in an attempt to get an answer. This has gone on for a year....No one seems to be embarassed by this, On the other hand, the past due bill notices are remarkably efficent, coming out generally within a week of mailing the bill. In further irony, they remind the reader of the Admission Agreement terms. Sadly, the agreement doesn't contain anything about communication.

This facility is a joke!!! They pretend to care about people but as soon as you leave your love one there they are neglected, drugged and left alone. They do not want to be bothered by anyone. They treat people like animals. The rooms are dirty, the staff doesn’t communicate with each other. The doctor is a joke. If you care for your love one run away from this place. There should be a 0 rating!!!!

My husband was here for about a month. is room was very nice; the food better than I had expected, and the staff friendly and helpful. Sandra in Unit 300 was excellent with the care she gave him. Always pleasant and funny, she never hesitated to go the extra mile. We can't say enough about her. If he has to come back, we want her unit. Overall, the stay, while longer than we anticipated, was quite pleasant. I was free to go and come at any time; never hassled when I stayed 8 hours or more, and treated with care and respect. The PT crew were amazing! His improvement in strength and balance was quite surprising. Good job, everyone!

My sibling was transferred here after a head injury for rehabilitation. Each visit landed something "off" and staff initially seemed over worked. I filed a complaint because I popped in early one morning to find unsanitary conditions. Buffie, her nurse continued to be rude throughout my visit because I addressed the concern. Her attitude carried into the next day as well, stating she was having a "great day" since she was not assigned to my sibling. Even social worker dismissed the concern by assigning her elsewhere and defending her lies even with photo evidence - leading to arrogance the following day. Unprofessional Unsanitary with an "I am sorry you had to see that." li History of the same while stating they are trying to change. Dinner is always late & cold with missing items. If your sibling cannot communicate, they will be over looked. Rooms are small. Lots of visitors with courtyard access on some days. Calendar of events with alot of movies. PT seems to have their act together, although she passively eluded that I was not always there to have an opinion - sorry lady, my daughter had to move for college while my sister's have all day to chat with you.

My mother was in the care of Sandra at Forest Hill Health and Rehab on her last days on earth. Sandra made her happy and comfortable and provided her the best imaginable service. I can't thank her enough for giving my mother the most wonderful possible experience she could have received. She also made me feel comfortable when I wasn't able to be there that she was treated with grace and had someone there to go above and beyond providing her needs. Kristen her nurse was also another fantastic staff member that cared for my mother. She really touched my mother's heart and it was evident that she genuinely cares for her patients. Both of these individuals as well as many others staff members and social workers although I can't remember everyone's name were very kind to my mother and our entire family during a difficult time in our lives. Thank you to the staff on unit 300 for all you have done for the care of my mother.

I know this facility has the #1 status for me .I have been there 4 times. These professionals are outstanding from the front desk to the custodians are very professional and compassionate to patients.The food was very good and an assorted variety. Physical physical therapist were great and engaging. If I were to be in need of nursing care again ,I would love to go back to Forest. Hill Nursing and Rehabilitation.

A family member is curt in this facility. He is very pleased with the physical therapy department. Unfortunately numerous bad experiences with staff answering the “call button” in a timely manner. This evening waited 1 hr and 45 min for someone to come. He sat in urine for over an hour. The attending aid did not bother to clean him properly and when he asked the aid gave him an attitude. In addition he has had money and his hearing aids stolen and not replaced. Would not recommend this facility.

They were communicative and helpful. They were able to get the equipment and pharmacy we needed in a timely fashion including CPAP, and prescriptions. They went over and above to sit and discuss what was going on and provided all records that were requested quickly. Like all healthcare facilities they seemed overworked, but were still able to make a personal connection and listened to our needs. I recommend Forest Hill Rehabilitation and appreciate their staff.

Our family member and our family have absolutely nothing positive to report about this center. (1) First and foremost, the center has absolutely no security at the main entrance/exit. Visitors are asked to sign in, but photo IDs are not checked/visitors not screened. Absolutely anyone can enter and access your family member and his/her room. (2) This facility is SHORT-STAFFED by their own confession as well as our direct observations. (3) Within the first few days, our family member had an accident and fell to the floor even though the staff had been informed that their patient was at high risk for falling and was already suffering from a prior fall. (4) Their idea of engaging patients is to put the mobile patients into a room to watch a movie. There are no organized activities such as music therapy. (5) The facility has a lack of organization and communication from the top, down. (6) They were quick to push psychotropic medication for our family member without thorough evaluation of medical history. (7) It is extremely difficult to reach staff by telephone--from the administrator (who never returned a phone call--down to one of the social workers and the floor's nurse's station. Even the front desk staff were frustrated when they were unable to reach co-workers by interoffice telephone. (8) The meals looked to be the cheapest, most high caloric garbage you can imagine. (8) The facility has no on-site dedicated physician. The dr. rotates between facilities. Therefore, when our family member fell, only a nurse evaluated our family member for injuries. Our family member was not taken to a nearby hospital for evaluation or x-rays. (9) A few of the staff members are very sweet and kind. However, they are overworked and cannot give your family member one-on-one time due to the staffing shortage. Their schedules also rotate, which is confusing for the patients. Forest Hill is a throw-back to the eras when nursing homes were unregulated and patients were mistreated/neglected. We transferred our family member out of this "rehab" center and into another center, a place where healing can actually begin.

What a horrible sad experience our family had to endure. My father was admitted after being released from the hospital recovering from pneumonia. He was admitted in great spirits, good health and still full of life, all of which was taken away from him in 3 weeks. I could go on to elaborate more on the horrific, uncaring treatment but i will do that in writing in much greater detail to whom it may concern. I would like everyone to know that when my father passed away (which evidently was during the night) I received a phone call at 7:06 am (possibly shift change time when he was noticed) the caller went on to inform me that my father had passed away, but continued to assure me that he had such a rough night that it was OK. Needless to say: I thanked her for making me feel so comforted in knowing that they did all that they could do to make his final hours be so peaceful.

I arrived at this facility for aftercare of a knee replacement. After I signed the paperwork, I was advised they did not have my prescribed medication and find not know when they would have it. The beds are like rocks and I am in extreme pain. Never come to this place. It's awful.

While it is true that the facility isn't as bright and new as some other facilities in the area, the care is what is most important for my father. He is non-ambulatory and dependent for most activities of daily living. He has no cognitive limitations and craves stimulation so he really enjoys talking with his therapists and caregivers. They are very cheerful and encouraging, and this keeps him feeling connected to the world. The OTs/PTs/Speech therapists are top notch. He likes the food! (And he has been in three other hospitals/facilities in the past few months). It does seem as if they could have more aides on staff, as at meal times, the aides are busy serving patients and not as available to help with personal needs.

Forest Hill tours very well. I have a relative there and the Food is great and the Care is wonderful. The staff are compassionate and caring people that have a calling to do that job. My mom has been there for 2 years and has never been mistreated. She always has a smile on her face and loves her caregivers. I would recommend Forest Hill to anyone who is in the situation that needs to place a family member.

My sister is in a nursing home named Forest Hill. It is very nice. It is nicely laid out, and they are accommodating. They have bingo and a bunch of other things like making things and music.
