I am a friend or relative of a current/past resident

My wife is in StoryPoint Fort Wayne West, in the memory unit. She's been here a little over 10 months now, and there have been some things that were lacking. She has fallen three times in her room and they were slow in checking her. It's always happened at night. Sometimes, she's been down for three or four hours before they found her. They're supposed to do room checks every two hours, so I wasn't too happy about that. Another incident was, they have a list of prescriptions they're supposed to get every day and we just learned recently that one of the prescriptions, which happened to be for Alzheimer's dementia, got taken off the list for no apparent reason. So, she went about a month without that before we discovered it when we took her to her primary care physician. In the 10 months she's been here, they've changed the executive administration, and that happened at the end of June, and the new administrator has done a much better job in the quality of recruiting staff. It's been night and day, so there's been an improvement since she's come on board, but on the other hand, like a lot of memory units, there's quite a bit of turnover. Finally, we're getting some continuity in the staff, which with the turnover comes training or lack thereof. I think that was part of the problem as well. I have lunch with my wife about every day here, and it's excellent, both the quality and the quantity. The housekeeping is excellent. The building is older. I think it was built in the 1970s. Therefore, they have some issues with balancing the heating and air-conditioning because it's central. Depending on where you are in the building, your room may be very warm or very cold. I understand it's an old heating and air-conditioning system. We went for a while without an activities director, so the activities almost came to a halt. Since then, they've hired two people in the activities area, and so there is a pretty wide range of activities, including access to live entertainment about three or four times a month. The cost of various similar facilities here is about the same regardless of where you go. None of them are inexpensive, but at StoryPoint, they get to know not only the residents but the families like myself that they see frequently, and you can tell that many of them care for the residents as people, as well as customers.

Provider Response

Hi Norman, Thank you for sharing your experience. We’re glad to hear that you’ve noticed improvements in staffing and leadership and that you’re happy with the meals, housekeeping, and the care our team provides in getting to know both residents and their families. It’s also great to hear you're pleased that the activities have expanded with new staff, bringing more engagement to the community. We took your feedback to our team and want to clarify some information. We do not withhold medications for any reason unless directed by a doctor and a POA. In the instance you mentioned, the medication was stopped due to a stop date set by the doctor on the e-script. As soon as this was brought to our attention, we immediately addressed it and resumed administration. Regarding room checks, we conduct them throughout the day and night and notate any incidents for our records. There would not be an instance where someone is left on the floor for three to four hours, as our team is regularly checking in on residents to ensure their safety. We understand that staffing changes can be challenging, but we remain committed to maintaining a strong, compassionate team. Thank you again for your feedback—it helps us continue to improve and provide the best care possible. If you would like to discuss anything further, please connect with our Executive Director directly.

My Mom and Dad have lived at Storypoint West for 5 years. They tell everyone that they love living there, enjoy the food, amenities, the staff and activities (especially Bingo!). The building and grounds are well kept. As a family member, we couldn’t be more pleased with the care they receive. They moved in as independent residents but due to declining health issues, they transitioned to assisted living. The staff walked me through the transition as if they themselves were family members. It is comforting to know that they have my parents’ best interest in mind.

Provider Response

Hi Kathy, Thank you for sharing your heartfelt feedback! We’re so happy to hear that your mom and dad have enjoyed their time with us at StoryPoint Fort Wayne West for the past five years. It’s wonderful to know that they love the food, amenities, staff, and activities, and that they’ve felt well cared for throughout their journey. We’re especially glad to hear that the staff provided such supportive guidance during their transition to assisted living and made the process feel like family. Your peace of mind knowing that your parents' well-being is a top priority means a lot to us. Thank you for trusting us with their care and for your kind words!

SP offers offers a number of room sizes & configurations to choose from depending on the resident’s needs. My father’s room gives him just the right amount of space he needs, The staff is friendly & accommodating. They provide excellent care meeting the needs of the resident’s care plan. They always greet residents & visitors with a smile. The daily menu in the dining room is diner quality. It meets the nutritional requirements. There is an alternative menu offered to residents who would like something else at that meal. The special menu offered at holidays are exceptional with many options.

