My husband was at Creekside Village for a while. The staff was very nice. The rooms were very nice. There're shared units with a shared bathroom, but it's separate where you don't have to be in contact with your roommate. I like them a lot. The food was very good. And the building was very nice and very clean. The care was very good.

Food was fair. The staff was very dismissive. My Aunt went there after her hip surgery and developed swelling in her legs. She had never had swelling before. Her legs continued to swell and she began getting nauseous. She complained of poor urine output dispite her being on a high dose of lasix (water pill). The nurses kept telling us her vitals were normal and she has to be peeing because they can hear the urine going into the toilet from outside the bathroom door. I kept telling them it’s got to be her heart. And put a hat in the toilet to measure her urine output. They did nothing. She was eventually transporting to the ER. On arrival she was instantly diagnosed with atrial fibrillation with ventricular tachycardia. So her heart rate was very high which should have been detected. They inserted a Foley catheter and she had 0 output and was in kidney failure. She is now on kidney dialysis. So I’m not sure what they were hearing when she complained of no urine output. She was touch and go for quite a while and is still in the hospital. I would never recommend this facility to anyone.

My husband was in short stay for 7 weeks and I can't say enough about how good his experience was. The staff was very attentive and caring. I'd like to give a shout out to all the staff, the nurses, the aids, the housekeeping, the transportation and the cooks. I'm very glad we choose Creekside for my husband's care.

My family member was a resident at Creekside Village for several weeks. During this time she was neglected, disrespected and ignored. Many times I received phone calls from her in tears and in distress because they would not give medications as ordered or make her wait hours to receive her meds. The staff was disrespectful and did not treat my family member with dignity. PLEASE,PLEASE DO NOT PUT ONE OF YOUR FAMILY MEMBERS IN THIS FACILITY!!! I’ve been a nurse for many years and never have I ever seen such despicable treatment of a human being!!! They advertise hospice care yet they would not work with the Hospice service They were rude and disrespectful to the hospice nurse is coming in to help provide care. Several times the hospice nurse stayed for extended periods of time to provide the care staff there should have been doing. This place is deplorable. I tried calling them several times to address issues and didn’t get calls back.

Awful! Do not put your loved one in here! Terrible care, bad communication, understaffed... ON AND ON! My elderly loved one caught Covid here rehabbing from a fall. All they did to treat it was give him syringes full of water in his mouth with some vitamins. He could barely even speak and they wouldn't even let him talk on a phone. Sounds like a prison to me! Thank God he has been transferred out of there now to a hospital for some actual treatment! His POA had to facilitate this through his family doctor. Different stories from different nurses, staff doctor not available to talk. Just NO!

My mom was at Creekside Village for three months. Many times we had to call them for something to be done but it didn't get done. It was like three weeks before my mom got a shower. A lot of things fell through the crack. We weren't really pleased with it. Once I put a note on Facebook that I wasn't very happy with them, all of the sudden, they were trying to take care of Mom a little better. Still there were just things they kept saying that they were going to do and they didn't get them done. Well, the physical therapy part was excellent. They did a great job working with my mom and getting her stronger. They had activities like on Sunday you could go down and get ice cream. They had a great Christmas Party, too. They needed to be managed a little better and the staff needed to be overseen by someone that would make sure things get done. Their biggest fall was them not following through and letting people down.

Under staffed and staff that is in place have attitudes and are not held accountable. Management is horrible starting the administrator. When you try to get a resolution, either it is no call back or "we are aware . . ." which means we don't care. Activities needs a major over haul starting with a manager that has a hands on approach and brings new life to the department and not something that only a few are interested in: happy hour, casino. Beauty shop is deplorable. No set hours, shows up whenever, no set fees. Cash customers and those who tip come first. Beautiful building hiding a deplorable, poorly run establishment and will not change until a major change happens starting with management.

My dad was at Michiana Health and Rehab, and I was actually very disappointed with this place. I heard that they had changed ownership, so it may not be the same anymore. Their rooms were just like hospital rooms. Their care was very poor. The building itself was amazing; it was a very new structure and beautiful, but the way they were running it at that time was really horrible.

I really like the nurses and the person who assisted us. I can't remember what their status is, who comes in and takes care of mom, trying to get her to eat and trying to get her to wake up. They've been really good.
