I am a friend or relative of a current/past resident

My mom is currently in the Life Care Center of Gwinnett where she's getting therapy right now. She's there to get help with strengthening her leg muscles. She's working with a physical therapist, so they have her doing all kinds of exercises. They're very helpful. My mom's room is modest, but it's very clean. I have not tried the food, but she likes it. For amenities, I don't know about a library, but I have seen a small courtyard off the dining room. The staff members are excellent, though they could use a few upgrades.

The staff at Life Care Center of Gwinnett were friendly. At first, they kept me informed on what progress my dad was making and then when we tried to get him moved to another place, they stopped informing me altogether and I couldn't get enough information out of them to get a discharge date. They didn't tell me he had COVID at all. We walked in down there, put all the PPE clothing and stuff to go into his room and then some of the people who were trying to come to assess him, knowing that we were going to move him somewhere, didn't tell them that he had COVID until they got there. One would say he no longer has COVID and then the next time you hear he has it. It was just conflicting information all the time or no information at all. The room was just really dirty. There was a roach bug in the corner of his room, a big one because you could see it. It stayed there. I cleaned it up myself on my second or third visit. There was a lot of trash and stuff. The person that had been in the room beside him had the trash all in there and the bed was never made. I even told housing that they needed to clean up, and there was stuff in the toilet that had been left behind by somebody else. To me, it was just dirty.

Rehab wing is barely adequate. They DIDN’T really rehab our relative and they ended up becoming a long term resident. Because of COVID we couldn’t find another place. Facilities are old and marginally maintained. Some staff are wonderful, some are just collecting a paycheck. The food is more miss than hit! Many families provide extra food for residents to keep in their rooms when the food is inedible. Laundry tends to lose their clothing (which is clearly labeled) and someone else’s ends up in their room. The activities dept staff are very nice and try to keep residents engaged in anything to lighten the mood of being in a nursing home. Choices for this level of care are limited. So families have to be very observant and advocate for their loved ones. Most staff are NOT vaccinated so they have had to quarantine occasionally as staff test positive. Families can only see their loved ones through the window or FaceTime. Then they test all residents every 3 days. It’s uncomfortable and frightening for the residents but as I said…there’s no place else to go.

My mother was transferred here after hip surgery. My mother has dementia. She fell at this place in less than 12 hours of being admitted. She was admitted in the hospital after they finally decided to take her to the emergency room. She had fractured C2 in her neck, fracture in lower back and 2 broken ribs. I called and left a message for the director of nursing. They never called me back to this day. For a fall like that I'm sure it was pure neglect. Would not leave my worst enemy at this place.

I wish I had known how evil and in compassionate the nurses and techs were. They were all rude and lacked empathy. I am ashamed that I admitted my mother for rehabilitation, after suffering a stroke. My mother has to call me just to get help! No one would check on her for hours! The physical therapists were Amazing! The weekend staff were evil! My mother dropped the phone. We were not able to contact her. She asked the nurse or tech if he or she could give her the phone. The individual told her to shut up and just eat her breakfast and walked out the room!! This is the level disrespect that my mom suffered. I can go into more detail, yet just know...I called the police to help me retrieve my mother’s belongings, the nurses told the police they did not know me, and they did not want to be liable giving my mother’s belongings to me, yet I have power of attorney. If I can save a patient and their loved ones from this evil place, I will continue to SCREAM! Do NOT TORTURE YOUR LOVED ONES!!! I can only PRAY DESPERATELY THAT THID PLACE WILL SHUT DOWN, OR FIRE ALL THE NURSES AND TECHS! GIVE EMPLOYMENT TO LOVING AND CARING CARE GIVERS! Lastly, to The Life Center of Gwinnett, KARMA IS COMING FOR EACH AND EVERYONE OF YOU!!!

My mother, Sara Padgett is a resident of this facility and has been since 2006. She was diagnosed with pneumonia on Thursday 12 26 2019. I am pleased the facility maintained her without sending out to hospital. This nursing home took excellent care of my mother. With their help my mother's temp has gone down and she is responding to the antibiotics. The nurses are so pleasant and were so kind to my mother and me. I am so glad the facility did what was needed and my mother is improving. I thank all that were involved in my mother's care. I thank you so much. Robert Padgett

Do not send your loved ones there. Administration and the social worker cover things up. Injuries happed due to neglect and they call the family instead of 911 and talk you into talking the patients to the hospital because they do not want any injuries recorded. They release patients no matter their condition. They just keep them the allotted number of days and then disregard their condition. They send patients home in worse condition than they get there in. Patients who are not able to talk of care for themselves are left in the hall alone unattended. They are left in poop and pee until a family member cones and says something then they try and cover it up by saying we were just getting to the patients mean time it was over an HR and 50 mins and no one was there helping. Life Care Center of Gwinnett was wonderful. My mother was there for rehab. The food was very good. The room was very nice. Everything was very clean and functional. When I went to visit her, she was already done with her activities for the day, but I know they did physical therapy with her. It is worth paying if you have the money, but very few people have that kind of money.

Rooms small. Nurses nice . Food horrible. My mother got very ill while in their care and had to be taken to hospital , I think they should have caught the problem before it got so bad, I had to go to social worker and tell then she wasn't right. They did rehab with my mother until the insurance ran out then gave me 2 days notice before her discharge, LOL 2 days!! Of course she could stay until I found a place for her but she had to pay 290.00 a DAY!! out of pocket. The rehab refused to work with her until her discharge. I needed a wheel chair and a lift for her, The social worker was suppose to order it 3days before the discharge date . Its been 15 days and still no wheel chair. When you call the party you need to speak with no one ever answers the phone and will not return your call when you leave a message, I would not recommend this facility.