Provider Response

Hello, We're glad to hear that your father is comfortable and well cared for. We also like hearing that the team has been friendly and supportive. We appreciate your feedback and look forward to continuing to meet your family's needs!

I visited this facility

I toured StoryPoint Fort Wayne West. First of all, it is in the country, and it has a beautiful pond out front with a fountain. They also have a back area, so every room seems to have a good view. All the spaces that the staff showed me, even the one with a studio, was bigger than what my sister had had before. So, I was surprised and pleased with that, that they had larger rooms. They had a movie theater and dining room areas. It was close to lunchtime, so there were quite a few people in the dining area. I really didn't meet too many of the staff, just the lady that took me on a tour, and she was very nice and very knowledgeable. She gave me some tips on things, too. She did wonderful.

We knew very little about assisted living until a few months ago when our sister had a stroke and needed assistance. We are so pleased with StoryPoint Fort Wayne West. It is such a comfortable facility with such kind staff who are giving her the assistance she needs to function daily. We see improvements as the staff helps her. We enjoy getting to know the knowledgeable and helpful staff. They seem to enjoy their jobs and caring for the residents. The food choices are wonderful also, and there is such a variety to choose from. The rooms are cleaned regularly and very comfortable. Her laundry is done regularly and all her needs are taken care of. With her own furniture items around her it is homey and warm...not like a hospital or nursing home. We are so pleased with activities that are planned regularly and outings to restaurants, etc. We feel we made the perfect choice for our sister.

Having met with the staff for the first time I was impressed with their sincere interest in my mother's well being. It is never easy to trust the fact that others will provide the same care as you would. There was not one day I felt my mother was not being taken care of. If you are looking for that one place that could remotely substitute someones home Story Point is the place to be.

My Mom’s experience in Memory Care at StoryPoint was amazing. They worked with her to get her medicines balanced correctly and were able to greatly improve her nutrition. They also added plenty of mental and physical stimulation which all resulted in her ability to move to assisted living where today she is thriving.

I am a friend or relative of a current/past resident

My parents moved to Indiana 2 1/2 years ago. They are very happy being residents of StoryPoint West. They enjoy being able to choose activities each day and Bingo is their favorite. They also enjoy movies in the theater and occasional bus trips. Meals in the dining room have a 'restaurant' atmosphere. They say that the staff are very friendly and helpful and we, as a family feel the same. Their apartment is perfect for them and has become 'home'. One benefit that we appreciate is the Wellness Center. The medical staff is very caring and can triage or give direction if a medical situation arises. We believe that they are in a safe and caring environment.

Overall the facility is nice. There is a lot of potential at StoryPoint West. However, like many other senior living facilities they are struggling with staffing, particularly in the dining room. The service in the dining room is often very slow and the hours have been cut. The food is sometimes not great, although overall my parent enjoys it. Perhaps my greatest concern is that the emergency pendant system is "first page, first serve". They do not triage the pages. A resident could be having a heart attack or a stroke and if 3 other residents paged the staff first, it could be awhile before a staff member reaches your loved one's room. My research of other facilities has revealed that this lack of a triage process is not uncommon. This a definitely a question you should ask when vetting a senior facility. The rooms are a nice size in comparison to other facilities I visited. The staff is very friendly and helpful. They seem to have a lot of activities for the residents, but my parent rarely participates in them.

I’m just learning about senior living. I’m loving the interaction my mom is receiving & the calls I get from staff concerning her. I’m only a month into this new life for my Mom but I wish there could be more fresh fruits & vegetables. That said I do understand institutional food & maybe the difficulty ordering for the facility. Thanks for all you do.

Provider Response

Hi Randy, Thank you so much for your review. There is so much to learn about senior living, and our goal is to make that process as smooth on you and your loved ones as we possibly can. We are so happy to hear that you and your mother feel at home and that you both are receiving the attention you deserve. We do offer fresh fruits and vegetables, and if the ones available do not suit your needs, our culinary team is very open to suggestions. If you are interested in providing feedback, please reach out to our community and ask to speak with our Executive Chef. Thank you again for your kind review.

