Our ClientCare Management Service is a comprehensive, client-directed approach to caring for Elders facing ongoing health challenges. Working with families, our Care Manager provides expertise and guidance to ensure quality care and an optimal life for our clients, thus reducing worry, stress and time off of work for family caregivers.
Care Management Services Include:
Assessment
- Evaluates and provides an assessment of the clients current situation
- Provides a plan of care to be followed up on by the client, the family or the Care Manger as assigned.
General Well Checks
- In-home wellness checks
- Ongoing evaluation and assessment of general wellness
- Head to toe examination, including vital signs, state of home (cleanliness, safety), nutrition, compliance, etc.
- Communication with primary care physician and family members on status
- Healthcare referrals (connect with OT, PT, hospice, etc. as needed)
Coordination of Healthcare
- Ensures communication among care providers
- Keeps family members informed of client condition
- Tracks appointments and follow-up care needs
- Arranges for transportation/Arrange for accompaniment to physician visits and other healthcare practitioners
- Helps understand follow-up care and instructions, etc. and monitor adherence to medical orders and instructions
- Collect and communicate to family information about client doctor visits
- Presence in hospital or facility to oversee care and communicate to remote family
- Communications with outside venders such oxygen and DME’s
Medication Management
- Initial evaluation and ongoing tracking to ensure compliance and prevent contraindications
- Verification of physician medication orders
- Set ups (i.e. preparing medications for the week/month)
- Re-order and pick-up
- Electronic medication dispenser set-up and ongoing monitoring
Safety and Security
- Monitor client at home
- Recommend technologies to add to security or safety
- Observing changes and potential risks of exploitation or abuse
Remote Care Monitoring
- Personal Emergency Response System set-up
- Environmental monitoring of home – evaluation of monitoring needs
- Telehealth – remote monitoring of vital signs, weight, etc
- Medication management technology
Nutritional Assistance
- Tracking nutritional directions from health care professionals and assistance with compliance
- Arranging for assistance with meal preparation
Household Management
- Assistance finding household vendors for:
- Repair services such as deep cleaning and lawn services
- Home modifications such as wheelchair ramps, bathroom grab bars, etc.
Community Resources and Referrals
- Local, cost-effective resources identified and engaged as needed
- Continuous learning classes, senior community centers, etc.
- Support groups (through disease orgs.)
- Financial Advisors, Accountants
- Elder Law Attorney’s
- Funerary Services
- Etc.
In Home Care Service Monitoring
- Assess the need and make referrals for in home services such as:
- Home Care
- Home Health
- Housekeeper
- Meals on Wheels
- DME
- Assist to engage and monitor the services
- Family Caregiver and Client Education
- Information handouts
- RN consultations
- Homewatch CareGivers University
Transition Assistance
- Consultation in selection of retirement home/alternatives
- Continuation of care services in new home
- Help with introduction to new facility
Psychosocial Needs
- Planned socialization activities
- Arrange for transportation to and from social events and activities
- Bereavement Support
- Arrange for house cleaning and preparation for post-service entertaining
- Company during mourning process
- Assistance making arrangements, referral to community resources
Document Organization
- Legal-Will, Advance Medical Directive, POA
