I was at Pinnacle for 4 weeks and was very well taken care of. From the administration all the way to housekeeping, I cannot say enough about the staff. They were kind, attentive, had a good sense of humor and truly cared about the residents and did their jobs vigilantly. I enjoyed my stay!

The rooms are kept clean. The food needs a lot of improvement little to no taste and severed cold most of the time to the room

I had a close family member stay at Pinnacle. The facility and nursing staff itself is ok. I definitely was impressed with the food quality and kitchen staff.

My family's experience with Pinnacle far exceeded our expectations. We found the facility to be very well kept (clean, organized and secure). The staff was very caring, courteous and professional. Even the administrator(s) were very receptive and responsive to our questions and concerns. I feel as though our loved one was very well cared for during his time there. I would definitely recommend Pinnacle to others for their rehab needs.

My husband was sent to Pinnacle for rehab after trying to commit suicide. There he was supposed to receive treatment for psychiatric care. He got everything but……his issues were not addressed until the last two weeks of his stay. He was also supposed to receive Ot and Pt. It was not consistent nor administrator on a daily basis. The nursing staff was wonderful and very attentive. The physical faculty was a disgrace….dark, dank, smelled, and the mattress on his bed was not fit for anyone to sleep on, but maybe a dog!

My dad was at Pinnacle Rehabilitation in Smyrna. He was there for around 2 months and came home just before Christmas. I would give it a higher rating, even though it was not as pretty. Just looking at it, it looked more like a hospital. However, they took good care of my dad. Everyone was attentive. They watched over him. The food was good. The people who served the food were nice. The people who cleaned up, if he had an accident were nice. They had watchable TVs in every room too. While the furniture was definitely old (and it looked old) like it's been there a long time; but when it came down to it, it didn't really matter. Everybody else was nice. In this particular place, I had access to both the rehab and the nursing. The nursing part of it wasn't separated into two, so there were people who were there because they needed nursing care as well as needing rehab. My dad was among all the others that have the ability to walk around or roll around in their wheelchairs. People watch them, but they have a little bit more freedom. They had activities to do, and they definitely engaged everyone. I was very, very surprised because when I looked online, it didn't have a great rating. The food was very good. They had a lot of different varieties at first because he has had a heart attack and dementia, so they were giving him purees. Then they went to cut off ones, and then he went to whole foods. This was all in conjunction with the occupational therapist. He was closely monitored, so he went through those 3 stages of food. The facility was clean and maintained well. It was all on one level, and there were corridors to different sides of the building. It was all connected at the same time. They have wings, I guess you can call them. They gave them different names. I didn't see that there was really any outside area for my father to be in. The residents in the nursing home section (there was a nursing home long-term care section) had a living room area that went out to the back yard. We could have gone outback, but this was November or December so nobody was going outside.

My dad was transferred to the hospital and the attending physician who treated him reported the condition he was in after residing in Pinnacle to the State of Delaware. That should be enough to discourage anyone from placing anyone they care about in this facility.

The worst facility I've ever seen. Staff very unprofessional. There is obviously no dress code many dressed in poor trashy taste. One girl, just a kid, at front desk dressed like she was going to play softball. Nursing leaves a lot to be desired- half of them you can't understand what they are saying. No to mention there attitude. The caliber of the CNA's...the bottom of the barrel. Rooms are infested with ants. The food my dog wouldn't eat. One nurse told me my Uncle would be back late afternoon because he was at dialysis. The problem was...my Uncle not a dialysis patient. Do not recommend.

They make you wait for meds, to use the restroom. they left marks on my mom moving her to a wheelchair. I would not send a dog to this place and am so sad my mother had to go here. They also "lost" her mail at Christmas.

I can't even begin to even select a star for a rating but I had to at least select 1 to get my story to post. To be very honest, I wouldn't kennel my dog at this facility. My mother-in-law was brought there for infection control due to MRSA. Due to her mental state, she hid what was really going on until she had a nervous break down and went as far a calling the police to have her removed, which escalated to them calling her an ambulance to transport her to the hospital for immediate medical care. Upon admitting her to the hospital for sever dehydration, it was discovered that she had an extensive blistered purple colored infection from inner thigh to inner thigh from being left in dirty diapers. Are you kidding me??? We also became aware that other then physical therapy, they would leave her in bed all day. No staff, no matter how many notes of request were left would ever return phone calls. This place needs to be completely investigated and we have contacted our attorney.

I have a good friend who was placed at Pinnacle after he had hip surgery. I have had an earful of complaints, from not providing pain meds ( I have to wonder what the staff is doing with the pain meds), to not providing meal items, e.g., milk. On several occasions while i was visiting him, I saw first hand exactly what he was complaining about. All I can say that if I was there, I would crawl away if I had to.

Grandfather was a resident at Pinnacle nursing home before he passed away. In general the facility was clean and most of the staff were pleasant. There were some exceptions on the nursing staff, which prevents me from rating the facility any higher. We were able to visit as we pleased and he seemed comfortable in the environment. Another positive was his medications were readily available and assistance was there when needed. We had one issue where he appeared able to function on his own and wanted to leave, but staff would not let him. Discussed with senior management of the facility who failed to address our concerns timely and really did not provide appropriate focus to his needs and concerns, but we were able to discuss with lower level staff and resolve concerns. Maybe we did not put enough pressure on management to address, but should patients really need to pressure senior management for action when escalated that high? I think not either way my rating is a 3 as overall experience was just average. Would not recommend to others until staff and management communication issues are resolved.
