My wife stayed at P A Peterson at the Citadel under memory care. They did exactly what I would think somebody would do. The gentleman there was very kind. He was very considerate of my family and my wife and you couldn't ask for a nicer guy to help you, especially at this time. He was there practically every day. He was a gentleman at the nurse's desk. He was in there constantly telling me what was going to happen and so forth, and I can't thank him so much for the comfort and understanding of the whole process. It's an older facility and there's been some updates. It was excellent that I needed somebody for skilled care because of her open wound and I wasn't finding any place that would take somebody with an open wound and then go to the long term. So, I could just leave her right there from skilled care to long-term if that was needed.

My mom stayed at PA Peterson at the Citadel for six weeks. I would take bites off my mom's plate because she didn't finish things, and the food was good. It had taste and was seasoned. The appearance of it was just like eating at home. As you walk in, it's very open. They have a friendly staff at the front desk. The facility is very beautiful and clean. The rooms are always kept clean. The staff interacts with the patients. Overall, it was just very pleasant, and it was quiet. The security is good. You have to be buzzed in, and then you have to also sign in at the front desk. The staff members were friendly. They are talkative. They answer your questions. I felt very comfortable. The communication is amazing there. The staff there would help different patients who couldn't do stuff. They were just very patient. They had jigsaw puzzles and board games. They did karaoke. It was a bit pricey but was worth it because my mom got the care she needed.

I checked out P A Peterson at the Citadel. I was pleasantly surprised that it was a very nice and clean place. It looked like they were very attentive to the patient's needs. I also thought they would help mom get more help that she needs to maybe have a better quality of life. But the cost is very expensive. The rooms were well put together. I love the fact that they have an actual church in there, which is nice. They have things that they can do for activities. The staff who showed me around was super nice.

The rooms are falling apart and smell of strong urine. The grounds look close to the same. Nursing staff swears in front of and around residents. Bathing is rarely done and complained about when requested. Any good staff leave or they seem to dismiss them? The food you can get better in the Winnebago County jail. This place needs to be shut down Period. I would rate a negative if the option were available.

I was sent to PA Peterson for rehab, and to still take IV meds. I had MSSA with sepsis. I didn't know what untill I went home. Some staff was super nice. Asia, April, and all of the rehab people. Nights and weekends are the worst. They ran out of washcloths to wipe me when I still couldn't walk well. I feel sorry for the good staff. I was left in my own feces for 2 and 3 hours. Twice. I knew I had a UTI. I would scream and yell to get attention. Takes more than 1 hours at times to answer a call. I complained to the head nurse. Some things got better. But one CNA chewed me out because I asked her to open my pop. And to wipe me. She said you are walking now you can do most of this on your own. In a snotty way. Then didn't wipe me good. They tried to tell me I couldn't go home on my IV meds and PICC line, and I had to stay there. My insurance wouldn't cover me going home. They just wanted more money from my insurance. I got a call from OSF. Saying I could go home. So I did and they pushed me out the door. Fast. It's an awful place. I heard older people moaning and complaining in the night. So sad how half of the staff is so lazy. Who knows how long they leave there regular patients. Very unhappy.

The rooms are clean and beautiful to be in All nurse's and CNA are very kind and respectful. Treat you like family. The food was very Good. I enjoyed my stay and would recommend this place to my friends an family. Very positive staff. Physical therapy I can't say enough good things about them! Keep up the great work.

The staff has been great. I am out of town and the only relative my loved one has left. I call daily to get updates. I am very understanding and I am a retired nurse. I know the demands of the nursing staff and totally understand that they cannot always be available. I do not understand and cannot accept that The phone rarely gets answered on second floor. I highly recommend that P.A. Peterson hire a Unit Aid or Unit Clerk to answer phones and take call back messages for the nursing staff. It’s unacceptable to call in the A.M. (after report) and P.M. (before report) and the phone rings endlessly! You are lucky mr rating was 3.

My mother was at this facility for rehab (from having a knee replacement). Needless to say, I would NOT suggest anyone else go there and expect to receive top-notch care. The nurses seemed very incompetent and with very little concern. Several times my mother had to wait up to 30 minutes after pulling the string for assistance. Some of the nurses would come in without introducing themselves and just say here's your medication. One nurse said, "here's your medication and then said why are you in here." Therefore, she did not read the chart! There was one day that mom didn't receive breakfast or lunch. Which was fine because I took her food daily, the food they provided was not appealing or tasteful. The list could go on but I feel horrible having to write about so many negative experiences. OT & PT were helpful. The facility definitely needs to be updated. Mom was there for a week and the board in her room was only changed once. Therefore, we didn't know some of the nurses' names & many of them didn't introduce themselves unless we asked. The was spacious. No activities or hygiene items were offered. The neighborhood is beautiful.

I went to P A Peterson Center For Health. It's not as new, the newness makes a difference in the upkeep, you can't make a place new. The staff was very encouraging, very knowledgeable, and very pleasant. I know two other people who are currently living there, and they also give them a five and tell me how nice and enjoyable their stay there is. When I visited my friend, she shared some of her food, and it was very good. Her room is very nice. It's very convenient. It's in a very safe neighborhood. They're the second cheapest place we toured.

