My sister and her partner were at St. Augustine Health & Rehabilitation Center. I would not recommend it. They were in the same room. It's a combination nursing home, and rehab center. It was a little rundown. The room was very tight in terms of having any guests in your room. There was no place to sit. The room was small for two and it was dark. It wasn't well lit. It's just an older-style facility. It's got a number of years on it. I think my sister got adequate care there. In terms of the nursing care, I thought the care was OK, but I don't think that they were equipped to handle the type of rehabilitation that my sister needs. While they clearly can do some rehabilitation, I think she needs to be in a place where people are more qualified and the equipment is more extensive. They do physical therapy there, but my sister is in a state where she needs some major rehabilitation. She can't walk. She's a heavy person. She's a two-person lift, so she's a little difficult to manage. They have movies, and they get together in a social room. My sister's partner did leave his room to go eat in the dining room. He did partake in some religious services. I was in the room doing some work when they were doing the movies for the seniors. I don't think the food was exceptional. My sister was eating very minimally. Her partner was eating perfectly fine. He didn't complain to me about it. I didn't hear anything negative. I didn't hear anything positive. The reason why I would give it a three-star is, that it is more about the age of the facility, the darkness, and the small rooms. It's very difficult for you to have visitors. It just needs renovation.

St. Augustine Rehab was great. It was clean. My dad was there for a while, probably 3 months. Everyone was nice. The staff was very good, and the nurses were excellent. It wasn't far from where he lives, so it was nice. They took him to doctor appointments. Everything was nice. The only negative of it was he would need his own room. There were plenty of activities, but dad is not that social. They had bingo and pizza night; they have an activity calendar. I didn't see anything wrong with that place.

My husband has been in St. Augustine Health and Rehab since February, and he's doing fine. They have moved from rehab to skilled nursing and the care there is excellent. We're very pleased. The staff is wonderful, and I am very pleased from admissions down to the aides who work with them. The place is very clean, and the food appears to be good; my husband is eating well. It's a new building, but they're constantly working and redoing things, so I'm very comfortable. The rehabilitation part is the best he's had since he had a stroke in 2009.

My mother is in St. Augustine. The staff was very attentive. I wish they allow the patients to be on their own to see what they can or can't do. The administration was fabulous, and the nurses were good.

My father was in this facility for 25 days. The therapy he received was good but nothing else was good. The response times for everything was poor. My father was left on a bed pan for an hour and a half, left with spilled food and drinks all over him for an hour, many of his clothes were missing...not a personable staff except for the therapists.

I was recently a resident in rehab after total knee replacement. I can not say enough about the wonderful care I got while in this facility. Day 2 we had to be evacuated to Ocala due to hurricane Matthew. Day 5 we were returned to St Augustine. With all the confusion and extra work this staff had to do i was amazed at how cheerfully they did it. The care was efficient and timely and respectful. Meals were good and the facility was clean. Therapy was cheerful and almost fun. When it was time to leave I was a little sad because I felt like the staff were my friends! I would definitely return here if needed. Thank you to the great staff of St Augustine Health and Rehab!!

My father went to this facility right out of surgery at the insistence of the managing dr. We went to visit him and found that they were serving him dry pizza. He was unable to wear his dentures at the time. He suffered terribly there for two days until we hired an ambulance to take him out. He died in the hospital the next day

I stayed at this nursing home from about April 13th until about may 6th 2011. I was staying here for rehab after sugery on my back that affected my walking. The office staff was very good at getting me into the facility and worked with me when I was able to leave to get me into an outpatient rehab program.During my stay the nurses gave good care. The amenities in this facility were not very good. There was not an area for patients to walk or ride in their wheel chairs except in the halls. They had and outside smoking area but if you did not smoke it was not a nice place to sit.Alson the rooms were small and not arranged well for people in wheelchairs. I was always hitting the wall or having to ask my room mate to move his things so I could get out the door. I would hate to have stayed there a long time but as I was there for rehab I was able to leave as soon as I could walk withna walker. The rehab department did a good job helping me to recover my walking ability. They were very good at their jobs. I would recomend it for rehab but not long term.
