This place is a joke! No one ever answers the phone, and all voicemail boxes are full except the administrator who never calls you back, staff is hard to locate even when you are there in the building, and trying to get any paperwork or documents is impossible. I don't know how this place is even open. Good luck trying to get anyone to actually do their job. Fayetteville Center for Nursing needs to be shut down or completely revamped with staff that actually cares about the patients and knows how to do their job correctly!

I would give them zero stars if allowed. My brother was there for physical rehabilation. During his stay he fell out of the wheelchair because he was not sat back in it far enough. Busted his head and sent to hospital. Came back broke his foot being transferred from wheelchair to toilet with assistance. X-rayed three different times with no break. I took him by to a outside orthopedic and his heel IS broke with possible Achilles tendon tear. Went back to facility. I went to visit on the evening of Oct. 31st and he could hardly breath and barely responsive. I had a ambulance take him to hospital. He had CO2 poisoning. Was in ICU and step down ICU for 5 days and currently in a regular room for 7 days. Nobody cared or did anything except had him on oxygen. They never called the family. They said they ordered x-ray that moring but it was 5 pm when I got there and it has not been done. There for Physical Rehabilation not medical problems. Only 72.

My mother is in Fayetteville Center for rehab. I'm not happy with that. They stopped or attempted to stop physical therapy on my mom and said she wasn't making progress. And she went from being totally in the bed to standing and attempting to move. There's no communication with the caseworkers and social workers. The nurse practitioner. Joan has been the only saving grace at that place. She needs consistency.

MY MOM IS IN THERE AND HERE WING/FLOOR HAS NO PHONE!!! SO IF THERE IS AN EMERGENCY THEN WHAT? EVERY TIME I DO SPEAK TO HER SHE IS CRYING!!! SHE IS NOT GETTING ANY REHAB FOR HER FRACTURED AND SURGICALLY REPAIRED HIP & ROD. SHE SEEMS MORE CONFUSED AND THE STAFF NEVER REPLIES AND THEY HAVE NO IDEA EVEN WHAT'S WRONG WITH HER!!! MY MOM SAYS SHE GETS THE SAME THING TO EAT, SO SHE DOESN'T EAT!!!! SHE HAS BED SORES ALREADY AND IT'S JUST BEEN A WEEK!!! HORRIBLE!!!

The rooms in skilled with rehab are nice and have updated built in furniture. Very bright big windows and a caring very polite staff. This place stood out to us for these reasons. We are very happy with our choice.

My mom is at Fayetteville Center. The facility itself is very nice. It's all on one level, which is good. We were looking for something that was located pretty close to our home. When we got in there, it was the people that attracted us to it even more because they're very outgoing and friendly. Everyone smiles and talks to you when you walk in the door. It just feels like a family, which is nice. The room is more of a mid-century modern home. The rooms are not cluttered. The furniture could be updated a little bit, especially the hospital beds, but everything is clean. They take good care of cleaning. The rooms are big. My mom is in a semi-private room right now, and there's plenty of room in there for the two people who are in there. They have their own storage area and nightstands. They have their bed and a chair. Each one of them has their own walker and wheelchair as well, so I would say the rooms are very nice. My mom seems to like the food. They have a menu that they give to each one of them. It shows what they're having for the whole week, and my mom gets excited about what she has. She's been able to eat just about everything they give her. They ask her what her dislikes are, and they do have alternative menus so that if she doesn't want what they're serving, they'll give her something different. The activity room is nice, and a lot of people do utilize it. Everyone that I have come into contact with seems very satisfied with it. I have not seen any issues or anything like that, so I think it's a good atmosphere.

Food isn't you 5 star dining but neither is school food. The food is nutritious and right for the person, just may not be seasoned to your normal liking or made the way your use to. But food is served 3 times a day and is hot. Staff...all wonderful, never had an issue with anyone there. All very nice and loving to patients.

This place doesn't even deserve one star. Do not I repeat Do not take your loved on here. My grandmother was very mistreated she fell out of bed but the staff and manager informed us she was lying but then sent her to the hospital for a fall. The staff is very rude on the phone when the actually answer and I was informed are very rude to the patients. They are getting cussed at and informed they have no sympathy for them to stop crying. This is the worst experience not to mention we brought stuff to her that they never opened or gave to her and now a nice portable charger is missing. This place deserves to be closed down.

