If looking for a hospice company for your loved one, this is the one to go to. They were always there when things got rough. The entire staff were so kind, comforting, and knowledgeable. The nurse and shower aide were always on time and very flexible if we needed them to be. The nurse was always attentive to the needs of our family member.

This hospice team is truly an amazing bunch, how much they care shows every day. they are extremely helpful and very educated in the process of hospice. The staff is always on time and even tend to show up just a little early. The nurses and health aids are so nice and caring you really couldn't ask for a better team of individuals. I would recommend them to anyone and everyone who is looking at hospice.

Hospice is about living with comfort and dignity during life's final journey. It is an additional layer of care with another set of eyes and ears ensuring that all their medical needs are being met by our hospice team. At the core of Silver Linings Hospice Care's charter and culture is the belief that there is a need to offer the value of hospice while preserving dignity at this stage in life.
Services We Bring To The Patients
Your hospice team members include:
- A Physician or Nurse Practitioner who provides pain and symptom relief and overall medical direction.
- A Registered Nurse who visits 1-3 times a week more if needed.
- A Health Aide who gives personal care, such as bathing and overall hygiene which is a basic necessity.
- A Social Worker who offers emotional support and help with matters such as assistance with insurance or , power of attorney paperwork.
- A Chaplain (when requested) who assists with spiritual needs and bereavement concerns whatever your faith may be.
- A Volunteer to visit patients if requested.
Additionally we provide Durable Medical Equipment, such as wheel chairs, medical supplies such as wound care and medications for pain management or medications related to their diagnosis . All of this is brought to your home and set up and managed by your visiting hospice nurse
Patients can keep their own physician if elected. Our team works with the patient’s physician — or the patient’s physician may choose to have the hospice doctor oversee the care. Many just want to be kept in the loop regarding changes in the patient's status.
We serve communities in Santa Fe, East Mountains and the greater Albuquerque area.
Our Team:
Michelle Bacharka, RNMichelle is the founder and CEO of East Mountain Hospice, LLC and Silver Linings Hospice Care. As a practicing RN with a BA in business, she brings a diverse mix of experience to this hospice venture. She has years of experience as a former case manager and as a hospice registered nurse. Most recently she was the Executive Director at one of the larger hospice agencies in Albuquerque.
Prior to working in the hospice healthcare segment she was working at a level 1 trauma hospital as a medical surgical nurse. Before entering into nursing, Michelle utilized her marketing degree in varied business interests. She has spearheaded, owned and managed previous businesses ranging from non-profit consulting and public relations firms to an event management company.
Meet Our Medical Director:
Dr. David HemphillDr. Hemphill is a 1995 graduate of Columbia University College of Physicians and Surgeons. Before moving to New Mexico in 2001 he served as an active duty flight surgeon in the US Air Force for 5 years. He completed residency in Internal medicine and a fellowship in Geriatrics and maintains board certification in both specialties. Over the past 15 years Dr. Hemphill has practiced extensively in geriatrics and hospice medicine in a variety of settings. Over the past 10 years Dr. Hemphill has practiced internal medicine as a hospitalist in the Lovelace Hospitals and was for 3 years a member of the faculty in Geriatrics at UNM. He has served as medical director for two local hospices and has attended the Hospice Medical Directors conference sponsored by the American Academy of Hospice and Palliative Medicine in recent years.​ In his free time he and his wife enjoy the trails of Corrales with their dogs and horses.
