Azalea Trace is really old. They don't have anywhere for a person to go outside, so they're inside all the time. My brother said the food isn't good. The staff seem to be OK. I saw their calendar and the only activity I remember is bingo. My impression of it isn't really good. It looks dark in the dining area. He had his meals mostly in his room and was there for a short period of time.

The facility is clean inside and out and free of garbage. The staff seems very well trained and sincerely cares about their patients. Seems like a great team to care for family members in need of rehab and assistance.

Azalea Trace Nursing Home is very clean. My mother is in a semi-private room. She has therapy every day. She likes being there, she likes the nurses and she also likes the food. In fact, she put on about 16 pounds since she's been there. Everything has been fine. I'm more than satisfied with their services. If there's a problem or if she needs something, they call me.

My uncle is currently a resident at Azalea Trace in Columbus Ga. My uncle suffered a massive stroke on April 1 2011. He was placed in Azalea Trace two months later. My uncle has no health insurance therefor rehabilitation was not offered by the nursing home or the hospital at anytime. Due to the stroke my uncle can no longer walk or stand on his own. His mental capabilites seem to be affected greatly as well. After the family found a rehabilitation center that was willing to come to the nursing home to evaulte my uncle and give the family feedback as to if he can be rehabilitated, the nursing home proceeded to tell the rehabilitation center there was no need to send anyone because my uncle will not progress, he has peaked. I thought that decision was up to the rehab physicians and not the nursing home. The fact that they told the rehab representatvies not to come was totally out of place in my opinion. Since he was placed in this nursing home his siblings, espicially my mom, have had several arguments and meetings with the staff about several tidious things that were not provided for my uncle. Tidious things like not bathing him, not changing his diaper, his food being out of reach or cold, and his roommate constantly keeping him from sleeping. My uncle recently suffered a urinary tract infection while at the nursing home because they were not changing his catheter at least every 72 hours, which is standard code for the state of Georgia. This tract infection was so bad that he was rushed to the hospital because his blood pressure had dropped to almost 40/20. After this my uncle's siblings (6) had a meeting with the staff in which they denied any wrong doing. He was placed back in this same nursing home several weeks later. We, as his family, visit him everyday because we are afraid that the staff is not doing what is needed. I honestly can say that everytime a family member visits my uncle there is something to complain about. My uncle is sleeping on a bed with no sheets, the smell of the entire nursing home is terrible as well as the food. When we bring these things to the attention of the staff, they always have some type of answer as to why his standard of care is not up to par. I pray for my uncle everyday and night that he can continue to keep his spirits high and understand that we as his family are trying to get him help but time is money in the current health care system, and money is something we do not have a lot of.Thanks

This facility had a great atmosphere. Gave me a great feeling of placing my loved one there. Azalea provides an array of neccessary things needed for each individual. I went there and found out they have majority of their services onsite and whatever is not onsite, they have the resources for you to find it offsite. Most of their onsite services include but not limited to: Physician, Pharmacy, Speech/Language Pathology, Social Services, Physical Therapy, Occupational, Activities, Dietary services and more. It gave me a wonderful feeling of knowing my loved one wont have to be transported back and forth off of the facilities headquarters.