The facility has done good job during these trying times. The facilities have been clean according to my resident brother. The staff is always helpful and courteous when discussing care for my brother. AND - communication about Covid status has been OUTSTANDING, with calls daily when needed. (this was not the case under the previous ownership, who didn’t return calls even in pre- Covid times). In regards to food, my brother, a picky eater, is satisfied with it. No complaints! After I gave my brother a walker for Christmas, the staff worked with him to ensure he was able to use it properly. I have quarterly phone meetings with staff on his care. Activities are obviously limited during these Covid times. But my brother is still in good spirits because of the staff treating him nicely. As my brother’s guardian I MUST be attuned to this facility. The new Condor ownership has made improvements in all categories of care and I am glad my brother is under their care.

My wife stayed at Concordia Nursing and Rehabilitation Center – Lafayette. It was not very good. None of the people that worked there were very helpful and not particularly nice. They do not try to make you feel good about your loved ones being there. This is just a job to them; all they care about is their paycheck. I could not go in, but I could look through the windowpanes, but most of the time they had the blinds closed. I took a trip out there twice a day, 40 miles, and tried to look through the windows, but I couldn't because they had the blinds closed. The building was old and not well cared for. I would say avoid this place at all costs. I did not have a choice with my wife, she was placed there by her social worker, and they said it was the only place available in the area.

My mother was mistreated by this rehabilitation center. Do not put your loved ones in this facility. We were mislead and it took me several hours to get my mother out today.

If you want your love one cared for respected please I repeat do not put your love in there my love one is know longer with us he died 2 months ago being cared for in that facility. The workers some not all are rude inconsiderate of feelings of the loved ones family members just a terrible place I repeat do not put your love one there and if they are there now take them out now right now!!!!

Several concerns on various levels regarding staff inattentiveness, condescending manner when questioned about meds, equipment not working properly, incorrect meal delivered and general in humane treatment. Very disappointed with overall experience since they were recommended quite favorably by the hospital they came from. And even after a site visit before admission, the experience has been nothing like the pitch given by staff when reviewing the facility.

My advice is to stay away from this place. Staff is not very professional. Rehab personnel are good but others do every thing they can to avoid taking responsibility for anything.

I was impressed with what I read on this website about Kindred, unfortunately our reality was quite different. The first sign should have been that there was no orientation for me as the wife of my husband who was being admitted for rehab. I got the answers to my initial questions only after I agresdively pursued a meeting with the rehab manager, who was three days delayed in responding to me. A health care facility is only a effective and efficient as the staff it hires. I found the personnel on our wing to be aloof and incompetent. They must learn to speak professionally and be proactive in dealing with patents needs. We were very disappointed because what we expected was only an illusion. After only 2weeks, my husband had a crisis that needed immediate attention and after five hours there was no response. The part-time. Nurse practitioner tried, but the resources to help him were just not available. We had to rush him to a nearby hospital for emergency help and he as ineed in need of acute care medical expertise. We will not return to Kindred nor will we recommend this rehab facility to anyone!

Parking was good, and the lobby was pretty OK at Kindred. The room in the nursing home side was very good, but when she was on the other side, it wasn’t as good. She did not like the food. It facility was OK, but it wasn’t like it was just like one level. The people were friendly and very forthcoming. They have persons to do her hair or take them out. They have a schedule of activities. The facility was older though, but it was a pretty decent dining area. They have wonderful bird cages, but when you come in the door, it doesn’t seemed like anybody there to greet you.

We chose Kindred Transitional Care and Rehab for my mom. The place is very clean. She is in a shared room, but it’s fine and clean. They have a rehab program and the doctors that were assigned to her are doing wonders. They helped my mom straighten out and give her the right medications. Overall, I am very pleased.

It is my wish to write a wonderful review for this faciliy, but I cannot. This is not a rehabilitation center in my opinion. The care received in this facility seems to be basically nursing home care and not very good care as a nursing home either. I have had to constantly question them about therapy and care. If you are considering this facilty for the rehabilitation of a loved one, don't. Take them somewhere else!
