My father was there several weeks after contracting COVID19 from a previous rehab and not being properly diagnosed . While there he developed a rash. claiming it was a "side effect" from COVID19, they drugged him up on Benedryl like med and Hydrocortisone. When his benefits were exhausted they sent him to ANOTHER REHAB . This rehab saw the rash, nothing was on my dads paperwork that came - they did a scaping and found out it was SCABIES and he got it from improper cleaning of bedding, clothing and contact by staff over at OAKBRIDGE. The newest director called me asking me was I aware of this latest situation.

Was send to palm harbor nursing home after testing positive for covid 19 along with 3 other patieints without getting negative tests first.when talking with my mother-in-law she was so doped up she could not talk and said they were keeping her "happy"

I was in here for one week in February 2017. I was rehabbing from a stroke and found the rehab people to be excellent. As far as the rest of the facility it was not so great. It was obvious to me the staff was not capable of handling all the patients in their charge. I left after only a week and finished my rehab at home much to the dismay of I guess the administrator who wanted me to stay.

My husband has been in this facility since March 29. It seems like no one knows what is what here. It is like pulling teeth to get anything or any answers to any of his questions, Everything is done very haphazard. There have been at least 3 evenings that there wasn't a tray for his supper and had to ask for it. He can't even get a towel and washcloth, I have had to go to someone to get them. He wasn't even given admission papers to sign until he had been there for about 4 days, Also, he has wanted to talk to Social Services and has been told they would come talk to him and finally I had to go to her myself.. The only good thing he has said about it is the Physical Therapy people are good. This could be a nice place if only this was run better. I know all these complaints are true because I spend the biggest apart of my day with him.

This place is absolutely horrible... do not put your loved ones here, my grandmother layed in the bed for 3 days with pneumonia and nothing was done, the dr ordered xrays and it took them 3 days to get the results back... absolutely horrible...

The moment I walked into this placed with my Mom I wanted to turn around and take her home. It smelled of urine and was very dark and gloomy. The furniture in the rooms are old and worn from the 1980s, the sheets are thin and never had washclothes,the equipment was poorly kept and soiled with build up of crud. Accucheck meters and some blood pressure equipment not working. My mother didnt get her meds for about 24 hrs because they said they hadnt arrived from tampa or St pete(they couldnt decide which place) get blood pressure was high and nurse was concerned and decided she'd see if she could rustle some up! They never had her on a list for dietary ai she didnt have meals sent on the cart for her, after the third day a dietician came up to ask her preferences and the next day she got things she told her she didnt like and was cold.( the reason they said its cold is because her hall was the last one to get their trays! WOW! The food and appearance was not good . They're only allowed 5 briefs a day to soil in, her sheets and bedfing not changed for 5 days. No shower as of the 5th day although dis finally get a basin of water on the 4th day to wash up with.housekeeping seems to do a good job considering the old decor, though they need to give some attention to wiping the goop on the bed controls and the base of toilets, oh and the closets need to be cleaned out and mopped and throw out some hangers! The therapist were great and knowledgeable. Nurses and people at main desk should ask if they can help you instead of staring at you like your a bother! The management office was great. The admission office took 5 days to do the paperwork, I learned that just because the area location and the outside look pretty and pleasant doesnt mean the inside is! So as they say" drive the car before you buy it"

The done care about patients, the food is serve cold,the employee are just employees half of them don't care for the elderly. , place always smells like urine

We placed our mother in Oakbridge Healthcare in Lakeland, Florida for rehab. The facility was clean; however, upon admittance, I immediately detected the smell of cigarette smoke in the room. Upon investigation I noticed residents and guests smoking on the porch across from the nurses station. No Smoking signs were clearly posted in the area. Each time the door would open the smoke would filter throughout the hallways and into patient rooms. I approached the nurses station and was told during the day there is a designated smoking area away from patient rooms, but during the evening, the only place patients and guests could smoke was on the porches in the no smoking areas. I found this to be unacceptable as my mother is allergic to smoke and the fumes irritated my mother. I contacted the facility administrator who assured me this was unacceptable and the matter would be addressed. Unfortunately, patients and guests still smoke on the porches. One evening my mother had an accident in her bed and I asked about changing her sheets. I was told sheets were only changed every other day. I questioned that and found some sheets on my own and made my mother's bed. The nurse asked where I found the sheets and I told her. She replied "don't let laundry know you did that". I also addressed this matter with the administrator and was told that matter would be addressed. To date, my mother's sheets are not changed every day. My mother indicates the food is tasty. Her only complaint is that although she clearly identified the types of foods she does not like, i.e., cheese, pasta, grits, gravy, etc., the cafeteria staff continues to bring those foods to her and do not give her the alternate option. We found one nurse, Evelyn, to be very kind and helpful.