My dad was a patient on the dementia floor and my mom became a resident on the second floor shortly after. The service both of them received was beyond amazing. My dad had nurses that gave him their full attention, spent one on one time with him and tried to make the best for him. The floor my dad was on was always clean as well as his room. The accommodations that are made for families visiting is nice. There are areas the family can sit and visit and there is also an outside balcony that my dad loved. The outside of the facility is placed in a beautiful setting across from a golf course. We were allowed to take my dad out and walk around the facility with him. My mom was included in my dad's decisions and daily living which helped her immensely with the fact my dad had dementia. The nurses and staff also took extremely good care of my mom. The staff has been nurturing, compassionate, supportive and loving. This starts at management level and continues to everyone that was involved with both of my parents. I would highly recommend PA Peterson to anyone who is looking for their family member to be treated with love, respect, and kindness.

I was a patient here for about a month. When I originally came here my room was not ready, no TV or remote, no seating, and found out my room was the “go to” for things that other rooms needed. After the first day I was all set. I was on the second floor at the time, waiting for surgical wounds to heal, for about 2 weeks, when I was moved to a suite on the first floor, where I remained until I was discharged. (Not sure if I was moved there because I was totally coherent and was aware of everything going on or if I was moved for a quiet recovery) The food I had was absolutely horrible!! I was on a restricted diet, and when broth was requested, it was the broth from a can of condensed soup with no water added, same with beef and so salty I could not consume it. Soda,juice, jello etc were completely generic and sugar free, which I do not do. Grits were palatable. The facility with dark walls and dark floors was really depressing, my family hated it. One day the elevator broke, people and the 2nd floors’ lunch were stuck for a few hours The staff was great, responsive to needs and requests, some would even visit if they were not working 1st floor. No attitudes, and every one was kind (hopefully that’s the way they always are) There were s few times that staff would come in after midnight to change dressings and what not but I ended that immediately Getting hair washed was a chore (not able to shower), but if salon was open no problem. There seemed to always be some sort activity going on in the foyer, but it was over the holidays, concerts, plays, games etc Did not appreciate religious beliefs of pastor forced when he visited. Overall my stay was okay. I did not have a choice of another facility

Since Citadel has taken over there has been big changes. Can't wait to see how the remodeling of the rooms turn out. Staff is friendly and always fix any issue I may have.

Pa peterson is an awful place its not a place to heal its a place to die. Anx not in a pleasant manner i am absolutly positive more than half of the residents who have passed in pa peterson have been caused by lack of pressence and attenfion by the cnas present my grandfather is not taken car of he hasent shaved in weeks he is fed uncolored canned boxed garbage thay makes him have to use the bathroom which the nurses take too long to respond the halls smell like urine and are not taken care of and im more than possitive some of the cnas are on pills or dope. Please take care of the ones you love share what im saying let people know this place is awful you would be better to send them off to be homeless.

My husband is there.it's horrible for the most part the food is terrible .he has to wait up to an hour a few time to go on a bed pan or come off it .he has to call me at home I need to call them to take him off of it or give it . a fewstaff members are nice and seem to care but only afew .the isolation room once was so full. Of traash it was piled up by his door .they had left his disposable transit there for two days .I would not recommend this place to anybody

This place is absolutely horrible. My grandma stayed here for a few days and hated every second of it. The staff was horrible, not very helpful. She would press her call button and it would take them 15+ minutes to answer her call. There would be people in other rooms screaming for assistance to go to the bathroom and the staff would ignore them. The hallways smelled really bad and the whole place is very dark. When it was finally time for my grandma to be dishcharged, the nurses took their sweet time and was not very helpful in explaining what was taking so long. Over all this place was very horrible.

My husband was here almost a year and died.. Cnas were on phones, telling loved ones to hold on while they did personal unrelated stuff. House doctors double charge medicare and medicaid if your not paying attention. I tell everyone and anyone not to put their loved ones here, especially the dementia and altimers floor!!! Asking any questions you get the run around from management and staff blaming either the next shift or previous shift. My one star rating is way to high. So if you are the kind who really watches over your loved one and asks a lot of questions everyone avoids you when you walk into the floor. Such a shame. DO NOT bring anyone you care about to the 3rd floor. Just can't say enough about this horrible place

I went to visit an old neighbor of mine and was very appalled at the way this place was running. Seen nurses leave meds at the lunch table for a resident and walked away and when my neighbor needed help to the bathroom I asked a cna at the nurses desk to help and she said yeah I'll be there in a minute and continued chatting. I also saw a few cnas on their phones and not paying any attention to the residents I was very shocked at the way things were being done here. No one seems to care about anything very sad to see.

My Grandmother (GG) was at PA Peterson this summer. I had heard stories about poor nursing care after the new company took over. I toured many facilities before we made our choice as to which facility my grandmother would go to. She was very ill at this time. Contrary to what we heard this facility was very clean, staff were very friendly, all the patients looked happy. So we choose this facility. My grandmother stayed there for many weeks recieving thereapy and great nursing care. I feel you can not always believe what you hear. You need to experience it for yourself. My grandmother is home now i believe because of the good care and great therapy she was able to get back home. Thank you PA Peterson!!!

I would never recommend this place, My mother was there for about 2 weeks and almost died on Christmas Eve, They called the house to say there was no therapy that day and if my Dad and sister hadnt gone up there to check on her she wouldnt be here. When they went up there within an hr of the call she was laying in the bed unresponsive and they had to ask them to call 911! Unacceptable! Never again!

We tried to look at PA Peterson in Rockford. We found it very clean and very good. There are a lot of activities. They have a place for water aerobics twice a week. It is quite nice. I'm not sure that where she would go, but it is one of the places she is considering. My observation is that it is a fairly well run place. They have a shuttle, and it is a part of the package.
