We chose The Peaks Care Center for my mother-in-law because they helped with the process of Medicaid. They take Medicaid. They are one of the most highly reviewed assisted living centers here in Longmont. They are helping her with everything, daily living, and medications. The nurses, they're angels. It's an older building, but it's clean. They care about their patients. They have a couple of different nurses stations in different areas, so there's always something going on there. They have a dining area. They have activities. They have a lot of freedom within the building.

My 88 year old father was released from this facility 16 days after being admitted for skilled nursing care following hip surgery. What? The facility reported his progress had 'plateaued' and he declined services. My dad weighs 125lbs, is anemic and his right leg was swollen so bad he can't wear shoes, thanks to a compression sock being left on for 3 days. I recommend NOT sending your loved one here! It's impossible to get hold of staff (especially on weekends) and everyone has a different story. There is NO communication between staff, the patient and family!!

My loved one recently transitioned from skilled nursing to hospice care at the Peaks Center. I cannot stress how helpful, sensitive and caring the staff were. Another bed was put in his room so family members could rest at times. A cart of food and drinks and extra blankets was supplied. We were given privacy during this time. Everyone was supportive. What could have been utterly traumatic time was very much softened by the wonderful folks at Peaks Care and Hospice. I would heartily recommend this homey and wonderful place.

My mom (92 years old) was in rehab at the Peaks Care Center during the height of the COVID pandemic (November 2020). There were so many challenges during this time of not being able to physically see her except for visits through the windows and phone calls. The Peaks Care Center was amazing. The care, kindness and compassion that the CNAs, nursing staff, physical therapy, case worker and administration staff was wonderful. I entrusted the Peaks Care Center with caring for my Mom and I know I was getting the best care under such challenging, stressful circumstances. I would not trust just anyone to care for my Mom and I was not let down in any way. Thanks again for taking care of my Mom.

The staff members -- all of them -- at The Peaks were fantastic! My 93-year-old mom was there from Dec 2019 through May 2020 when she died. From my first contact with the social workers and admissions people to the end, all of her caregivers were compassionate, kind, helpful, and willing to do anything to make my mom and me comfortable. I am especially grateful to The Peaks' staff for all they did for my mom during the pandemic. For example, when I couldn't visit her in person due to the pandemic, nurses and CNAs gladly arranged video chats. And, when my mom moved to an outside room, staff encouraged and helped us make and post signs on her window. All of my interactions with everyone at The Peaks were positive, reassuring, and helped me feel less stressed about my mom's situation. I wish I had moved her there sooner (Mom even said so) when we selected a different assisted living place (where she was for a year before The Peaks). I am so very grateful for everyone at The Peaks. They do a very demanding job with grace, kindness and humanity -- especially during this pandemic.

I can't say enough about the Peaks Care Center and how they've taken care of my mother. She initially entered The Peaks on an emergency basis (in November of 2017) when her CNA caregiver passed away and The Peaks was recommended by Mom's Medicaid social worker. Because her care was so good/loving, we chose to have her continue to live there. The activities offered, the food (many choices), the clean and totally loving environment have reassured us that she in good hands and we made the right choice. Kerry, Cala, David, Ashley, Janet and so many others currently and in the past have made me feel that my mother is a person not just a resident client. All despite the fact that she's become more feisty as her dementia worsens... and more importantly despite the horrid situation they've all been thrust into during the pandemic. They cared for those who became sick while still making sure that the lucky, like my mother, didn't get ignored. They have been true heroes during this healthy crisis. An extra benefit of the loving environment and sweetness of the residents, my mother met and befriended a gentleman friend; until the virus hit, they were allowed to spend most waking hours together. Love. What more can one ask for their mom? Dementia is a horrible condition but all the staff (from the skilled CNA angels who change her briefs to the nurses who check out any aches and pains, to the visiting docs and dentists, PTs and OTs. My mother is in the best of hands. THANK YOU!!!

My mother was there for rehab. We moved her after a week. She was mostly ignored as her calls for assistance took a long time to get a response. Our family was left out of the loop on her care and information was hard to get. The case worker took days to get back to us when we couldn't meet with her at the only time choice she gave us. Don't go there!

My mother-in-law is a resident of the Peaks. The rooms are reasonable, clean, and spacious enough. The staff is caring. They have activities daily based on their schedule. The dining area is OK, and the food is very good. They have an outdoor area out in the back.

I had my mother there on the long term side. the nursing staff and CNA sat around at the desk and the call lights went un answered. The housekeeping staff did a terrible job. the floors had urine that was sticky and it smelled. The food was cold by the time it got to my mom it looked like it might have been a great meal breakfast and lunch was more appealing then dinner. The director of kitchens was rude and did not answer any of my mothers concerns nor did the administrator. She was very rude and not customer friendly. I would not recommend the Peaks to anyone.

We like Peaks Care Center. My wife had a nice room, and the staff were nice as well. They have movies, exercise, and activities. It used to be a hotel, and their location was nice.

I was in the Peaks Center and the therapists that worked with me were really good. They checked on me and kept the pain medicine going but don’t over medicate you. I just noticed that the patients don’t eat very well and a lot of food gets wasted. They don’t have their own van though and they hired somebody to take residents to the doctor. At one time they forgot about me and we had a hard time reaching the person to take me. I have a small room but they change the linen every day and kept new, clean towels in there. I thought they did a good job.

The Frontier Rebab Center is all about care and great customer service. The nurses are well educated, trained, and very caring. The CNAs work so hard to meet everyone's needs. The PT, OT, and ST staff and caring and well trained. The unit is beautifully designed with wide halls and lots of windows. The rooms are nice with a large bathroom. The intake went smoothly and the discharge was as smooth. Thank you to all of the staff.
