I am a friend or relative of a current/past resident

My mother was in Willow Tree Care Center. It was challenging. There were problems with documentation that made the insurance keep trying to refuse payment and we had to continuously appeal the decision. She was there for three months, and I did at least six appeals on them doing noncoverage. It always came back with the reason being insufficient documentation from Willow Tree. The staff was very caring, but the management was distant. I kept asking for care plan meetings and in the three months she was there, we only had one. Her physical therapist and head nurse were never notified of the care plan meeting. The facility was old and dated. There were ants in my mom's room the day she moved in. They started doing a lot of upgrades right before my mom was discharged. They were replacing flooring and doing painting, so I'm optimistic that they've made improvements. The toilet in my mom's room sounded like a helicopter taking off whenever she flushed it and she had to have maintenance come in at different times to work on the toilet in her room, but they never replaced it. The food was typical, like cafeteria-type food. It wasn't anything special. It was edible, but they had a lot of just sandwiches.
