I have tremendous hope for my family member that just got moved here, my aunt lives here in the past and loved it! They have excellent staff, and cook hefty meals that is one of the reasons we loved it before! I foresee a great road to recovery for him here!

Very bad no ac in rooms. Won't answer phones when call to check on people. Always smells like piss. Been by there 3 times in one week still same. Vary,vary sad place. I no it old place but this is their home. Should not be like this.

This facility is awesome, and I just don’t know how to explain how above and beyond they go! Always finding different interventions to keep residents safe, and on the road to recovery! I can’t express it enough this facility is the best!

This facility is great! I have family in the memory care unit for women, and those ladies get treated like a big family. This facility is awesome, and always seeking for improvements!

The CNA’s here care a lot for each resident, you would never think at an older facility they would give such great care but we love the care my dad gets in the men’s unit here! The nurses are outstanding as well! We can’t help but notice how interactive each employee is here!

Do not place your family here. The facility is disgusting and has the worst management I have ever seen.

The outside area for patients is disgraceful. The small area outside had no grass, nasty cigarette butts on ground and in wet ashtray on table. Visiting room was very small with few chairs. Was no activities. Basically nothing for people to do to keep minds alert and active. Would never put a loved one here.

my sister is there. where they sat outside was dirty with trash and dirt with no grass. cigerettes all over the ground. rooms are very small

Ozark Terraceis an embarrassment. It is old and unkept. Obviously the owners are not interested in spending a dime inside or outside. Rooms are cramped and dreary and depressing. The food is bad, and served cold. Equipment, if they have it at all, is old and junk.The bathroom hot water is barely warmer than the cold, and they give bed bathes with that water. I witnessed an elderly patient walking the hallway with her pants and diaper falling off and her backsides exposed, nobody did anything to help her. Finally, the employee turnover seems very high, and the administrators lie to you whenever convenient.

The staff and mainly administer are very unprofessional. I would never have dreamed in a million years that a professional would scream at you on the phone for something you can't control but it just made me think she was more childish and wonder if she is capable of doing her job.

The staff, although kind, was unkempt and rather sloppy in appearance. The care was minimal. My father received one shower in one week even though he was incontinent. They reduced his fluids and peg tube feedings and he lost 10 lbs. during the one week that he stayed there. The director of nursing was unresponsive to any requests. The social worker lied to the new facility when we tried to transfer him and said that my father wanted to stay there. It is incredibly dark and depressing there and very poorly maintained. Overall the facility and it's management was unprofessional and completely unsatisfactory.
