I was recently released from Moultrie. Knowing I was going to be there for considerable time I had a problem accepting it. But by the time I was discharged, I had become very comfortable in the facility. This was partly due to the wonderful nursing staff. So many wonderful nurses. The physical therapy that I had was outstanding. Beth was my primary physical therapist. She is very smart and informative of the process she was putting me on. Dorothy was my occupational therapist and supported my treatment that Beth recommended The whole physical therapy department worked so well together. Wonderful therapists. I only have good recollections of my stay, that initially I feared.. Thank you to everybody who helped me.. can

My mother was at Moultrie Creek Nursing and Rehab Center. She was there for about a month. It's full of roaches. The food was not terrible, but because we found out that they had so many roaches, you can only imagine what they're serving in the kitchen. They had one nurse who was cussing my mother. I pulled my mother out and brought her home. It was that bad. There was nothing about them that was worth anything. The rehab was horrible. They didn't do anything at all. Some of the nurses were okay, but there were a lot of them that had a very bad attitude. The place was nasty and not clean. The roaches were swarming. I did not know the roaches were that bad because I went in the daytime. And then I went late in the evening around 6:00, and I looked down and they were swarming around my mom's bed. I'm talking about hundreds of them. Regarding amenities, they have a rehab, outdoor spaces, a little ice cream where they could go down and get ice cream cones, and a beauty parlor. But they need to improve on how they treat the people. They don't want to take them to the bathroom when they buzz for them, but then they get mad when they use the bathroom in their britches.

The facility is old, not very well maintained and filthy. The assisted doors were broken for a long time before they were fixed. The serving tables in the rooms are rarely cleaned. Alarms ring constantly because the staff is very slow at responding to call buttons. Constantly short staffed especially weekends and holidays. The staff are constantly on their cell phones when they should be watching, feeding and caring for residents.

This facility is a very dirty place! In order to get the toilet fixed in my room, I had to call the head honcho of the facility, and only then did the maintenance man bring in a new toilet! I was having to pull the plunger inside the tank -- a patient doing this?? Rooms are not cleaned as they should be! I did not have problems with the staff members! The food I woul rate at a 3! Physical Therapy was the most disorganized I have seen! They played musical stools in the center of the patients (like a circle)! The beds were hard and uncomfortable that caused much problems with the severe pain in my left leg! Had to take tylenol every 4 hrs while there until I was able to come home and have not done so since! They had an ice cream bar which was pleasant! The 6 weeks I was there was not all that pleasant! But Physical Therapy was a very bad experience! i got bronchitis twice while there! First time Dr Patal put me on a nebulizer and within 2 weeks! Patal is not a pulmonary doctor, only a cardioogist! I asked for a pulmonary doctor and he would not provide! I have come home with this bronchitis and even my nebulizer has not helped -- I continue to cough up gobs and gobs of crap out of my bronchioles! I have a nurse practition coming in on Wed, and see what she has to say about this ongoing saga!

The facility was clean and staff was friendly. Food was good, the dietary staff brought me different food choices when I asked.

I feel like Moultrie Creek Nursing and Rehab Center doesn't communicate well. They're not forthcoming with a lot of information. The room my uncle has is just adequate (although it's more like a hospital room situation). They have bingo, puzzles, and other basic activities. Some of the staff are very good, while most of them are very unsympathetic. The food is horrible. To me, it's very basic and very bland. They don't cater to any of the diabetic foods that you may require. It's one menu that fits all. My uncle likes it though.

My family member has been here for almost four weeks recovering from surgery and has progressively gotten worse and worse. From leaving them on a bedpan for upwards to three hours, to being angry with her when she had diarrhea, to even almost giving her two wrong doses of a blood thinner medication that could have been toxic, they have not cared for her. And when another family member asked for assistance on her behalf the care got worse and even less frequent, as if to punish her for needing medical assistance. Her bed sores have gotten infected and they argue over who has to treat them. Very disappointing.

My sister has been here for three years and the care is amazing especially during this difficult time of Covid. They stay on top of everything and they provide the best care possible. I would come here myself if I needed to be somewhere other than home. Highly recommend this facility.

his has to be THE worst Nursing & Rehab Center in the World! If you want your family member to die take them here. My Mom was treated so very poorly and disrespected to the point we had to get her out of there before she died. They kept her so doped up when we would go to see her she'd kept falling asleep. They wouldn't brush her teeth or even take her to the toilet and allowed her to defecate in her pants. She fell out of bed more than once and they blamed her. She was injured and they put the cost back on us for her treatment. The staff is lazy and very unprofessional and non caring to any ones needs, their cell phones took priority over patients. There were a few exceptions but the majority need a new career. You've been warned!

They did a wonderful job during the hurricane everyone pulled together awesome job they were under staffed and went out of there way to help everyone thank you Jason and all staff they were under staffed and worked long shifts great job all!

She’s in Moultrie Creek Nursing & Rehab Center. They’ve been great. The location is fine, and she’s happy with the food. It’s a shared 2-bedroom; she’s OK with it, and she has a nice enough neighbor.

My father-in-law is in Moultrie Creek Nursing and Rehab Center. He likes it there because it is close to his church. He talks to his friends and watches TV. They serve three meals a day and have a laundry facility. The place is very clean. The staff is very attentive, honest, and very nice. He had a $100 bill with him when he moved in, and they took it and put it in a safe for him. They have in-house doctors, and when I have to take him to the dentist, they provide transportation because he is in a wheelchair.

Never never never allow a family member to stay at this place. They will keep them there as long as Medicare will pay even though they should not. My father did not walk or feed himself for three months while he was there and the first week he left he was walking and feeding himself. When something like this happens you don't know what to do but my advice is get them out of there as soon as possible!

Moultrie Creek Nursing and Rehab Center was a big facility for rehabilitation and physical therapy. The facility was located in a place where it was away from traffic and there were a lot of trees. The entire building was set up for rehabilitation as evidenced by the walkers, wheelchairs and other devices that i saw. It was well-kept, very tidy and did not smell like a typical convalescent center. It looked like the staff actively took care of the residents and there were doctors around also.