My Dad has been a resident of Story Point for four years. He likes his privacy of his own apartment. The room is cleaned once a week and they change and laundry his sheets and towels. Any problems in the apartment are handled promptly by the maintenance staff. The food is good, the staff is friendly, and all needs are met. Still waiting on an internet update and I miss the email updates that I used to receive from the Hearth. Would highly recommend Story Point.

I visited this facility

We toured StoryPoint Fort Wayne West, which was a very nice place. The thing that stands out the most about that place was the fact that they offered exercise at an exercise facility that could have helped my husband keep strong while he was there. The staff was very friendly, and the facility was very bright, very clean. The clients there were very engaging. When we came in, my husband immediately found a friend and they had a really good conversation, so I know that his social needs are being met there. They are willing to transport him to a place where he exercises currently. The staff was very friendly. The guy that showed us around was very, very nice, very accommodating, and very knowledgeable. He's had 30 years in the business and so he's had a lot of good experiences to share. The rooms were very clean, and very bright, with lots of sunshine. The place was very homey. There were a couple of fireplaces that were going, it was very clean and bright, the people were friendly and the clients were up and active and doing things.

My dad has moved into StoryPoint Fort Wayne West. He has been there for about a month now. So far, so good. His room is nice. It's a one-bedroom and a living room area. The maintenance and upkeep are so far, so good. He has been there that long. The staff members are very nice. Food is better than what he was probably doing before. I like that it's close to my sister's house. They have a social hour and bingo. He hasn't participated in anything yet, he's still too new, and he is just still trying to get used to losing my mom and everything else. The outdoor spaces are nice. The dining area is like a restaurant. It's expensive for what he's paying.

Provider Response

Brian, We appreciate your review! Thank you for taking the time to share your thoughts with us. Our goal is to ensure that our residents and families are completely satisified. If there is anything we can do for your father or your family, please do not hesitate to call us at anytime. Thank you again, StoryPoint of Ft Wayne West

I am a friend or relative of a current/past resident

StoryPoint Fort Wayne West was the first place I toured and that's the place we ended up choosing for my mom. She has already moved in and the deciding factor was, she's a smoker and there's a small area outside where residents are allowed to smoke. There's only maybe three or four of them that do, but that was what basically swayed the decision. They're in between chefs right now, so while they have the chef, the food is really good but that's a little touch and go now because they have other staff members filling in. They were doing field trips out to some places but most of their drivers just retired, so that's a little hit and miss right now too. They also have an activities director and there's like bingo going on in a day and all kinds of stuff. I think they keep them active.

We toured StoryPoint Fort Wayne West. It is very close to our home, and it is very nice. We toured the independent living and the assisted living. They have exercise rooms, a billiards room, and everyday they have group activities. There are games and different types of group activities. The staff was very nice and very helpful. They were very good and very efficient. I have not stayed here yet, but from what I saw it is a very nice facility.

My aunt is living here. It's excellent. It's clean, beautiful, and welcoming. The staff members are excellent. They seem to be very friendly and willing to please with different levels of care to accommodate the residents. The food is excellent. The dining area is facing the patio. It is very light and open with lots of window. It is very pleasins. There is a lot of room for people with wheelchairs so they're not bumping into everybody. There seems to be a lot of activities. Obviously there's Bingo, because everybody loves Bingo. The other day there was around-the-world trivia; I was very pleased with that because it's an activity that involves using your brain.

It has great care, and they do great communication with the customers. The outline on the rooms is a little larger, so you don't have that locked-in feel, and they have nurses and doctors on staff. They have different age-appropriate games like singing or cards. They try to get involved in activities that they would really enjoy. The meals are good, and they have a bus for transportation.

I visited this facility

It was very nice, very new, and seemed home-like. I liked that in the assisted living they had an actual kitchen with a stove and full-size refrigerator. If you wanted to cook, you could, and makes you feel at home. You would not have to rely on the dining room for all your meals. The staff is very knowledgeable, very friendly, and knew the answers to the questions we had. They were very good.

This is our choice for mom because the majority of our family lives in Fort Wayne area and they want her to be close to their smaller children to be able to participate in their activities. It has an excellent level of nursing care over and above the assisted living and she needs that. It is a wonderfully pleasant place. They have activities and they go on bus trips weekly. The staff is pretty impressive, very caring and shows her a lot of affection.
