My brother was at this facility for 2 weeks the first week & half he was improving from his stroke then one day he vomited next to his chair & started talking jibberish a sign of stroke & it took them 3 days before they figured that out & sent him back to the ER!

I was a resident there for eight days a year ago, I had physical therapy, the people were very attentive, it was very clean, and the food was very good. If I ever need that type of care again I would request to go there for care.

Do not bring your family member here at this time. Disconnected care. Lack of accountability, deflection and dismissal of famy concerns. My mother developed wounds on her leg as a result of poor management of edema (an issue that did not exist prior to her admission there), no shower for 6 days despite our daily requests, and she was sent home with no wound care instruction, no wound care supplies, and another resident's information, not her own. Home health, now seeing my mom at home, also did not receive any information about my mom's wound. We attempted mutiple times to have things corrected and were met with deflection or avoidance. To this day, 3 weeks later, we have no information how the wound occurred, no apology, and a lack of concern from the corporate level. They are a for profit healthcare organization-- the for profit focus is evident. Do not bring your loved one here.

Communication was poor. And continued to be poor after we asked for information. We asked for a meeting with administration and they failed to follow through twice. Mutiple excuses, no explanation as to how a wound developed on my moms foot. It took 6 days, despite daily requests for my mom to get a shower. No one took our concerns seriously. Terrible d>scharge planning. List goes on and on. Therapies were ok. But go somewhere else until they figure out better staffing and improve care.

There is something wrong when there are no personell available to help people get into beds for the night . People ask for help and get ignored or yelled at for asking for help. Customers are told to do things for themselves. , whether or not they can . I would not leave anyone I loved here . There is no rehabilitation aspect because staff is sketchy .... Thank you .

I have a loved one here and can tell you it is by far the cleanest nursing home I have been in. The staff is so good to my mom and family. The therapy is amazing. We have been in other homes and we won't ever go anywhere else.

My wife is at this facility she’s on a low sodium diet they feed her Italian submarine sandwich for dinner I told them that she’s can’t have no salt This morning she sees a cockroach I run up there I see more cockroaches I take A picture of it the lady tells me it’s not a cockroach I sent To about 15 people they told me it’s a cockroach the lady steps on it that works there I said you just going to bring that home with you maintenance man says no it’s not a cockroach

My sister-in-law was in Alden Debes Rehabilitation & Health Care Center. Is not that fancy and is a little bit basic, but it's always very clean. The community is easy to get around, there are no steps anywhere. There's a big yard with a lot of paved walkways and little patios, and things for people in wheelchairs. They have a dining room, and a therapy room with machines and stuff to work on physical therapy. They have different wings for nursing, assisted living, Alzheimer's and hospice all right there. They take Medicare and Medicaid patients, not exclusively, so you can't expect private rooms. There are two people in a room, but it's not that crowded and the hallways are wide.

Husband is here and is non verbal and can not move is total dependent on care by staff they do not provide proper care his hands have become more contracted due to not getting the restorative therapy they say they will provide and never happens please be educated on options and don't settle for this location with frequent complaints

