My mom was at this facility and the workers were very compassionate and knowledgeable. My mom had a broke hip and the staff took the time to make sure that she was not always in pain and helped here whenever she called for them. Whenever I came to see my mom she always had good things to say about the staff. I would recommend this place to any one who needs therapy. The therapy staff would come to get my mom on certain days and were always promp. The building was clean as well. That was a plus for me.

My Wife was there only two days before suffering a stroke. she was taken there for rehab for a stoke that occurred in October. Before coming to Tucker, she had been at Grady and Emory. She was rushed to the Hospital on the second day. Both days, l found her lying in her bed with bowel movement all over her , the bed and on her hands. The first day, it took me 3 hours to get someone to change her. The room smelled so bad. The second day l found her unconscious. Bowel movement all over her. No nurse or staff Doctor anywhere to be seen. Two E.M.T's were in the room but had been been called for another patient. I had to make a scene just to get someone to called an ambulance. Clearly she was having a possible stroke. Blood pressure was 210/ 117 when l looked at the monitor. Still no Doctor or even a nurse would show their face. My Wife died a few days later. An " office worker" finally showed up before l left to follow the ambulance to the Hospital. Showed no compassion only gave me excuses which were worthless. Still to this date, no one from this Facility has reach out to me or to my family 3 months later. I still remember her telling me the first day how much she hated this facility. I told her let's give it a little time. How wrong could l have been. I still have nightmares about what her last night was like before she became unconscious. l do not believe they will ever go away.

After breaking her femur, our 95-year-old mother just finished a 21-day stay at TW&R Center. She was well cared for by the kind and competent staff and rehabilitation team. While the facilities could use a facelift and a better system for patients to alert staff to their needs, they were clean and well-maintained. Our mom was frankly too weak to profit from the amenities but was pleased by the tasty meals and friendly manner of all who attended to her. We would recommend the center to others who find themselves in need of similar care.

My husband has been at tucker rehab for a week and I am more than Impressed! They started working with him immediately and he is up and moving for the first time in a long time. The staff go out of their way to make you feel welcome and your loved one cared for in a way that makes you feel good The facility is clean and always smells fresh! I was worried but now I'm thankful that he's was taken to Tucker Rehab. And a big thank you to the entire staff

My mother was a respite patient at this facility. The very first night mom fell out of the bed bruising her face and leg. Mom is non-mobile and yet others used her bathroom leaving feces in the toilet because the visitor's toilet was out of service. Mom's intake paper clearly states NO PORK and yet they consistently served her pork. The nurse's station is unattended resulting in no one available to answer your calls. My personal sitter and son had to clean the room. My mom took a new wheelchair with her and the staff acknowledged the chair was there but Ellen Smith refuses to reimburse me for the chair. The list goes on and on... PLEASE do not leave your love one in the care of the staff at Tucker Wellness. I reported this facilities to the governing boards for Nursing Home Facilities.

My mother is in Tucker Wellness & Rehabilitation Center for rehab. I think the lack of communication from the staff, they don't communicate with the family, and they make decisions, that's the reason why I gave them a three. Several times I've been there, her concern was that they will bring her a tray for her to eat with no utensils and she'll ask for utensils, whether it's plastic or whatever. She'll ask for that, and it'll take them another 30 minutes after they deliver some of the other trays to bring the utensil back. It's just small little things like that. By then, she doesn't want to eat the food. So I've on several occasions brought her something over there to eat so she can eat. I'm trying to also be considerate that we're just now during the tail end of COVID. And I understand they may be somewhat short-staffed or I'm not sure what their staffing levels are, but I'm trying to take all of that into consideration. Initially, she was in quarantine when she first got there and she was in a private room, and after the quarantine, they moved her to another room where it was with three other individuals. So there were four people in a room, the wall where the door is, it's kind of bashed in. So they weren't taking care of that. The baseboards, I don't even know whether it's mold, or what it is, but some of this stuff is stripped off, the paint is peeling. It's not what we expected it to be. This is the first time we've had to experience being in a rehab facility like that. I think it played a part in my mom being discouraged and feeling like she's not very comfortable there. They do have a calendar schedule of activities, but the activities require her to have to physically be out there in the common areas to participate. So a lot of the stuff, we bring to her, you know, like word search puzzles and stuff like that for her to keep her mind going. I think they have a lot of nice things to do. They do bingo. They do movie nights. I feel like they do have ample things that the person can do.

My father was transferred to this rehab center on a Monday and as of Friday we still haven’t seen him. We was told they had to do an assessment and it would be a couple days before we could see him. My mother was scheduled to see him today however when she arrived she was told that he was gone to dialysis. (They Lied he never got his dialysis treatment). This place is horrible with communication, don’t bring your love one here. It’s very hard to reach anyone besides the Receptionist that transfer your calls.

My father is in Grace Healthcare of Tucker for rehab. They lost his clothes and they have patients in the hallway by the nurse's station so they're able to keep an eye on them. All the two bedrooms were taken. He is in a room for four people, which they could do better with. He eats the food, but he doesn't enjoy it. They have a TV in the dining area. They're doing the best they can, but it's not good enough.

