My husband stayed at the Crossroads because he needed physical therapy. The staff is the best thing about the Crossroads. They are very caring and kind. I was very pleased with the care he received.

Understaffed. Response time is sometimes an hour late. Meds did not always arrive on time and I was left in a great deal of pain for over an hour. Rooms are spacious, clean and comfortable. Staff members were always courteous. Lots of planned activities and entertainment.

My mother was here for about a month in early 2020 after a hip fracture. The staff was competent and friendly but overworked. The facility is older and the rooms are small. More importantly, it is kept very clean. They seemed to always be buffing the hallways. I would give it 5 stars if they offered a gluten free menu.

I was fairly happy with the care my mom got at this facility. The rooms were clean and the staff was usually very helpful. Since she was only there for one month until her death, she did not do many of the activities while she was there. She was in the last stages of dementia and congestive heart failure. Overall, I am pleased with all that was done for her and thankful it went so well...

My cousin has been here since December after having had a horrible experience at another facility and being new to the city. They've earned my trust and confidence. As with any new relationship - of course, it takes work, on both parts but I can honestly say that the staff at Crossroads are professional, courteous and I believe they have the residents best interests at heart. They provide activities to stimulate the patients and keep them engaged. The facility is kept cleaned and is well maintained. The social worker works hard to satisfy the needs of each individual family's requests and that is a difficult tasks because it is hard to please everyone. The office/front desk personnel are very pleasant and friendly. I feel that they work as a team here and that is what counts.

My mother-in-law was a rehab patient and we stumbled upon Crossroads on the list of providers per insurance. Quick search found this was 4 out of 5 stars. Well that is SO NOT a true rating. The rehab part with physical therapy is the only thing they got going right. Response to call lights was not adequate - over 30 minutes at least for anyone to acknowledge the light. There was a major communication lack, and all her clothes went missing - despite having her name in permanent marker. The room mate was in constant agony - falling on deaf ears. Trying to get her discharged when surgeon said she could go home, was a pain in the butt. Despite talking to the social worker on Wednesday - and being told she would tell doctor at meeting and call me back that day - never happened. I ended up calling the insurance company to find out our options - and now the social worker is mad, and refuses to do her discharge job at all. If you are looking for a place - cross this off your list if you are wise. Otherwise your family will come to visit and be greeting by 10 guests in wheelchairs hanging out by the nurses station basically being ignored. Run the other way!!

My Dad was a resident of Crossroads for many years. We always appreciated the attentive and friendly staff. He made many friends while staying there and enjoyed the activities. Having physical and occupational therapy onsite was a huge benefit.

My father was a resident at CrossRoads for a few years until he recently passed away. He was very happy at the facility as they made him feel like family. They provided excellent nursing care for him and attended to all his needs. Although I am sad that he is gone now, I am glad that he had his last years at a facility that really cared about him and tried to make the end of his life happy and meaningful. I am very thankful to all the employees at the CrossRoads.

My mother recently needed short-term rehab for a compression fracture due to a fall she experienced at home. Since we live in the Davenport area, I needed a facility close to home. I was very happy with the caring atmosphere I found at Crossroads. The therapy team gave my mom the confidence she needed to complete her goals and now she is at home living her life with assistance with her home health agency. Thank you!

This place sucks. My mom has been there over 3 weeks. Doctor came to see her twice for about 1 min. I told the nurse tonight my mom was overdue for her pain meds and she about called me a lier (not to my face) and told me that even though the rx said every 4 hours, doesn't mean they patient will get meds every 4 hours. They can wait longer. Physical therapy hurt her 3-4 times already.

We chose The Crossroads because of the location to which mom was living, the hospital she goes to, and the location; it is in the town where she’s from. She’s been there for a week. They’ve been very helpful and very kind. She is in a shared room, and it is kind of small. An activities person comes in and talks to her, and a physical therapist works with her.

The only reason I am giving this place one star is because this site wouldn't let me give them 0 stars. Believe me when I say that they don't deserve even 1 star. I was in this facility after I was discharged from the hospital following knee replacement surgery. My husband had come to the internet to get reviews and this place had 5 stars so we picked this place to come to. Well I feel the only way they had 5 stars is if they are putting in the reviews themselves because there is no way in hell that they got that from someone that stayed there. Some of the staff were good but they were overshadowed by the worse staff I have ever seen. I am a RN and this place is horrible. They do respond to the call bell on a consistent basis. One night I had it on for 2 1/2 hours with no response and no pain medication. I did not have a PCP in this area so one had been assigned to me in the hospital and one of their partners followed me at Crossroads. I was told by staff that the MD wasn't responding to their calls so it was recommended that I change my MD to their Medical Director. I did so and that was a HUGH mistake because he took me off of the pain medication that worked and told me that "Pain was part of the healing process". I told them that my surgeon knew I was on pain meds and wanted me to continue on them. I was told that it didn't matter what my surgeon wanted because they were in control of what happens there. That was on a Friday. Saturday I told the facility that I wanted to be discharged but was told I couldn't start the discharge process on the weekend so I had to wait until Monday. On Monday I told them again I wanted to me discharged and was told I had to wait 72 hours because that was the facility policy. If I left it would be AMA and insurance wouldn't pay for the time there. So they held me there from Friday until Wednesday against me will and I was in pain all this time. They also kept the written prescription that my doctor had written in the hospital for me to take when I went home. They said they used my RXs when I arrived in the facility for my pain meds which is so full of it because they didn't have my pain meds for 36 hours after arrival and I had to us my own meds I had from home. I have told my surgeon about all that happened there and he was pissed. I have a feeling that they will be a complaint logged against them by my doctor. I know I am leaving something out but I wouldn't take a family member there if it was the last facility on this earth!!! Horrible place!!!

I wanted to move out of the place where I'm living now and move into a better facility. The Crossroads was clean and the staff was great. But the price is high.

The facilities are sadly in need of improvement and a substantial upgrade. There is little space for families to commune with their loved ones when they visit, and the residential rooms are simply too small to visit in. They didn't say anything, but my family members who stayed here seemed very uncomfortable in the living arrangements and I felt that they needed bigger quarters, especially considering the price of their living there in the first place. The staff is nice but not very well organized, and I consistently feel like asking the nurses what their qualifications are... in general they all appear absent minded as they work around the residents.
