I am a friend or relative of a current/past resident

Please DO NOT send your loved ones to this location! My Stepdad was sent here for rehabilitation. He was placed in a room with no tv, rust everywhere, broken toilet, and his bed was next to a closet filled with trash, mold, and Lord knows what else. He was covered in piss the next day when my mom came to visit. Keep in mind he can’t walk, and doesn’t have bed rails on his bed. This is clearly unacceptable. If you cannot provide a clean and friendly environment this is not the business for you! People are trusting that they loved ones will be taking care of and not placed in a jail cell like room with no real care. I feel so sorry for anyone who is there that doesn’t have family, I can’t imagine the hell and pain they feel. My prayers are with you! I need to find out if this is government funded and do they know about the conditions this facility is in?????? I cried after seeing these pictures and hearing the pain in my moms voice from the way she was treated by some of the staff. Please understand this is not the place for any human being! It smells of urine and looks and smell of contamination. I know this is a last resort for many as it was for us but the love we have for him would not allow us to leave him in there staring at the ceiling and in tears.

I would not put a dead dog in this place, if my loved one had not their right mind, they would of been given the wrong medication several times, some of the staff can not wear gloves, due to long fingernails, the place is filthy, roaches, & the staff is not trained in the titles they wear. This place needs to be condemned, food totally improper for nutrition/diabetics. Patient had to lay in feces for over 3 hours. Upon discharged, they could not let family pick up patient, due to Medicare, then at 9pm decide they can't find transportation, so family picked patient up after 11pm. Really? What is wrong with you people? No respect, no compassion, no wound care on weekends, went days without meds. If I could give zero star, that is what they deserve.

I visited this facility

My husband was there for 2 months The food was horrible for my husband is diabetic and allergic to tree nuts and was told when he got there and they tried to give them to him after being told and one day when I got there I found he was given them and was also feed lots of non diabetic foods the staff ignored call light (when it worked) all the time the bed was broken. They provide no tv or other things for people who can’t get out of there rooms to do. Complain if asked to help with anything there were a couple of nice nurses but most were rude and didn’t want to talk to me about my husband care the billing lady was rude when I was trying to keep him there longer so I could arrange a place for him they stopped rehab when he didn’t in 1 month fully recover then said he would never improve (he is still improving and has more movement than when he left) getting his underwear changed was like pulling teeth the room always smelled of urine DONT SEND YOU FAMILY MEMBER THERE!!!

My mom was at Fulton Center for 3 months. It was horrible; they're horrible. There were broken equipments, broken beds, broken call lights, and unresponsive staff. The people were OK. However, they just let them watch TV and sing some songs and that's it. There was no getting out; it was horrible. They need to get more staff, they need to update the rooms, they need to have more room checks for their patients and they need to make sure all call lights are working.

This place has recently changed management and while yes, the furniture is new in the communal areas, the rooms are awful. They try to mask the pee smell with air fresheners but it almost knocks you in your feet. The patient is in an adult diaper and it is always sopping wet. This person is never dressed in clothes because they always go “missing”. The patients don’t get phones or TVs in their rooms so they are forced to lie in bored misery until a visitor arrives. There’s no where for the guests to enjoy being outside aside from sitting in the drive aisle. The food looks like something they would serve in jail. The call buttons go unanswered for what seems like forever and the staff can be hit or miss. Places like this take advantage of the poor and it’s gross. Aside from random bingo games there’s no entertainment. There are no amenities to speak of. The guests who are mobile enough just roam the halls all day completely aimlessly. Absolutely would never recommend this place.

My aunt is at the Fulton Center for Rehabilitation. Everyone seems nice and attentive to their needs. They have a nice one-story building with residents of different ages. It's clean, and they have people giving therapy and making sure that the patients' needs are met. She's being fed by the nurses. They have a rehabilitation area and a nice big dining area.

I am a friend or relative of resident

1st of all I don't even want to give this nursing home any stars because it doesn't deserve any. My name is Shaunita and my mom is and Fox Glove Nursing home. I Didn't know that it was under a different name before until one day I was looking up another place to put my mom @ and then I found out this nursing home has been investigate for alot of wrong doing. Well I know that the staff is ridiculous all of them but The woman @the front desk Ms. Patricia and the admission woman for got her name but them are the only 2 that has a HEART! ! The other staff is their just for a pay check. They will walk pass the residents like they are not even there they don't answer call lights they will walk right pass them like they don't even see them on. My mom has had problems after problems. They just keep sitting a bucket of water in front of her to bath herself in she has real bad arthritis in her hand they don't bath them nor change them my mom is always wet and smelly. The staff is always on their cell phones and I have reported it and they still be on them cell phones. The food is really really bad we have to take my mom some food up there. I can keep going on in on about this nursing home but it would take me @ least 2 days to write. But this nursing home needs to be shut down, closed for good, investigated to the fullest. PLEASE PLEASE PLEASE!!!! Don't take your family member to this nursing home you will regret it.Thank you.

I am a friend or relative of resident

I have never been so disappointed in a facility as I am in fox glove on Springdale rd. The staffing has always been so sparse, the care is below standards. A lot of the staff is too young to understand the elderly and their's and their family needs. The counselor is non responsive to the needs of the family in caring for their loved one. I hate going there and hate worse when there is a problem and you can get no one to respond to you. The smell is overpowering when you walk in the door. Please understand, there are truly some kind and caring staff at the facility, but, so few. There is one in the offfice in particular that has given our family nightmares for years. She is constantly making decisions contrary to the family's wishes and my parent's benefit. I have found out many of the things through other sources there. I understand the facility is again changing it's name and management. My family's prayer is the bad ones get weeded out and more good ones replace them so we can work hand in hand for the best care of my parent. I would like to suggest you send out a survey so you can get to know what we think of the staff and their performance. Thanks for listening
