My dad moved into Jefferson Place. What I like best is the fact that they accepted him for a Medicaid bid. I think you can always improve the medical staff. The food is good. They have multiple activities every day. There is an exercise area, a library, a common area, a cafeteria, an outdoor garden, and a big-screen TV.

This wa a very nice place 20 years ago. But I guess Signature has not put a dime into updates. The first room they took my mother to was absolutely awful. Light over the bed didn’t work, bathroom was outdated and just gross. Furniture was broken. Colors were dark and depressing. The bed must have been 30 years old! They put my mom in a different room (after complaining). It was minimally better. Same bed. I went to Kohls and bought a mattress topper to put on that awful mattress. My mother was there for Rehab after a hip replacement. Physical therapy was less than acceptable. The food was just awful. Night time snack was graham crackers. Most of the staff didn’t seem to care about anything. There were a couple of good employees. Stephanie was one of the nurses that actually seemed to care and we really appreciate her. Thank you Stephanie. A rare helpful MA. I wish I remembered one of the good ones name. Most of the time the MAs were sitting in the side rooms doing nothing. Wow I wish I could go back and have had my mother somewhere else. I should say that we met some full time residents that were very happy there. Maybe it was just the rehab program that was far below our expectations. Mom did well and got out of there asap.

I enjoyed being here. I was well taken care of. This is my second trip here. The staff are very nice, awesome therapy. The food was good. Very nice room.

I toured Jefferson Place. It was a very positive experience. They had a lot of great amenities. The facility looked to be very high end. It didn't look like a hospital setting at all. It looked more like an apartment complex that anybody could live in. The units are suites. You enter and you're like in what they call the living room with a little kitchenette. If you go straight forward, you have a relatively small bedroom, but a pretty good-sized bathroom. Jefferson Place was well kept, from their landscaping to the paint on the walls. They had nice flooring. It was the caseworker that gave us the tour. She was relatively new. The areas that she was knowledgeable about, she spoke well to. And those she didn't, she just told us she would have to get us more information. She was friendly, and the residents were friendly too.

My mom was at Jefferson Place for two weeks. It was a very nice and clean facility. Everyone was very helpful and caring. The therapists were good. The nurses were great. It was a great experience on our end, and Mom did very well there. The food was hit or miss. There were a few days that she was not happy with the food. There really wasn't a selection process like there is at the hospital, but she was fine with it. We would bring her something if her lunch or her dinner was not up to par for her. We would just pick her up something and take it to her. There are all these different activities that go on in Louisville for the Kentucky Derby, and they did things at the facility that mimicked what was going on for the Derby Festival. They made hats, and they had a parade. They had some special activities, and my mom just happened to be there during that time. There was bingo, and there was a really pretty courtyard outside. Luckily when the weather was nice, her therapist would take her out for a walk. The only negative was the food, and that was just a few times that she just was not at all happy with the food. It's just personal preference, so that was really the only complaint that she had.

Physical therapy is wonderful. The rest of the staff, not even close to subpar. Residents sit and wait for a call bell to be answered for over an hour. Comments from staff to residents when call bell is rang is, “what do you want”. My family member was also given Norco when in no pain and it was never requested. Do research and find a good caring place for your loved one.

Stay with your loved one or make sure they are alert and oriented. It looks great but it’s only skin deep. My Dad has mild dementia and if a loved one is not there when we return he is soaking wet with urine and mentally distressed, screaming out. A few times his medication was very late and if I had not called staff no telling what time he would have gotten it. No meat at breakfast due to budget constraints according to a staff member. I’m there 5 to 6 hours at a time everyday and I have not met an activities person nor have I witnessed a resident engaged in an activity.

My mom was a resident of Jefferson Place for 10 days. The experience was not positive. They were vastly understaffed on the weekend. There were long patient waits for any type of assistance. The weekend staff was indifferent. During the week, the staff was great. But the weekend staff was kind of terrible. They didn't do frequent checks. The physical therapist and manager had no idea what was going on with my mom. The social worker lied to us on more than one occasion. I couldn't wait to get her out of here. The facility was very dated. There were geese nearby, and the geese pooped on the front porch, and they left it there for four days. It was very, very expensive. It came highly recommended, and there was a huge disappointment. I never would have let my mother stay there. They would need to improve on family communication, patient wait times, cleanliness of the outdoor facility, the food, and the update of the decor. And when she went to leave, one person said goodbye to her, one. They were terrible. I would never put a loved one in there again.

The facility was great. The staff kept my spirits up while I was there. Therapy was the best. I am mow back home snd doing well. I thank the staff for helping me reach my goal of returning home

My boyfriend was in this facility for PT and IV meds after surgery, he was repeatedly neglected the three weeks he was their had 2 showers during this stay. I called the administrator almost every day and was told he would take care of it and did for that day but it was back to the same things the next day. Was late with his medication every day, and some times didn’t get it at all. As of this week I have left two messages for them to call about things and to ask about the outrageous bill he was sent and no return call. BEWARE DO NOT send you loved ones here

DO NOT PUT ANYONE YOU LOVE HERE!!! Gave my mom the wrong medication and tried to blame her. She consistently waited hours for someone to help her. She felt and was ignored her entire stay. Absolutely horrendous level of care!

This facility is very poorly ran. They lack required staff to be able to assist patients. The nurses and CNAs try their best but are can’t get things accomplished. I was there after an ankle surgery for PT and recovery. While I was there I was diagnosed with the coronavirus. They shipped me out immediately and kept my belongings so I was taken to another Signature facility to be quarantined for 14 days. I would never put some one I loved in this facility.