We have been at the Oakbridge several times because my mom is there for rehab. The facility is very clean, and their care was quite good. My mother is quite pleased with it; she was sad to leave the facility. I think they just need to probably update their décor.

Poor service, poor management. The only bright spot was a nurse named Ashley who went above and beyond for my family member. If it wasn't for her, my mother would have been pulled out early from this place.

Oakbridge Assisted Living Facility has two levels: the first one is for rehabilitation exclusively, and the patient are not permanent. The second is the assisted living facility where the residents are permanent stay. The facility is fairly new, within the last 20 years, and while it is in need of paint and basic upkeep, its appearance is not troubling in nature. The residents have a lovely restaurant-style cafeteria where those who are able can order, eat a full meal, and relax with a cup of coffee afterwards. This makes the facility feel more homey, or that it offers choices. Many of the residents appear to be completely unable to care for themselves, and the restaurant-style is not available to them. The staff have very clear delineations about working spaces, and their schedule is very tightly adhered to. There were programs in place to help the residents try to feel "normal" such as outside time, gardening, and reading groups. In addition, they seem to take fall risks very seriously through the careful use of lap restraints on wheelchairs for those at risk of falling. Some of the residents do, however, seem to be overlooked. If they are very quiet, or not a disruption to the peace of the place, they can be overlooked, which is unfortunate. I also think that the activities need to be more universally appealing and perhaps involve music.

Florida Senior Living was one of the first assisted living facilities I visited when searching for a place for my grandfather. Throughout the tour I was able to notice that the facilities were super clean. Even the public bathrooms were really nice. I didn't get to check out most of the amenities, as the tour was fairly short. The staff is what set this place apart from any of the other facilities we checked out for him. Everyone was extremely nice, almost to a faked degree. Like they were just being nice because it's their job to get people through the doors I guess. Some of it was genuine, but I believe a lot of it was faked. I will say that most of the interior was a very nice deep blue color, extremely soothing. If I were to ever have to be put in an assisted living facility I'd probably want to go here. (Let's hope that never happens.) Overall cleanliness I'd probably give it a 9/10. The staff? Probably 7ish/10. Amenities 6/10 only because I didn't have enough time to go through everything.As selected above on the scale. Overall I'd give this place a 4/5. If you're in the Lakeland/Tampa/Orlando area and are looking for a nice place to go/place your loved ones then this should be one of your top choices.

The Oakbridge healthcare center is an great place for short/long term care. They have a modern facility with all the comforts of home. The rooms are very spacious and designed with care. The staff is highly trained, informative, and very caring. The staff has daily activity planned throughout the day for members to participate in. The staff is very accommadating for many of your families needs. They offer transportation to and from Dr. Visits and many other destinations. The facility has a security policy that will ease your mind on the security of your family member. The facility has great dining menues for any picky eater. The food is prepared with their diet in mind. The staff is fantastic and will befriend your loved one, just like mine. If you want your loved one to feel like they are right at home, this is the place. This facility is just short of a vacation resort, your love one will be sure to love.

I recently had to place a loved one in this facility. The admissions and business staff were great. The therapy my loved one received was sufficient as well, but I think this was do in great part to the motivation level of my loved one. I was concerned about the security door that was used to monitor who comes in and out of the facility. The code was known by all residents and they freely gave it out to any random person who came up to the door. The location of the facility was also a concern. Though it is in a decent neighborhood, the area is quite woody and was not well lit which was a concern for me and my loved one. Overall, besides one employee who was always in a bad mood, I was satified with the facility.
