I think that the staff would like to give better help to the residents, but they are severely short staffed. My uncle has been there for a year, he's fallen twice, broke his hip, has had multiple UTI infections and is in and out of the hospital with sepsis. Many times his TV is off for days at a time because his remote has dissapeared and he is sitting staring at the wall for days. Each time the staff is unable to help us, they can't find a remote. It seems like a trivial thing, but when that is the only thing for the resident to do all day long, to be without it, when it is such an easy fix, is just very frustrating and sad.

Please do not put your loved one in this facility it has to be one of the worst places. My dad was there for seven years due to a massive stroke and on set dementia and other health issues that came along. We were told this was a 5 star facility what a joke. My family and I were there every day and let me tell you the things I witnessed with my dad every day when I walked into his room No one should have to go through that because the ones taking care of him should no better. My dad was parilized on one side and not able to help himself so he relied on the cnas. In the beginning we had the best but as time went on and ownership changed the whole place went to hell in a hand basket. I have so many pictures of my dad laying in a soaked bed of urin for god knows how long. Fecies all over the bed and all over him. Bruises from where the staff would grab and bruise him fingernail claws in his arm and so much more. I finally started taking pictures and I can back all this up. So please think twice before you put your loved one in here.

Stop!!! Please do not place your loved one at this Horrible facility!! My grandmother fell and had surgery on her femur she needed physical therapy and was placed at Willow Creek Healthcare Center for physical therapy. Now where do I begin with this nightmare my Grandmother had to experience. Shortage of nurses (why except new patients on a daily basic) her hearing aide was lost, she was not turned every 2 hrs as instructed by her physician causing several bed sores, medication was not given on a timely manner and her Pain Medication was not given for 2 days!!! After several attempts to contact the Charge Nurse, Director of Nurses, and Administration nothing was done!!No family should have to go though this! It was Horrible!! Please rethink placing anyone here!! Shame on you Willow Creek Healthcare Center!!!

Nurses and Nursing Assistants were exceptional, knowledgable and beyond caring. I highly recommend them to home health, or any other worthy institution. I think they are all getting the short end of the stick working here due to the simple fact that they can not get patient meds and needs met at all. Patients and staff are at the mercy of some kind of outsourced courier who shows up when they want, hours and days after scheduled. I fell and broke my leg in two places, and since being transferred here, I have not had any pain medication. NOT THE FAULT OF ANY OF THE NURSES. OWNERS OF THIS FACILITY NEED TO CONTRACT A BETTER SOURCE OF TRANSPORTATION FOR PATIENT PAIN (controlled) MEDICATION.

Had to pull my mom out of this place. 1. First nite she was there they didn’t know anything about her 2. They didn’t even know she was in the room and missed her for breakfast 3. A week before her sheets were changed 4. Not watching over her well enough as she slipped while getting onto wheelchair. (Wheelchair was not locked) 5. Lack of communication amongst nurses and with family 6. PLEASE DON’T PUT YOUR FAMILY IN THIS PLACE.THEY DON’T CARE. 7 if I could show you the bruises on my mom Ifrom her fall, You would think twice about putting them there.

My husband and I were both in this facility as patients for a short period of time. Nursing staff ignored buzzers, rooms were filthy, food was horrible...Nursing staff did many code violations in the short time we were there. Cockroaches running across the floor in my room. They were giving my husband his medications an hour before he left to be dialyzed so the meds were just dialyzed out of his system and not doing him any good...I checked us both out.....I was supposed to get physical therapy from day one while I was there....Didn't see the physical therapist until the day before we left. They were supposed to order me a walker which they never did. only 1 or 2 of the nursing care staff showed that they cared. Couldn't get a shower and was lied to about what time I could take one and was told I would have to wait another week before I could get one...the rest of the nursing staff acted like they were only there to collect a paycheck...Would not recommend this facility to anyone.

