INDIVIDUALIZED PLAN OF CARE

We offer an individualized plan of care specific to the patient's condition and requirements. Lead by a physician, the plan of care is developed keeping the wishes of the patient and their family members in mind. It is aimed to keep the patient happy and stress-free ensuring that they spend the last days of their life with dignity in the comfort of their own home.

PAIN AND SYMPTOM MANAGEMENT

Our team of nurses and therapists make regular visits to manage pain and other symptoms. Pain makes the patient suffer and limits interaction with their family. It makes thinking, eating, and sleeping difficult. Pain management keeps the patient comfortable and improves life quality.

MEDICATION FOR SYMPTOM CONTROL & PAIN RELIEF:

Our individualized plan of care also takes care of the patient's medication for symptom control and relief from pain. Once again it makes the patient feel relaxed and comfortable ensuring that they get to spend maximum quality time with their loved ones.

HOSPICE AIDE SERVICES:

Personal care during the patient's final days is important. It keeps them hygienic and well groomed. Several times all that is required is companionship that allows the patient to live the remaining days with dignity and helps them to share their emotions and stress.

MEDICAL SOCIAL SERVICES

Patients and family members often need assistance and counseling during the hard times of losing a loved one. Social workers assist families to define their personal goals and help them make decisions. They help in understanding the challenges faced before and after the loss and direct them toward appropriate resources.

RESPITE CARE

We offer Respite Care for your regular caregivers and family members to take a break. Constantly caring for someone can be exhausting both physically and emotionally. Respite care could be availed just for a day or several days of the week to rejuvenate, and recuperate from the stress, and hardship of constantly caring for that someone.

24/7 TELECARE:

Our care is provided 24/7. During the day, our team of professional are available for support and care for patients and their family. And during the night our Telecare is on the job to take your calls and handle all your queries.

SUPPLIES & MEDICAL EQUIPMENT:

We offer quality medical supplies ranging from catheters and wound care supplies to wheelchairs, walkers, and other durable medical equipment. Hospital beds can also be provided depending on the health condition of the patient.

SPIRITUAL COUNSELING & BEREAVEMENT SUPPORT:

A patient often turns to their spiritual side for comfort and acceptance towards the final days of their lives. Spiritual counseling addresses this need and helps bring them peace. Family members going through the various emotions of eventually losing a loved one can also find their way through spiritual counseling. Our bereavement support includes identifying family members in need of support prior to and following the death of the patient. The family strengths, concerns and needs related to the patient, are addressed. It helps family members process the various stages of grief and finally accept the loss of their loved one.
