This place is clean and their staff is warm caring and cheerful! Great physical therapy department. My father was able to recover and return home. Everyone knew him by name. After a very bad stay at Granite Creek in the past that almost did him in ...a nightmare really......this place was a pleasant surprise. The staff is friendly, helpful and cheerful. He was always clean and dressed when I would drop in unannounced. He had no complaints about his care. He improved greatly.. enough to come home. My family is very grateful to have had such a positive experience.

The rooms were small but clean. The food was as good as could be when trying to accommodate so many with dietary restrictions and there were lots of alternative choices. The staff was incredible. Always ready to help, answer questions or just offer support. Very friendly and concerned staff

Wow. Finding placement for a parent is an emotional and difficult trial. Mom was at a different facility and people in the know mentioned Haven Health and I am so happy that I was able to place her there. She has many physical issues and Haven Health was able to provide physical therapy along with support, encouragement and, most importantly, safety. Jazmin the Social Worker has gone above and beyond the call of duty in caring for my mother and helping us complete important financial paperwork and work with Medicare. Most importantly, my mother likes it there and she's not easy to please so that says a lot! Thank you Haven Health!

My mother was well taken care of during her stay there. Several employees stood out for their assistance, their pleasant attitude, and making you feel welcomed: Jasmin, Raquel, Kelly, Rich the driver. I was very leery of putting her into any facility, but my apprehension was soon removed after meeting above mentioned people.

My mother in law is here. The nurses, Nicole, Kelly,Anna, Juan, and the driver Rich are wonderful! They are always friendly, take very good care of her and know how to deal with most of her aliments and needed care. Rich, the transportation driver is always nice, in a good mood, goes above and beyond to help with things other than just driving. Makes sure she gets to her appointments on time and is taken care of. The front girl Reagan is a doll! Super professional, is always cheerful in her voice when you call and goes above and beyond to get msgs or a phone to have mom call me and relaying msgs to nurses. The main staff can sometimes be a concern because things are not taken care of in a very timely manner, or we are not notified when something happens,such as her falling which has happened a few times. We need to be contacted when things happen and needed paperwork is requested . The building is very old, her screen was falling off for a month, there were bugs in the sink, sometimes they forget to give her a bath twice a week, and they aren't doing much physical therapy with her anymore. She can not walk or stand alone., And they are no longer setting her on a toilet. These are some concerns I have. Especially since they are charging 7,000.00 dollars a month for care.

I just recently moved into Prescott Nursing and Rehabilitation Center. I think it had been an older building, but they've got it fixed up nicely. The staff were all welcoming and they've taken good care of me. The food is good; better than what I've had before. My insurance and Medicare are paying for it right now, and I'm not paying anything right now.

I was required to stay here for six weeks as I needed intravenous antibiotics every four hours. The staff is professional, skilled, friendly, and solicitous. I could not have been happier with the facility. The food is good, and you are given a choice of two entrées. You can eat in your room or go to the dining hall with other residents. The rooms are kept very clean, and the staff is very kind and helpful. Most of the times when I turned on the light for assistance, someone came quickly. There is an Activity Director and an area for sitting outside. It is very much like a hospital but more relaxed and homey. I will need to go to a rehab facility again when I get my hip replaced. This is the place I will choose.

My husband was a patient for two months in the fall of 2016. As a retired nurse, I was totally impressed with his care during his stay. The staff is caring to patients and diligent about their focus on mission and values. I will use them agaiN in the future.

My relative is in Prescott Nursing & Rehab. He complains about the time it takes for somebody to come and assist him with certain toiletries and bathing and stuff. He’s 63 and most of the patients are age 73 and above, so he’s like the youngest guy there. He doesn’t like being there, but he has to go to rehab. I’ve been in situations like this before and most of the night staff is crappy, but the day staff is attentive and very knowledgeable. He has a couple of them that come and help him with different matters. In general, they’re all friendly. The place is very clean. My mother had a stroke in 2009 and was in a facility that always smelled terrible. This place, however, is clean and smells clean all of the time.

Prescott Nursing and Rehabilitation Center is not a traditional retirement home; it is more of a care home that requires skilled nursing, so they have nurses all the time. It is more akin to a hospital, and they have a very good nursing staff. My sister stayed there for a year and a half until she died. She was pretty happy there, and she was comfortable. The people were very kind and generous with their time.
