This is a really beautiful nursing facility inside and out with a caring staff. I was a patient/resident for 4 weeks in 2018, while recuperating from a broken hip. I had a large nicely furnished beautiful private room and bath; the nurses, physical therapists and support staff were all caring, attentive and encouraging. The physical therapy department was superb. For entertainment we had top notch musicians, art classes and a small library. The beautiful dining room was large and inviting, with delicious meals, and I will always remember the best ever brownies. There are no comparable facilities where I live in So. Ca. I rate my total experience there on a scale of 1-10, a well deserved 10.

This facility is one of the worst in Fort Collins. As a resident there when I was not well and could not advocate for myself. They completely ignored me. As I didn’t have any family members nearby. The underpaid staff only made me feel more alone. Especially on the weekends when no one would come and see me. I was often left for long periods by myself. The care team coordinator made my visit even worse. Giving me very little hope telling my that the chances of me getting better were slim and I would most likely end up a permanent resident there at the assisted living. This caused me to spiral into depression that I never thought I would get out from. Fortunately my daughter came and managed to rally me to get better. If it was up to the facility they would have been just as happy to have me pass away there after collecting as much as possible from my insurance company.. Please do not allow anyone to go through what I did. STAY AWAY

The CNA and staff is terrible. Countless times things needed to be done and they refused to help.

Lemay Avenue Health and Rehab was very nice. The rooms that I saw were excellent and clean, my friend had a private room but they also have options. She had no complaints because the staff seemed very good. It's a very large place with an area outside, and the facilities also look very nice.

My husband, who suffers from dementia, went to LeMay for rehab following a fall but he will remain there long term because the care is extraordinary. The CNA's and nurses are top notch, but more than that, they are so kind and loving to patients. He is much happier there than he was at his previous memory care facility. LeMay is wonderful.

While there are several caring nurses here, I have multiple times seen my mother left in here own urine for quite a while. I have seen here left sitting on the toilet for over 20 minutes. I was told I would be called ahead of her PT, to help motivate her to "make progress" so she would not lose here medicare coverage, and this did not happen. Once I was assured she would have transportation to an early morning neurologist appointment, and the driver never showed up.

Not only is it bright and modern I've found the staff to be very caring and attentive. Mom needed transfer help and they have a great system. She is in a semi-private room but the layout is such that you never see the other person.

We moved my husband into a nursing home in Columbine Health Systems but he died three days later. Their rooms were fine. They were great and I would give them a high mark.

My sister was in Lemay Avenue Rehab Facility. She was there for what Medicare allowed after her hospital stay. That facility was very nice and she enjoyed her stay there. She did participate a little bit but not as much as she did when she was in ALF. They had music where they would bring in people to sing or play the piano. They also had people come in and do their nails. They had activities all the time. The people there were very good about trying to encourage their patients to participate in different activities. The food was adequate. The staff was very nice. There were a couple of times we came there once for her birthday party and they were very accommodating and bent over backwards for us. They were very accommodating with the families. I would recommend this place. She was pretty happy there.

The staff at Lemay Ave. Health and Rehab are 4/5 star. Most are exceptional but the night crew (10pm-6am) has had many complaints about not being very polite to the residents. The caseworker on the 2nd floor, who helps us, is extremely good at her job and always helps us. The building is only 4 years old so it looks great, smells clean, and looks like a beautiful mountain lodge. The facility has a pretty good activity department but I have seen better activity departments at other nursing homes in town. The kitchen manager was recently let go because after 22 years with Columbine company who owns Lemay, it was discovered that she did not have a dietary nutrition degree. I'm glad she was let go - the food was not all that great. But now the new kitchen manager is revamping their menus to better reflect the needs of elderly, low-sugar, low-salt and various diets. My mother has lived there for a year and is happy to be there.
