I am a friend or relative of a current/past resident

Everyday my loved one is here are some of the most stressful days of my life. They lied and said the transferring hospital didn’t give them what they needed to know to care for her..Claimed she wasn't given orders for certain medications but when we checked with the hospital they showed the orders were sent MULTIPLE times and when the doctors office inquired they were told shes already been receiving the medication. They are rude everytime she asks for help, if she even gets an answer to her nurse call button. Everytime im there it takes 3-5 times asking for a need to be met before she gets what she needs. Lost test results (multiple times), "forgotten" medication, one night she had a sore that bled all night bc her bandage came off and the nurse just cut the call light off. Had to be cauterized the next day to stop the bleeding. They don’t have rails on the bed, bed alarms or any other way to prevent falls. The rooms are set up in a way that makes moving around dangerous. There is feces and who knows what else on the walls, furniture and floor. Paint is peeling from walls, mold around windows. When you walk down the hall the smell of feces and urine is enough to gag you. She has been dazed and confused multiple times when i came and gasping for air bc her oxygen tank was either empty or not turned on. They were allowing an unauthorized family member to make decisions and have information despite having a copy of her power of attorney paperwork. The insurance/rehab policy seems to make it Impossible for us to move her to another place without forfeiting her benefits..I want to let someone else know how horrible this place is before they get “trapped” here for rehab. Don’t do it...find somewhere else...look in other cities...drive further if you have to. Whatever you do please don’t put your loved one here for nursing or rehab. I'm terrified we won't get her out before it's too late.

Do NOT send your loved ones here. The admin is awful…specifically the DON. They are too busy playing on their phones to care about the staff and the patients needs. The place is run down, the a/c and heaters are always breaking. There are any and bug infestations. I literally have a pic on my phone that has a bed pan full of feces that took over 8 hours to be cleaned. They constantly ran out of patient meds. They would only staff 1 nurse and 1-2 cna’s per every 30 patients. It was hell for my family member there.

My father is in rehab at Carrollton Nursing and Rehabilitation Center. We had his care coordination meeting yesterday, and we're pleased with the director of the therapy department participating in that meeting along with a nurse focused on medication and another nurse focused on safety. My dad has a large room that would normally fit three people, but due to COVID, there are only two, so he has more room than usual. He is a large man, and they have accommodated him with a large bed that is more comfortable for him. During the coordination meeting yesterday, the speech pathologist was there, and she had already identified a couple of issues with them only serving and providing a plastic spoon instead of a metal knife and fork. They are making adjustments to meet his needs. It is an older facility. You can tell it's not new, but it has no worrisome condition. I like the focused attention from the therapy environment. My dad is allergic to tea, but they brought him tea and didn't replace it.

The nine days my grandfather was here were some of the most stressful days of my life. He was transferred here for rehab after a car accident. The first day he was there for 4 hours and had a terrible fall. They lied and said the transferring hospital didn’t give them what they needed to know to care for him...They were rude after his fall and basically said they didn’t have rails on the bed, bed alarms or any other way to prevent this from happening. He fell 2 more times before we were finally able to get him moved to a more suitable place. The insurance/rehab policy almost made it Impossible for us to move him to another place without forfeiting our benefits..I want to let someone else know how horrible this place is before they get “trapped” here for rehab. Don’t do it...find somewhere else...look in other cities...drive further if you have to. Whatever you do please don’t put your loved one here for nursing or rehab.

I am a friend or relative of a current/past resident

My mom was at Carrollton Nursing & Rehabilitation Center. Their staff was quite helpful there. Their dining room was very nice. There were several places for him to go to, and they had different areas for the patients. I think they have bingo, and I think they would have a day where they would make things. They had church on Sundays, and somebody would come out and hold a church service. I liked that. We were very pleased.

They need to watch the patients more closely there is no communication between the shift employees. My mom was so weak they put her in a wheelchair and let her fall out I wouldnot recommend this place for anyone.

I am/was a resident of this facility

I was there for 14 days and it was a delightful experience! The nurses are very caring they see that you have what you need asap. The CNA' are very helpful there there as soon as you turn your light on. Even the food was very good . I'm completely satisfied with my stay there ! They have the best physical therapy group they try in every way to meet your needs! Thank you Carrollton Nursing and Rehab for giving me a new outlook on nursing homes!

Carrollton Nursing and Rehab's staff were very friendly and wonderful. First off, they're helping me get my mother-in-law Medicaid approved. They call all the time for anything that happened with her, little or big. The food is good. The place and the dining room are very clean. Outsiders are not allowed in the dining room. The only drawback with Carrollton is size of the rooms. They are not spacious enough and they are shared with another resident.

I am a friend or relative of a resident

Mom had an infection on her toe. For eight months, Carrollton Nursing and Rehab didn't address the issue, so it got infected to the bone. And my mother's toe had to be removed. Because of that, she would need assistance getting up going to the bathroom. She goes to a dialysis three times a week. But they wanted to put a diaper on her. My mother could not use those things. One time when I went up there, I was surprised to find my mother in the bathroom. She said she had been pressing the buzzer but nobody would come. I think they need to improve on how they could keep a constant watch of their residents.
