My spouse moved into the Lake City Nursing & Rehab Center. They are the only ones who called me when she had to be transferred from the hospital, and so far they are doing good. The staff is very helpful, nice, and very respectful. The facility is nice too. It seems to be very clean so far. I haven't seen any bugs and rats, but they could use a little paint here and there. She's only been there recently, so she hasn't been there long enough, and I'm giving them a little more room to grow.

I wish I could -100 stars. This establishment needs tp be shut down. This is the “facility” from HELL! I had a sibling who stayed here for a few weeks and they treated her horribly and barely tended to her needs. The staff treats guests like they are animals and need to be fired at once. So unprofessional and unaware of new lifted Covid-19 restrictions.

My mother had an infection and no one bothered to contact the medical staff for her 5 day stay. They returned her with someone elses cbelongings while her belongings were not fully returned She was not the same person when they returned her.

We chose Lake City Nursing & Rehab Center. The staff is very attentive. I haven't tried the food, but grandmother seems to like it. It looks like it's in pretty good condition. It seems very positive and welcoming. Her room is small with a private bathroom and a single bed. There's a diner, a hair salon, a courtyard, and there's an activities room. They have an activities director.

Understaffed, staffed with teenagers in the afternoon as young as 15, no assistance moving in or moving out, the senior staff who are the most highly paid are the least helpful with the exception of the actual LPN shift nurses. Choose another facility.

I am a long time resident of this facility. They have taken very good care of us during the virus, making sure we are safe. Staff are friendly and caring. They help me talk to my family that I can't see because of the virus.

The foyer is painted and is very inviting. The lunchroom is a big selling point and leads to a great catfish. The portion of the facility where the clients are kept is smelly and I almost started to gag. The soiled linen needs to be thrown away right away. You can't capture stank in pictures. The staff was friendly. No interactions with the residents.

It was like being on vacation! The staff and therapists were excellent and provided great care. My physician was more than pleased with the progress I had made with my rehabilitation after surgery!

Lake City Nursing and Rehab has a caring environment where everyone appears to be family rather than a resident. The facility is clean, safe and welcomed family feedback. Staff are knowledgeable of residents needs by providing customized care.

the worst elderly home ever , they lied to my family saying they would move my grandma room,, it took them about 2 weeks to move her, she fell out of the wheelchair , also her nurse left her in the bed naked all day with the AC on, my grandma is 86 years old and has Alzheimer's it was 4:00 in the pm she was wet in the bed naked , so happy we popped up that day, and we put her in the car and brought her home

She didn't get enough to eat, and her clothes were stolen here in Lake City Nursing. They mixed up her medication too. After 3 months, they said that we had to pay extra. Neither she nor I could afford the increase. There were three or four nurses that really took care of her. She had a bed sore on her behind, and they cured that up real quick. They had a team of doctors and nurses that just took care of it. They are good. As far as the caring goes, those particular people are good. But somebody got her medicine mixed up. We had to rush her to the hospital.

My grandmother was in here for such a short time, only a day and then she died. Very concerned about this facility. She has respiratory problems and they failed to notify my mom in a timely manner. They said she had been dead for a little while.

Lake City Nursing needs to improve on everything. Everything is sugarcoated. The staff are sugarcoated too. They said they have activities. But if the patient refused to do them, they don't make them do it. I wasn't very pleased with it. I just don't like it.

My brother was admitted in 12/11-he now has stage 4 bedsores; need I say more?