If all you need is therapy, this would be a good place as the therapists are great and also the food is pretty good. If your loved one will need alot of attention from nursing and aides, this is not the place for them. They have too many people working there that either don't care or are not very sharp. Housekeeping did not do a decent job either, I will not let my Mom return there.

Because of my profession I am in "nursing homes" quite a bit. I have seen many, many of them in three different states. Jefferson Place, in my opinion, is below average. The staff is kind, no doubt about it. That is the most positive aspect. It smells which means beds are not clean, nursing homes don't have to smell. A relative of mine got bed sores while there. Inexcusable. This should NEVER happen if folks are paying attention. I would not send a loved one there.

Staff is friendly when you find them. Long waits for call button to be answered. Food is good, but the food trey stays in room sometimes until the next meal is served or relative/visitor brings it out. Therapy is everyday and the therapist are good. Courtyard is pretty. CNAs are usually really busy in "other rooms" and they tell you that they are short on staff. Church services on sunday are wonderful. My fathers hair was trimmed she did a good job but was charged $50. He had been there a week and got some redness on his underside, medicated cream was ordered. Medicine was put in his room on a wednesday but not opened until saturday night by the aide. Trash in room was emptied every day.

My mother has been a resident for over 4 years, and I visit an average of 5 times a week at different hours. Mom has always been treated with respect and received individualized care. She is always clean and dressed neatly. She has difficulty taking her oral medicine and the nurses have tried many different ways to administer her medications in the way Mom chooses. They are extremely patient! Dr Babar and his nurse practitioner are very available. They are skilled, kind and encouraging. The administrator Jeremy, listens and strives to meet the concerns of residents whether big or small. Monthly residents' council meetings allow residents to express their complaints and praise. As a result the main dining room has reopened and even the supervisors participate in serving the residents. At the residents' request, cloth napkins are provided at each meal. Jeremy has made sure the beauty shop was staffed when the beautician was off an extended length of time due to surgery or illness. Activities keep my mom busy. Typical offerings include daily exercise, bingo, music performances, trivia, painting and crafts. There are several daily options on the calendar. Residents have gone to the State Fair, the zoo and to restaurants. Religious services are available on Sunday. The building and property are well maintained, Currently, nine baby ducklings and their mom are providing lots of entertainment in the courtyard pond! Overall, we are extremely satisfied.

My dad was in Jefferson Place for two weeks rehabilitating from congestive heart failure. Three days before he was released he started to complain of difficulty breathing. He was released went into cardiac arrest two hours after release. One of the key indicators of congestive heart failure - BNP level was 15000 when we reached the ER, normal is 200

My grandmother was in Jefferson place for about 25 days, we were looking at her staying there, but it was too pricey. She liked the staff a lot, and they were very patient. The room looks more like a hospital room, but it was less aesthetic, and cosier. I did not like the common dining area because it looks more like a cafeteria type, it has little tables spaced out, and people sat down. It wasn’t a warm feeling for me.

All I can say is REHAB, REHAB, REHAB!!! Jefferson Place is truly amazing with its rehabilitation department. I had a surgery and there is no way I could have recovered as fast anywhere in the area. The nursing was also very good and the building is beautiful.

I can't say enough good things about Jefferson Place! Everyone from nursing to administration where incredible. I agree with the other reviewer, this is the best therapy in Louisville. I have been to several facilities in the east end and from Middletown to Brownsboro Rd. none hold a candle to this place and their therapy department. This is the "Place" to go!

My mother in law was recently discharged. We were disappointed. The service were awful and the nurses acted as though they didn't care. She would buzz and ask for service or pain medication and they wouldn't show up till 39 minutes later. The weekend staff Was better than the weekday staff. She waited an hour while she was there because U of L was playing and no one responded.

I never thought at my age I would need a facility like this but after a horrible accident I found myself with both wrist crushed and helpless. My daughter is a RN who did her research to find me a place that would be the Best. She was right these people from the administrators to housekeeping care above the just average standards and became not only my care givers but true friends. Thank God there are truly a place where people that love what they do come to work every day to make what could have been an extension to a nightmare a wonderful experience.

The people at Jefferson Place were wonderful to my mother. It was a beautiful place and very expensive. That being said, they were not staffed appropriately; none of these places are. There is not enough staff needed to do the work. I actually thought about moving her, but she was happy enough there. I would recommend Jefferson place to anybody. They had parties and activities all the time. Given the clientele that lived there and the cost, I thought it could have been a lot better; it wasn't as creative as I think it could have been. But, I knew that my mom was well cared for and that she was happy there for the most part.

Jefferson Place is a nursing home located at 1705 Herr Lane Louisville, KY 40222. The phone number is 502-426-5600. At the time I visited my grandmother, Jefferson place was doing upgrades though I could not see a need for it. The hallways are carpeted to keep the noise level down. There is very tasteful art throughout the hallways. One of the best features of the nursing home is that each resident gets a private room. The kitchen takes an order for each resident and delivers the food to their room. The staff is very friendly and helpful to anyone. The nursing stations are centrally located for easy access to all hallways. The small TV rooms across from the nursing stations have TV's and some birds for the residents to watch. Some of the activities include Wii, exercise, and painting nails. There are many wonderful attributes to Jefferson Place that it dose not really feel like a nursing home. I highly recommend Jefferson Place if you have to put a loved one in a nursing home.
