The CONCEPT is quite simple. As members of our community, we reach out from our hubs to provide services to our community, developing PROJECTS which meet the needs and wants of our customers while supporting our customers in supporting their community. Operating from each customer's person centered plan, PROJECTS are identified (or created) which will meet the needs and wants of the individual. To encourage team work, the support team will work with the individual in identifying other customers who might be interested in the same PROJECTS and will support that individual in coordinating a team. The center offers adult day care and in-home care services to seniors in the surrounding community. At Truelove's Adult Day Health Care Center, seniors have access to a licensed professional nurse who is available to monitor their health and wellness. A compassionate staff is in place, offering services that include personal care assistance, medication management and stroke care. Truelove's Adult Day Health Care Center provides seniors with engaging socialization, games and exercise. Seniors visiting the center have access to nutritious meals and snacks, and they can take part in off-site trips to movie theaters, museums and restaurants in addition to participating in on-site crafts, table games, group discussions, and stretching movements.
