To provide the best care for the patient and family, Oxford HealthCare Hospice utilizes a team of highly qualified professionals from many disciplines as well as experienced Hospice volunteers. The following team members are dedicated to making a difference in the lives of people and work together to provide care and support tailored to the needs and wishes of patients and their families: Patient's Physician Medical Director Registered Nurse Hospice Aide Social Worker Chaplain Grief Counselor Trained Volunteers Patients and families benefit most when care begins early. Support is available 24 hours a day, 7 days a week and Hospice may provide the following services based on specific needs and the plan of care for the patient: Skilled nursing visits Medications, medical equipment and supplies required for diagnosis Telemonitoring Bathing and personal care Volunteers may offer assistance with shopping, errands and companionship Family conferences as requested Coordination of short-term inpatient care for pain and symptom management Skilled counseling for end-of-life care Bereavement program Community grief services When you choose Oxford HealthCare Hospice, you receive additional benefits exclusive to Oxford. Clients receiving home care services through Oxford can transition to the Hospice program without a complete change in staff and caregivers with whom they have developed relationships. In addition, our telemonitoring system gives concerned family members and caregivers the peace-of-mind that a professional clinician is monitoring the patient on a daily basis and communicating important information to the physician. Oxford HealthCare Hospice provides the same specialized care in both home and nursing home settings. Hospice care may be provided to a patient in a nursing home upon agreement between the nursing facility and the Hospice program. In this case, the nursing home staff serves as the primary caregiver for the patient. Regardless of the setting, the Hospice team is available to assist the patient and family in determining the appropriate approach to care to provide comfort.
