Horizons Health Care is an integral part of the Surface Creek community of Cory, Austin, Eckert and Cedaredge. We are located in the shadows of the Grand Mesa on nine acres in rural western slopes of Colorado. We care for individuals with chronic diseases, and for those recovering from injury or illness.
