It is not easy to have a loved one in the hospital during a pandemic. You often worry about if they are being treated right or if they are lonely. My uncle's stay at Signature was no different. I am delighted to say that this was the perfect place for him to be. He said that the people here were very nice and treated him like family. He also said that the food was very tasty. Mrs. June at the front desk was also very friendly to us. Thank you for everything.

checked myself out of this facility after 2 days. Management is the problem. The facillity is old (my room wasn't renovated. the room I was in had a bed with controls that was from the 70's or 80's by the design and rust on the railiings and the furniture was outdated too. I had them take the chair out it smelled like gramma's house when she could no longer keep up cleaning. The staff tried their best, but there are not enough of them and the facility messed up by not having my medications, one of which I needed daily for me for more than a day and a half. That's when I left (called a LYFT). The one and only time the doctor came in instead of telling me about my rehab, she was more interested in probing family questions and I realized later that she was trying to see if I had family (if your familly member is especially vulnerable I would avoid this place like the plague). The intake person also tried to give me an injection saying it was a 2 part TB shot required with the second shot given 8 days later. (yeah right). I am ex-military and knew that was a lie. I informed her that I would _not_ be taking any injections without signed documentation on what it was and why is was required and she backed down. I never saw her again after that. I lived in the area and it was not far from Piedmont Hospital were I was discharged from. I admit if I had checked for the reviews first I never would have chosen this facility. Stay far, far away. They have 2 stars or less ratings elsewhere.

On 5/22/21 noon, I arrived to Signature Healthcare of Buckead to visit my husband who suffers with Lewy Body, and who was admitted for rehab 5/20. I noticed the door was ½ way open and I found him shaking, naked, & wrapped with toilet paper. He smelled like he had done number two. I think this is the reason he was left on a chair like being punished in a degrading/humiliating/undigniifed position. I took pictures & a video of the bathroom which did not have grab bars. I went to the nurse station which was only a few feet away from the room and I asked to speak with the manager in charge. A nurse told me that the manager of the floor was out but that she was in charge. I told this nurse that I wanted to take my husband home that I have found him alone and naked. She did not act surprised, she did not apologize, she gave me the paperwork & I went back to the room complete the paperwork. I believe I completed wrong because when she came to pick up the paperwork she said she would bring another one and that a nurse will come to dress my husband. After she left the room, I decided I would record on my cell the conversation so I would have a record of the incident. I have a recording of my conversation with the two nurses. Unbelievable what the employees said about the facility, such as “we only work here”; “short-handed”, etc. The nurses did not even know how to care for my husband, how to clean him, how to dress him; they couldn’t help him to the toilet, etc. All this can be heard on the voice recording I took. I stayed calm. I have heard so many horrible stories that I was thinking if I say anything they will kick me out and I will never see my husband again, or they will do more harm to him. My goal was to get him out of that horrible place ASAP. My husband is worst than when I took him. The damage done to him mentally will take a long time to recuperate. Signature of Buckhead needs to be closed.

I have been getting reports regarding staffing. After speaking with Nursing Dir, Kim I was assured the staffing was adequate. On 7/26/21, I was advised there were no nurses on the night shift; my loved one was threatened by another patient's aggressive behavior. The behaviors were caused by lack of staff, patients not being giving evening medications. When my loved tried contacting nurses station, there was no to answer. On 7/27/21 - tried contacting the Nursing Director, the voicemail is full... This is ridiculous and I suggest not to place your loved ones in this facility!!!

I was recently discharged from this facility after a month and would not recommend it to anyone. The best part of my stay was the PT and OT, they were top notch. They are professionals. The management and staff there overall is poor. They do have a few CPN's and CNA's that are great (Christina, Aysha, Lynnise, Theresa, Ty, Doneyshia) they are a few that really care. Overall, this facility does not care enough about patient care or their employees that's why staffing is so poor. My insurance company, through my employer, recommended them for the therepy, but otherwise won't be recommending anyone else.

How many negative adjectives can we use to describe the deplorable conditions this place forces residents to live in? Horrible, dirty, neglectful, infested with insects.... how has the place not been shut down? There are never any staff, ever. When you call your greeted with the rudest people, the Nurse manager that refers to herself as a doctor is one of the most abrasive people I’ve ever come in contact with and there is no management in sight. My only positive interaction was a few weeks ago with my moms PT and OT who took the time to call and update me after her evaluation. Even after that I was greeted with rudeness from their manager, the social workers and other nurses while trying to understand what was happening with my mother. If you can avoid it, do not send your loved ones here.

Although the facility is located in Buckhead, do not get fooled by the location or the facility name. The care for long term residents is deplorable. Residents are often left unattended and majority of the medical staff appear to be more engaged in idle conversations than patient care. If you are are way from the facility and calling to get an update on your loved one, you are lucky if anyone answers the phone where your loved one is housed. This is extremely frustrating and the staff turnover is unbelievable!!! Nothing about this place is signature.

