The room was in the older part of the home so it does need some TLC & the food was average but my husband was on a special diet. The staff was great. He received wonderful care. They were very friendly & caring. He has to go to dialysis 3 times a week & they had him ready to go on time & a lunch packed for him. Special thank you to Contina & Ophelia.The facility took care of setting up the transport & all the at home care. A big Thank you to all the staff.

Dirty rooms Bugs in bathroom Only a few CNA'S are good Let's you lay there with low blood pressure Smells bad Food is not good they do not adhere to your diet but act like they will.

The rooms were dirty with the curtain having a very large stain on it that was black. It had bugs running around the room. And the bathroom had white stuff on the floor around the sink and toilet, I complained about it and the smell. Never did they clean it up. I ended up with either a rash all over my body or bed bugs. Not sure what. But when I complained about my rash they told me have someone bring in benadryl. The food was always cold and a sandwich contained two pieces of bread with one slice of cheese and one slice of cold cut. Food was so bad that for the most part I did not eat it. Asked for hot water with my meals and for the most part got it afterwards when they where picking up the trays. The place was so bad I ended up going to the hospital with an asthma attack due to the fact they refused to give me the medicine I needed to keep it under control. So I spent from Saturday to Monday afternoon in the emergency room being treated for that I had three room mates. Two of them got transferred out to another nursing home and one went to the hospital not to return and she was not in good health. I was on pain meds and a lot of times it would take up to 3 hours to get it after I had called and talked to at least 3 or 4 different people. People across from my room also had a bad rash too. So if you care for your loved ones to not send them to this place. If it is your choice to go to rehab do not go to this place. I spent 60 days there. I have to have surgery again in the coming year and will not be going back.

Overall, this rehab facility is better than most. A recurring problem is that some ordered medications were not regularly delivered. Sometimes they were omitted for several days in a row! The problem was supposedly rectified, but it re-ocurred at a later date, even though the administrators had been advised. I recommend all problems should be brought to the attention of the facility's social services department, as they seem to get results.

Absolutely the worst facility. DO NOT bring anyone that you care about here for care. They are incompetent and not responsive to needs. The staff turnover is daily, and they have no training, not sanitary. The management staff is useless, that is if you can even find them. I am certain that my Dad would still be alive if he wasn’t in this place. This was the only place that would take my Dad's insurance and he had to go here, and the severe bedsores he got from being here contributed to the infection that killed him. Run as fast as you can from this place.

I really like having my dad here. The staff are considerate, and take the time to answer all of my questions. The building is clean and they take really good care of him. The past year was difficult with visiting him, but that was not their fault- it was covid guidelines. The food is good, I’ve actually ate dinner with him a couple of nights and now he is able to go to Bingo, enjoy Christmas singing and it’s not just through a window. Rooms are always clean and I never smell any urine or the other!! The place is beautiful, and I see housekeeping, nursing and maintenance always working hard and they are always busy. The doctor is decent.

I'm currently in Terrace Health & Rehabilitation Center. I've been staying here for around seven months now. I don't really know if I'm qualified to be here, because I'm quite independent. It's a decent place if you need a nursing home. Most of the people here are qualified to be in a nursing home; they're all in wheelchairs and not really mobile. We're not able to leave the building; as much as they help in the rehabilitation, we can't go anywhere. The rooms are basically double rooms and pretty good sized. We get daily housekeeping, and it's a fairly nice place, a decent building, and decent quality. We have CNAs in the halls, we have seven halls in this building, and there is always a nurse in the hall. Generally the staff is very caring. If I need a nursing home, and if insurance can afford it, I'll probably be satisfied with this place. We eat a lot of chicken. It's kind of bland, but it's sufficient. It's usually cold by the time they put it on the plate and serve it. There used to be activities, but because of the virus we're not allowed to congregate together. They used to have bingo and painting, and I'm not sure if they stopped it just before I came in, or when I came in.

My mother was transferred from Nort Florida to this establishment without notice to me as her only child and care giver. They lost her clothes fed her food I would not give To a dog and treated her as a unwanted guest her entire stay. I reached my limit and told them to have her ready for check out with five days notice and they did not even have her medications and oxygen ready. I was firced to delay another couple of days. Do not let your loved ones be brought to this establishment!

My friend was in this facility for just over a month and the care was awful! Most of the staff were unprofessional and plain rude. No work ethic. She received the wrong medication, was give a cup of water which belonged to a patient with pneumonia, degraded by staff as they argued outside of the door on who would change her diaper as no one wanted to, on their cell phones, were not even knowledgable of what her illness was and it goes on. She was told by the Dr to let the supervisor know, but nothing was ever done. The Social worker brought a paper in to be filled out with these complaint and she never came back and got it. No follow through what so ever. The place looks great. Very deceiving. The 5 star sign should be taken down. I am a RN and would never ever conduct myself like they did here. Again. NO WORK ETHIC! There were a handful of staff that were great and had the desire to give good patient care. I doubt they will stay long. This facility is truly pathetic.

My sister-in-law is in Terrace Health & Rehab Center now. It is a nice rehab center with nice programs. The people are nice, kind, cheerful, very pleasant to work with, and very caring. The room that she is in is a double, so she has a roommate, and they have a bathroom. The whole place is nice. She eats everything that is given to her, so the food is good. I think it is an excellent facility. She has been in other facilities before which we thought were OK, but this is a much better facility than she has been in before.

I was in Woodland to visit, but I wasn't overly impressed. It seemed kind of cold to me. It didn't feel too comfortable, and it was like cold. The staff did their job. I hate to say they were impersonal because they were not. But they don't have time for the little things that they could do if they had just one or two more people. Woodland was clean. The rooms were small, and some of them were shared.

If you care for your family member then don't leave them there. Terrible care. Our father suffered a stroke and was transferred to Woodlands for rehab. He needed assistance with eating, transferring, grooming, etc after the stroke. Their idea of assistance was heavy sedation and leaving the food on this bedside table. There was no assistance with grooming, eating, brushing teeth, etc. After 2 months he developed decubitis ulcers, a UTI and pneumonia. He was admitted to a hospital with a UTI, pneumonia, dehydration, severely malnourished and died several days later. Horrible facility.

My daughter was admitted here after a knee replacement. The staff who wheeled her in had no knowledge of joint replacement protocol. They dumped her in her room and no one checked on her till I made a stink. They did not have her meds. They could not provide a clear answer as to when they would have her meds. The nurses were surly and non responsive. There was NO TV in her room. But the worst part was laving to listen to the other patients calling for help over and over and being ignored. They had a really LOUD tv at the nurses station, I guess to make it easier to ignore the patients. Overall horrible experience and I checked her out of there after five hours.

Woodlands Care Center was recommended to us. I like the rooms, they were spacious. I think the staff was acceptably helpful. It would have been a fine facility but I was put off by the hospital atmosphere that they have.

this has to be one of the worse facilities i have seen. the night staff let my husband fall when he needed to use the restroom and he ended up with trauma to his rib cage. two days later the director told him that if he did not particapate in his re-hab they would have to let him go as a patient. After two days he was brought to the hospital where they confirmed there was trauma to the ribs, he had a lot of pain. after an extended stay at the hospital, i brought my husband home. would not leave my dog there