They are very nice when you go on the tour. The facility needs a huge update. Rooms are mostly small doubles, drawers are broken, not enough room for a wheelchair to move around, and that is all they give residents to use. Room had a leaking air conditioner and mold on the ceiling in the bathroom. They finally fixed those issues after asking three times. They quit giving my mom her medication that she was on that can not be just stopped. The side effects of not taking the meds is dizziness. She fell and hit her head, had to be rushed to ER and get 4 staples in her head. Once we found out they had not been giving her the medication no one would call us back. We took her out and took her home. We followed procedure they have set up on getting her medication records, (in writing and giving them 24 hours) and to this day have not gotten them. They said their records department deleted the order for the medication and that is why she did not receive them for over 2 weeks. Most of the employees are nice and caring but short handed. Many days there was people begging to be taken to the bathroom, screaming for help and they would walk by the room and do nothing. They would rather them go to the bathroom in their diapers than take them to the toilet. I would not recommend this place to anyone.

Life Care Center of Gwinnet is a great place. The rooms are OK; most of them are double. I am in a single room, in isolation, and this room has a twin bathroom. It is very nice.

I visited this facility

This past December I had my mother at Life Care of Gwinnett in their rehabilitation wing. I found the staff to be very professional, caring and concerned about their patients. They communicated with me constantly about my mothers condition, and I was happy with 9 out of 10 of the personnel that I encountered there. The one factor that prevented me from giving the center a 5 star rating was the physical condition of the center. In the rooms that I visited, I saw many pieces furniture damaged, holes in the walls and things in disrepair. This caused me to pass up on the facility in my initial consideration of rehab centers. However the professionalism of staff that cared for my mother made up for it.

My mother is in Life Care Center. It's nice, and they love her. They're caring, and my mom is happy. It's a hospital-like environment. I don't have any complaints about it. They don't have apartments. The rooms have four walls, a bathroom, a TV, and a bed. My mother is comfortable. It's a decent place for what she needs. It's not luxurious, but it's not supposed to be luxurious because it's a hospital. I don't have anything negative to say about them.

My Mother has been in Life Care Center for 5 years now. We have had our ups and downs. The rooms are rather run down and in need of repair and just a basic face lift. The food is a constant complaint of my Mother's. It is a balanced meal but there is a lack of variety and mostly pretty bland tasting. Most of the nurses and CNA's are nice and helpful. I have had a couple of problems with billing. They charged for things that I did not ok and that Medicare did not approve. It was resolved but do watch for billing mistakes. They try to have activities regularly and Mom likes her Bingo. A big shout of praise to CJ her nurse, Sherlyn, Sarah, and Margaret for taking such good care of her.

So far the service is adequate at Life Care. They seem to know what’s going on and they’re attentive to her needs. They are at this point pureeing her food, and, in fact, I need to have a talk with them, but she seems to be eating and she doesn’t have a problem with it. The facility is very clean, a little bit more homey, one-on-one, and for me right now I want her to have a specialized care but not a hospital-like feel. I don’t want to say anything negative because they are a center that specializes in more intense care than where she was at a senior living facility, and that’s different because it’s a little bit more homey and one-on-one. I’m a little biased, but I don’t think anything's wrong with the facility. It’s just that I’m looking for something different for her.

I am a friend or relative of a resident

Lifecare Center is only good for people who are not difficult. They have a good track record of helping people that need physical therapy. They're dining room is wonderful. They have a nice beauty parlor. Although I wasn't really impress with all the nurses there. I wasn't happy with the doctor on staff there. I wouldn't want her to be my doctor at all. The director of nursing was sweet to me, but she really didn't help me in terms of trying to get my mother to stay there until she was better. They don't know much about physical therapy. The place was clean, it looks nice when you come in, but I just wasn't happy with what happened to my mother. Some things were overlooked. She got sick and they didn't even know why she was sick. They need to have a little more caring. They need to hire more CNAs, so the residents will have proper care.

I visited this facility

The facility was very nice and clean. It had a nice dining room and offered decent food options. The residents seemed well cared for and my grandmother was treated well by the staff. The main issue I had with this facility is that my grandmother had a roommate that watched T.V. at top volume. The T.V. was on all hours of the day and night. The roommate was never told to turn the T.V. down by any of the staff. We complained to the nurses about the T.V. and the roommate would turn it down, but that would only last while we were visiting. Once we left the T.V. went right back to maximum volume and the staff did not ask her to turn it down. We had a meeting with management regarding the issue and was told they had no place to move my grandmother. I was disapointed that this situation was not resolved and my grandmother had to spend her last months bedridden listening to a television blasting all day and night without anyone coming to her aid.

Initially, what happened was my mother was in another facility. That facility had an accident and the roof collapsed in the dining room. So they had to move all their patients to other nearby facilities until they get the roof fixed. So they had moved my mother to Life Care and when we went there to visit her, I find the facility nice. The nurses are really good. The doctors are great. My mother just liked it there. So we didn't move her back to to the other facility. She had to option to stay or go back. I asked her and she said she wants to stay so that's why she is still there now. She is sharing room with someone there. My mother is an old Latin lady and she is very picky when it comes to food. She doesn't like American food. No matter what food she eats, if it's nice rice and beans with some enchiladas, she's not gonna like it. I've seen the food served to her and they do have a well balanced meal.They also figure out the patients needs when it comes to food. My mother does not like to eat red meat so they are making sure that she is served chicken or fish and food like that nature. I really don't want to take away my Mom from there but it's so far away from where I live since we just moved to Cleveland, GA this year and it takes me almost 45 minutes to get there so we are thinking of moving her near us soon.
