Heartland of Zephyrhills is very clean, and they are very loving towards the patients. At first, my father went into rehab, but he is going to be there for long-term care. He is on a diet where everything is ground up, but the food seems to be fine. He is not eating a lot, but then he doesn’t usually eat a lot anyway. The staff is wonderful. The nurses and aides are very caring, willing to help, and do what needs to be done. I have seen them interact with him and other patients, and they have all the patience in the world. He has dementia and Parkinson’s, and it is not easy interacting with him or other people who’ve had a stroke. They are very kind, and we are very happy with them.

This is ment as an advisory to those who are thinking of sending a love one or being admitted to this facility…..DON’T!!! My mom was admitted here and within LESS THAN 24 HOURS had to be removed because she was LEFT ON THE FLOOR crying out for help FOR THREE HOURS, when she fell out of her bed at 12:30 A.M!!! While visiting her yesterday, hours after being admitted, I heard a patient continually crying out for help. So I went to investigate why. I WITNESSED staff, within earshot in the hallway, totally ignoring the patient crying out for assistance. The patient simply wanted someone to help her put on her slippers!!! It would appear that these are not exceptions, but the norm! My Aunt requested to speak to the Administrator of this establishment, but she did not present herself. My mom was not the only patient to leave this facility today. Another family also decided to remove their relative…except this one involved the Cops!! I wouldn't let my dog stay there!! Don’t say you weren’t warned!!

The only good thing I can say about this place is the physical therapist and occupational therapist are very good. The place reeks of urine. They dont wet clean the floors very often and should be done every day. When you push the button for help they may or may not come and if they do, it won't be for a very long time. I would never put a loved one in there unless you can be there every day watching what's going on and advocating for them. I had to speak with the administration daily. It's very sad for the residents that don't have anyone watching over them. The food is some of the worst I've seen. Sad

I don’t care how many stars they have on line do not place your family member in this facility! Dirty, smells of urine, my mothers bathroom stunk so bad my mom almost got sick from the smell, mold around the toilet rim like it had not be cleaned in years, floor in the bathroom was coming up, if that’s not a fall risk for a rehab patient I don’t know what is. CNA changed my mom’s roommates undergarment and just left it laying in the floor. My mom said that poor woman asked for water in the night they said ok and NEVER BROUGHT HER WATER! RUDE NURSES AND CNA’S, all and all this place is disgusting. Mean to patients when loved ones are not their. Neglected to care for my mother and a lab called to say my mothers blood level was low asked why he didn’t call nurses station said he had been trying he couldn’t wait needed to tell someone she was critical, when I asked what the nurse planned on doing she said we have limited resources due to the weekend. 🤔Now to the food, first night there I have no clue what was brought for her to eat when the CNA came she said you not hungry I said first of all what is it, she said, I said would you eat it she shook her head and said no I would not with her nose scrunched up, so I said to her then what makes you think my mom will? Took the tray and never offered anything else! When the ambulance came to get my mother whose blood level had dropped to a critical state, the nurse didn’t even know what room she was in I had to tell them. THIS PLACE IS PITIFUL! I wouldn’t even let my dog go there if needed!

I choose this facility because of the 5 star rating, I should have done more research on what it takes to earn this rating. This facility on paper is great in reality it was an awful. My mother was there for short term rehabilitation. She came directly from hospital setting, None if the orders for follow up with specialists doctors were provided even though they were ordered. I asked several times had she been seen by certain doctors. The nurses would just say the doctor hasn't made his rounds yet. I spoke with the social worker she was very rude and unprofessional. She stated that I needed to be proactive in my mothers care and I should have asked for the specialist as soon as my mother arrived to their facility. If they are ordered why do I have to ask for them? She went round and round with me continuing to lecture me about being proactive in her care. She stated that the psychiatrists doctor only has to come in every 30 days and that this is why my mother wasn't seen this is only one example. She was not at all helpful or considerate to my mothers needs. Upon being discharged they did not consult with me on the items she needed for home care but asked my mother whom clearly was not stable and was being dismissed because of this. Not one time did the social worker call. The nursing staff gave me hand written dates of appointments to follow up on, and even they were wrong! I showed up to an appointment on the wrong day and time. How hard is it to write the correct time and date down. On a positive note about the facility the CNA's are excellent and care personally about the patients.The aides are very caring and willing to help and do whatever needs to be done. Many times when I can to visit my mother was placed outside of her door in a wheel chair just sitting in the hallway. This would have been alright if she were mobile enough to wheel the wheel chair around. She is not due to a stroke her left side is paralyzed. After breakfast, lunch and dinner they are placed outside the room to wait. After she received therapy she was placed in the hallway as well. Their is no common area besides the dining hall or patio outside. Everyday my mother was there she ate dinner in her room alone because the staff said no one wanted to go in the dining hall. My mother was not asked. I could go on and on about my mothers stay here but I won't I just wanted to state that this facility is not what is shows on paper. I believe you should look at other facilities for your loved ones. Heartland failed our expectations. I gave them 2 stars on because of the CNA's they tried their very best to make her stay comfortable. If I could only rate them the rating would be much different.

My Mother-in-law is at Heartland and it is a wonderful place. The staff is so helpful and professional. The facility is clean and I am so happy. Thanks Staff for all you do.

My husband has been in long-term care at Heartland almost two years. I have been so grateful for the office personnel and medical staff. From the beginning they took time to explain everything, and help with any resources I needed. The CNA's are excellent and care personally about the patients. One example that happened recently: my husband's roommate was in the last moments of life, and as his breathing slowed, a CNA came and sat with him, asked if she could pray with him, and sing for him. It was early in the morning, and no family members were able to get there yet. It left quite an impression on my husband, and I shared the incident with the man's wife. She broke down in tears, and said how grateful she was to everyone. Even the cleaning personnel take time to talk with patients and will get help for them if needed. Nursing staff is excellent also. Once-in-a-while a substitute nurse is on the floor, and they aren't always as friendly, however, they are taking care of medicine distributions etc. and have little time to get to know the patients as individuals, unless they are filling in for a longer time and get the routines down. I can't fault them, since they are excellent nurses from what I can tell, and their first priority is to correctly distribute medicines etc. They will all take a moment for a family member that has specific questions. I always heard horror stories about specific nursing homes over the years (my sister even worked at one as a RN and she even told me some of the home's problems where she worked). I know God prepared the way for an opening at Heartland on the day we needed it and from the beginning I have been blessed with the care and understanding everyone there seems to have. From the receptionist to the head personnel, I'm always greeted with a smile, and have been surprised that most of the staff know my name, and the patients names. They go the extra mile. They have a wonderful activities director, and staff, who always are looking for ways to reach out to the patients and include them in the different activities. For patients who are unable to do much to be involved they take the time to talk one-on-one with patients, trying to find activities they might be able to involve themselves. From what I've observed the meals are tasty and varied. Obviously, not all patients eat all the food due to physical limitations, but those that need personal care and feeding are taken care of with patience and love. There is a nice courtyard (fenced in) with open and covered areas. Those who enjoy the outside are outside at every opportunity they have. Heartland also has a rehab section, and I have heard good things about that area of the home also. Overall, I have been very impressed with Heartland Nursing Home and would give them an excellent rating.

My mother is at Heartland nursing home and she has been there for six years. It is a clean facility. Everybody was very cordial. They are attentive and helpful. The rooms are too small. The food smells good and its fine. They got two dining rooms.