Where shall I begin. My mom was at St. Anne's in Rockford on two occasions. For the most part the first time she was there she seemed to be taken care of in a very caring manner. A week or two after her release she returned after having surgery on her broken ankle. My sister who basically lived at the facility was appalled at the care she was receiving. Mom's medications were found under her bedding, call lights were ignored along with other communication issues concerning meals and care. . When funds had diminished and it was time to find a Medicade facility, St Annes could not accommodate mom. Being that I, who had POA over mom was out of town, my sister looked into Alden Debes along with a number of other facilities. She thought Alden would work for mom on a short term basis until another Medicaid facility became available Upon my return my first impression of the facility was not favorable. Her side of the room was smaller than the other side, the walls were cinder block and the bathroom was shared by 4 residents. Now that you have an idea what the facility looks like, let me tell you a bit about mama Rose, she was a handful!!! She kept begging me to get her out of there her complaints were the staff abuses her, they don't feed her, they make her sleep in the basement or on the ceiling, her neighbor kicks her all night, Cleopatra jumps through the window etc,etc,etc.. Now you're getting a good picture of her mental state. At any rate I looked into Rosewood right down the street from Alden, not once but twice. The second time mom was much more stable both physically and mentally. Yet they refused to take her in because they thought she'd be a bad fit for their facility. HMMM I really didn't think a state funded facility could do that but what do I know. So long story longer here's my final thoughts on not only senior care but Alden Debes. If your the type of family that refuses to be a part of your loved ones care and figures dropping them off and maybe visiting them a couple times a year, any facility will do because you really don't give a crap. If you're the kind of family that even though your loved one has become mean, stubborn, combative, and delusional, but you still love them and are willing to be an active part of their health care and lives then you need to look past the aesthetics of this facility and talk to the folks that care for it's patients. Maybe it was because my family was so hands on with ma or maybe that's just how the caregivers treat everyone, whatever the case they treated her like family with care and respect. The caregivers from housekeeping, nursing staff, physical therapy, to social workers are absolutely wonderful you made moms final months best they could be. Thank you Carmen and family

My mother left the hospital and was transferred to Alden Debes and left 20 days later worse than she went in, They did not monitor her care she was collapsing when standing they documented low BP yet never informed nurse or doctor. She was supposed to be on a heart healthy diet, she was given what ever they served. No other choices. They told her she had to leave after her insurance days were used up even though we were aware of that and then we would have copay to stay. They have no signed documentation stating she refused care. This is the story I got after less then 24 hours are being transferred home she is back in the hospital BP 90/40 blood sugar 425 , bladder infection and BED SORES. She is still in hospital 5 days later. Do not send your family member there...

Administration doesn't know what they are doing. Staff is way overworked. Residents are cared for as much as they can be but its always understaffed.

My first thought is to stay away from this facility. The care is non existent, nursing does not listen, as well as the administrator. My Mother has gone 3 weeks with out a shower as the last documented bath was 3 weeks ago. The food is terrible. My Mother always complains about food temps, so I purchased a thermometer and temped her food one meal when I joined her and my Father. Entrée was below 100 d, vegetable was 96 d and soup was 88 d. There was no one around to talk to about the temps other than the Aides that served the food. When I called to talk to the administrator, and voiced my concerns, she asked me if I would like a list of other nursing homes in the area my parents could move to. I told her too late, had already found one. I will say that one positive about the place is that it is very clean. But don't let that fool you, the care is pathetic.

My mother was not given enough care, she became worse during her time there and the nurse practitioner indicated that for some reason she was not getting her evening medication. They provided no discharge assistance other than from what I can tell a call for a taxi and assistance into it since my mother could not walk on her own. They did not notify the family that she left the facility either. I was told that they do not provide any discharge planning when patients want to leave against their advise. Doesn't appear to be within guidelines of appropriate client care. However, I will let the Illinois Department of Public Health figure out if this was within licensing requirements. I encourage anyone unsatisfied with their services to do the same.

I had relative in this facility and was appalled at the treatment or lack of much needed medical assistance. Treatment was asked for several times but ignored This patient is now in critical care in the hospital because of a week of neglect that could have been stopped.

Incompetent management, not enough staff for the patients . Management doesn't care about the patients it's all about the money.

I am very disappointed that a nurse and nurse practitioner caring for my family member recently, became very frustrated that we wanted to talk with them about medications and living arrangements. The nurse said the nurse practitioner was too busy to talk with us for 5 minutes but will talk with us tomorrow. That is not professional or caring. We are dealing with a lot and should not feel like a burden to the staff. Physical therapy, cna's and dining room staff were very nice though. so I will give this facility a thumbs up but the 2 staff members should not ever treat patients or families the way they treated us today.

My mother's been in Alden Debes Rehabilitation for about two weeks now. I'm actually very fond of the place. It's a very large facility and they've been really good. They have lots of staff, more like a hospital atmosphere so there are lots of people available. There's a calendar of activities that is posted. They're doing a good job.