This facility is a disgrace. The rooms are poorly cleaned. On various occasions my family has to come in and clean feces from my uncle's bed, curtains, trash can and bed rails. We have witnessed trash on the floor for days at a time. Window sills full of dead bugs. The staff members are extremely rude and careless. When asked for assistance, they promise to return in a few minutes, which usually takes a few hours, if they come back at all. Our loved one fell and was taken to the ER with a fractured jaw and other minor injuries and no one ever notified us. We found out upon arriving for our visit The 2nd time he fell, we called to check on him and was told that he fell hours prior. The 3rd time he fell at 5:15 AM and we were notified around 930 am. When trying to contact supervisors or the regional manager, they wont call you back. Even after meeting with them and them ensuring us that things would get better and even gave us their personal cellphone numbers, but we we're still unable to reach them nor would they return the phone call after leaving several messages.

Please do not send your friend or family member here! My mother is a patient and it took over an hour and a 1/2 for someone to come and change her. Then, when someone came, they gave me a hard time because they were the only person there. I have spoken to the PA there, for tests to be done and a week later, they have not been done. The food is horrible and always comes cold. Meds are given and, when asked what the pills were, we were told "her morning meds." What if she was giving her the wrong pills? So unprofessional and just plain lazy!

Signature Healthcare is a pretty facility but the patient care is horrible. My family member would call me crying because no one answer the call light when she needed help. I would not recommend anyone to go through this.

At Signature HealthCARE, it seemed that the room had initially been constructed for one person, but they attempted to put two persons in a room. My friend was against the wall, and there was nowhere for her visitors to sit when the door was opened, as it would it would hit her bed; it was just really bad in terms of the living space. In terms of the lobby and open spaces, it was nice, and the parking was great. Everything was great except the room, and I saw that she was in the corner with no space to put her belongings, being interrupted by the door if the nurse came in -- it was really terrible. It appeared to be clean. I went there during lunch and dinner, and the dining area was OK.

My grandfather spent some time there and there were no problems. The staff was very nice and accommodating. He never had to wait very long for them to respond to his needs and they always had a good attitude and smiles on there faces.

Not happy with this place. My dad contracted C Diff while here and had to return to the hospital via the ER. He said the food was not at all good. I know every night when I called (from out of state), it took no less that 3 tries to get the right room. Later I learned there is an automated number you can use and you can enter the room number in and the right bed.

My best friend was a resident at this facility about a year ago and she just recently returned she is a total hip replacement her first time there was wonderful. The staff were friendly and very helpful when she first arrived she was greeted by a concierge that went up to her room with her along with the EMT she made sure she was comfortable proceeded to get her familiar with the room she the left the room and got her nurse the nurse came down and introduced her self and made sure all of her needs were met overall the customer service was great the food was delicious as well they gave me 5 free what they call meal tickets so i could dine with my friend the first time she was there they did not have a customer experience director i believe that's what she called her self at least i didn't remember her from the last time but she did assure me that if had any concerns to come to her she also provieded me with a cell phone number to reach her directly that really put me at ease just to know she extended her cell number to me just made me feel my best friend was in good hands i would definitely recommend this facility to my love ones

I was admitted to a semi-private room on the 3rd floor at around 9:00pm on 11/14/14. The nurse introduced himself and advised he would be in to do a body check. This was not done until 2:00am. I could not walk to the toilet so I needed to use a bedpan. This was emptied into the toilet and rinsed in the face basin as there was no sink in the toilet. My room-mate had her personal items around the sink, needless to say they got sprayed with water from the bedpan. Pain medication that was ordered did not arrive until late on Saturday. When the nurse heard my room-mate telling me how unhappy she was at that place she loudly stated " Don't listen to her, I knew she was going to say that she has no right tell you that she is just mean" I was dissatisfied with the care I was receiving and asked for an administrator on duty. I was told there was none but I was moved to a private room even though I had no problem being with a room-mate. By the way the room was too small to appropriately accommodate 2 beds. Nurses took their own sweet time in responding to requests for pain medication. Several times I waited for more than 20 minutes after asking for pain medication, and on one occasion I asked the CNA to let the nurse know I needed pain medication which she did After waiting 55 minutes I walked to the nurses station only to be told the specific pain med. which was ordered much earlier in the evening had not been delivered. Now the nurse knew this but did not think it necessary to let me know an hour earlier. That is the type of treatment one can expect at this facility. Needless to say that particular pain medication was never delivered. A complication of the surgery I head is blood clots, needless to say no nurse checked to see if anything was developing. When I did notice redness and swelling and asked a therapist to report it to the nurse the response was "Oh that is from the surgery" However the nurse did come to see and an antibiotic was ordered. No other nurse followed through to see if the area was extending, or for any changes. The noise level at night was atrocious between loud talking and shouting in the hallway, loud laughter and the heavy duty cleaning machine, I felt sorry for those persons who must live there permanently. Those old people have no voice. In addition the food which was not so good ran out daily and so did the linen.. The therapy staff was on the money but it is the nursing staff that makes or breaks a facility and although the CNA,s worked very hard and did their best this place is surely BROKEN

