Addus Evergreen Clubs adult day centers that provide a variety of planned social, health and supportive services in a community-based setting. The programs are utilized by seniors and younger adults who have physical, cognitive and/or functional impairments, or by seniors who may simply need companionship and can benefit from supervised care in a safe place outside the home during the day. The programs also afford family caregivers respite from the demanding day-to-day responsibilities of care giving. Basic services consist of therapeutic activities, community outings, nursing services, transportation, meals and physical/occupational/speech therapy.

The CONCEPT is quite simple. As members of our community, we reach out from our hubs to provide services to our community, developing PROJECTS which meet the needs and wants of our customers while supporting our customers in supporting their community. Operating from each customer's person centered plan, PROJECTS are identified (or created) which will meet the needs and wants of the individual. To encourage team work, the support team will work with the individual in identifying other customers who might be interested in the same PROJECTS and will support that individual in coordinating a team. Within the framework of PROJECTS, on-going training will occur in "real time". About We provide multi-faceted day services to adults with developmental disabilities Company Overview Support Innovations is proud of our ability to provide individualized services to our customers with developmental disabilities. We provide our customers with true person centered planning. This process focuses on our customers, their family and friends in developing the support plans which best assists them in achieving their dreams and goals. Unlike some companies that only provide community...-based or center-based training, we are dedicated to providing either form of training and enrichment, or a combination of both, in order to meet our customers' needs and interests. We provide Medicaid funded services (Support Innovations Inc.) as well as Medicaid Waiver funded services (Support Innovations PLUS); providing all customers, no matter the funding, with the same high quality service because we believe that choices should not be limited by an individual's funding source. -based or center-based training, we are dedicated to providing either form of training and enrichment, or a combination of both, in order to meet our customers' needs and interests.