There is a handful of nurses and cnas that really care but the office staff cares nothing about the patients never asked if your doing ok or if theres anything i can do no did you find a apartment there ceo and assistant are a joke the cleaning staff is friendly and cares the receptionist tries to do the best she can the food is awful and they think were rich and tell us to buy food from other places are they going to buy it for you if you have a parent, friend or any kind of elderly person please don't come here its terrible i live here and see and hear everything its so bad nurses quit

Our family recently went through a very traumatic time with the loss of our grandpa and subsequent need for skilled nursing/rehab for our grandmother. We made the difficult choice to place our grandmother in skilled nursing after a bad infection and surgery that didn't go well. It was an especially difficult decision to place her with everything going on with COVID-19. I want to say unequivocally, that the care and compassion received from every individual at Willow Creek Healthcare from the CNAs, rehab personnel, admissions staff and directors, was far and above any expectation we had. They knew about the prior difficulties our family had recently had and they treated our grandmother with the utmost compassion and respect. It was so meaningful to us because we were unable to physically see her. It wasn't just the compassion that was wonderful, the actual care she received was amazing. She went into the facility unable to bear weight after a bad infection and surgery. Upon discharge, she was walking over 100 feet and her progress has continued to improve after discharge. We received updates everyday from her nurses and from the rehab director. They called us with updates on her vitals, progress with walking, etc. Not one time, did we have to check in for an update and that was so appreciated. This staff really went above and beyond and I could not be more thankful for everything they did. No skilled nursing facility is perfect, but this experience was as close as you could get. I would highly recommend Willow Creek Healthcare if you are making the difficult decision to place a loved one, either temporarily or long-term. They will be well taken care of. Thank you Willow Creek!

STAFF WAS CARING, COMPASSIONATE, ATTENTIVE AND PROFESSIONAL. EVERYONE WAS WILLING TO ASSIST EVEN IF THEY WERE NOT ASSIGNED TO YOU. ON A SCALE OF 1-10 I RATE THE FOOD 7.5---- DECOR COULD USE IMPROVEMENT, OTHERWISE I HAD A POSITIVE EXPERIENCE AND GLAD TO HAVE THEM THROUGH MY JOURNEY TO RECOVERY.

Worst place ever I regret ever having to put my mother there she always had fried food on her face stage 4 bedsore when my brother and I took care of her she never had bed sores the nurses and aids there are heartless asses we finely had to put her in hospice Thank God we got her out of there that place should be closed down I would come in and they would have her in a wheel chair her head hung way down poor thing

Understaffed nursing call lights not answered in timely manner person I was visiting asked for pain med waited 1 hr to get chairs in rooms dirty and stained I did not want to sit in them PT wonderful

I visited this facility

My friend had a terrible time there . The bathroom was unsanitary, the bed broken, stained furniture grainy tv, inattentive surly staff.

I am very unhappy with the care my dad gets here. Uncaring nurses, out of date building and technology, broken equipment and beds, broken call lights, tv's don't work, etc. the food is inedible and not enough to satisfy him. They do not abide by food orders or dietary restrictions. They are not sanitary and are grossly understaffed. My dads primary nurse hates her job and even calls me to harass me if I complain.. The staff have very heavy accents that my dad cannot understand. He is in the hospital now and because there is nowhere else for him to go, we are having to send him back. They went from a 5 star rated facility to a 3 in less than a year. I have tried talking to the parent company administrator, facility administrator, social workers, supervisors, ect without change. I now have to involve the ombudsman.

My dad was at Willow Creek Healthcare Center after a stroke. The people were very nice, and it was very clean there. I did not appreciate the discharge planning because there wasn't any, and the care wasn’t wonderful. When we got home, I realized that his bottom was really red and excoriated, so I wasn’t pleased.

My grandmother was a resident there for nine years. Willow creek became her home and the staff treated her as if she was their grandmother. I highly recommend this facility for your loved ones care needs.

It's not a very good or caring place to have your love one living there. Personal items have been missed place or removed. Place is very old and bed are not updated they are also old. I would not recommend this facility to anyone