This is my fathers second time at this location after having surgery. The first time was pretty good. The only complaints were the fact that it took some of the nurses forever to come to the room after being called. Also it took about two weeks for me to get a cot for his room so I would be able to sleep comfortably. The food wasn't great but it was good enough. This second time here on the other hand is terrible so far and its only the first day. First I was told my father hadn't arrived although the van that dropped him off was pulling off. The lady at the desk told me he hadn't even been picked up. It was soon verified that he had been dropped off and I was able to go see him. A nice lady name Tina, I believe, was quite helpful when getting us settled in the room until his Nurse and CNA arrived. It took about three hours before someone came in the room to check on him. Vitals weren't taken nor did he receive any form of medication. His vitals weren't taken until around 8 that evening. My father is a diabetic and hadn't received an insulin shot nor any medication since about 9 that morning at the hospital. We were told that the pharmacy had not filled the prescription. Apparently the prescription had not been filled until about 10pm that same night. Is that normal? For that to happen to a patient who just had surgery and/or who is a diabetic? The nurse seemed not to care at all. She was quite rude and quite condescending. I had to go get my fathers medicine from home so he would have something to take. It makes no sense that this had happened. We kept receiving the run around about everything. My father had enjoyed his stay there last year and had begun to recommend it to everyone. Never again.

I am not at all satisfied with the care my loved one received at this facility. Staff were not available to assist in getting this handicapped person out of bed & to the bathroom. Nor did they timely respond to calls for assistance. My friend was admitted with multiple conditions that cause diarrhea and was left laying in her soiled bed on multiple occasions, once for 2 hours. Friends were having to buy pads for the bed and bring them to the facility because staff claimed they had run out. Hard to believe. My perception is that they are understaffed and that the nursing and CNAs they have have the bad attitudes that come with that staffing level. Even more troubling, my friend went without the prescribed antibiotic for a hard to get rid of infection for two days, told that it was not in the pharmacy. Plus, while she was there for rehab, she went two days with no OT or PT.

Mom used to be at Signature HealthCare and the staff was good. Her room was very spacious. She had a sitting area and a wood bed with railings. The food was also good and they had a couple of outings.

My mother was there for rehab after surgery. The looks are deceiving. The Physical Therapy services were great. The nursing staff is horrible and uncaring and may seem to be untrained. There were no meaningful activities for residents and the staff was largely unattentive. Call bells are not answered most times because the staff doesn't want to be bothered. I would never take my mom there again and would not recommend it to anyone for any reason other than out patient physical therapy. Check other sites for similar reviews. Do not bring anyone you care about to this place

My introduction to this facility came when my husband needed a facility for rehab after an extended hospital stay. A representative actually walked into his hospital room to introduce the facility to me. I told her I was not interested in her sales pitch. Nevertheless, she continued to tell me about the facility. Because it was only three blocks away, I accompanied her for a tour. From the outside the facility looked like a small office building. Interior decoration was pleasant with subdued colors, but I was struck by the extremely low ceilings.All of the staff were neatly dressed and pleasant. But the common areas did not seem welcoming. Patient rooms were small. Hall were narrow.Dining facilities seemed crowded. I was introduced to the therapists and shown the workout space. The sales rep showed me a small outdoor "garden" that was being developed. She told me about the regular"Wine and Cheese" event to which families were invited. I was assured I was not restricted to short visiting hours. Still, there had been nothing that said I was looking at a place where I wanted my husband to spend however long was required. I saw only mediocrity. I also called the director the next day to tell her I did not appreciate the intrusive salesperson.

My first visit to this facility was far less impressive than I expected. Several staff members (at least 4)were standing at the reception desk engaged in what sounded like personal conversation and not one acknowledged my presence. I waited a few moments for their conversation to stop however I ended up interrupting in order to find out where to go to speak to someone about registering my father who had been admitted earlier in that day. It has now been a week and I still am not impressed. Thus far I have found the staff is inattentive, many demonstrating lack professionalism, inadequate communication among staff and patient, and no hot water in the resident room. I am hoping this is an off week at the facility, but I do not think I would recommend at this point.

While visiting this facility, I would have to say that I did not notice any activities going on with any of the residents here. I did not see any patients on the floor unless they were in wheelchairs by the nursing station. I would say that the facility was nice and clean, and the nursing staff were pleasant when spoken to. In my opinion, I just did not see anything that made this place feel like a place to call a home for a family member.