I am a relative of one of the residents that used to live there. I was told that they had bed sores, well what they had was pressure sores. Two holes in each side of their body that you could see through to the bones. I feel that they did not take care of my family member. The staff does not care at all. They are lazy and do not help at all. I would always have to drink water for my relative when I would come to visit them. I would not bring a loved one the this facility at all. I caught one of the staff members feeding a patient that could not feed themselves and pushing the food into their mouth when they were not ready. I would not like for my family member to be treated this way. This place has done downhill since the new woman administrator took over. The nursing staff could care less when you ask them a question about your loved one. Please do not take your loved one to this facility. I learned the hard way.

WORSE PLACE. Don't let them fool you. Nursing staff is awful. Call lights stay on with no response. Bed sores on family member. staff turn over is constant. No follow up on patient needs or appointments. Rooms barely cleaned. I can not say one good thing about the care my family gets. Food is terrible, salty and gross. Please consider other places...Unfortunately. This place is better than some!!

I was a patient at Grace Healthcare . I can't speak highly enough about the Physical Therapist and Occupational Therapist. At Grace Healthcare. They are knowledgeable and driven to provide each person under there guidance a healthy and compete recovery.. They will go the extra mile to make sure the resident they work with has the tools to overcome whatever the issue is that has them in rehab.. The nursing staff on the other hand needs to be replaced and start from scratch. The nurses act as if they are afraid they will do more work or work harder than another. No communication and always the same lame excuse' The patients do not get medication on time when they do it has been known to be the wrong dosage.. When new residents arrive the nurses are suppose to enter the medication into the patients chart and make the pharmacy aware of any meds the patient needs and dosage' these things happen days after they need to.and the excuse is always the same. They faxed it over or they called the pharmacy. and at first i believed them but then it started happening to others as well. point is. The nurses are lazy and love to grip and moan about the least little thing they do.. i can personally tell you this happened to me. This is not third party . I am speaking about things i have actually witnessed and affected me personally. i had a wound that needed to be dressed once a day it was twice a week and started getting worse because it was sitting in the drainage that was coming from it. i can go on but i will stop here. Best advice would be to find a place other than Grace to send your loved one. Sad because the Pt and Ot are awesome here

Grace Healthcare is a Excellent facility!!! My mother received rehab there for 6 months and she made a great recovery! The staff were all very loving and caring to all her needs.... they listened and were always there to talk. The admin staff were also very attentive and returned phone calls promptly, and they were very understanding and caring to the needs and demands of our mother. My mother and my siblings and I are very satisfied with the great, professional, care she received while in there care. Thank you Grace Healthcare for taking such good care of our mother!!

This facility worked for me. I came in weak, I was able to gain strength and confidence in my walking, standing and sitting. I thank the whole therapy staff for working hard for me because I came in the place with a bad attitude. But they put me at ease and made working fun! I thank everybody at the facility for helping me. I am able to go home now. Ms. E. Gleaton

My husband has had 3 different therapists since he has been here at Grace for the past 13+ months. And overall he has had good care although I would like to see more intense therapy. Now I admit my husband is hard to work with and that is a challenge for the therapists, but I just would like to see more intense therapy. The facilities are good and well kept, its always clean.

When I first came in here. I came from another facility. They did not do a good job with my knee (I had a knee replacement), so my knee froze up and I couldn't move it or bend it. When I came to Grace, I was scared and in a lot of pain. It hurt to bend it and I didn't want anyone to touch it. But when my family brought me here to Grace, they made me feel welcomed and put me at ease. The Nursing staff treated me beautifully, and the Rehab staff made me feel like I was their long lost family member! They were wonderful! My orthopedic doctor wanted me to get aggressive therapy, and the people in therapy worked me hard, and made sure that they took care of me and my knee. They even went to my house with me to see how I would do at home! I can now fully bend my knee, and I am no longer in pain, and I am now so happy and I am able go back home.

DO NOT BRING YOUR RELATIVES HERE!!! This place needs to be shut down. The nurses have bad attitudes, they never can answer basic questions about my mother patients, I came to visit my mom and she had a knot on her head and her knee. when I ask the nurses and cna ther stated they didn't know what had happen and said no one reported it and left it at that. there are so many horrible things wrong with this facility til I cant tell it all. but I do recommend looking somewhere else cause my family has been very unsatisfied with their services.

This facility is poorly managed, staff ratio is not appropriate to residents. Personell go and come when ever causing services not to be delivered timely. Administrative or nursing personnel cannot be easily accessed by phone or in person. Management is not responsive to relative caregivers. Presence of a physician was not noted after 15 days of stay. My Son fell from a wheel chair after being pushed into the hallway following a PT session. Overall, this is a "holding" tank for the physically and cognitively impaired. Attitude of lead staff, i.e. social worker, is nothing close to customer friendly. She is down right rude.
