Golden Years Hospice Care delivers comprehensive care for patients facing life-limiting illness. Attending Physician: Certifies the patient has a terminal illness and remains the patient's primary physician and works as a member of the team. Medical Director: Certifies the attending physician's prognosis, leads the hospice team in developing a plan of care; provides consultation to the attending physician. Skilled Nurses: RN's, LVN's evaluate the patient's condition during regular scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques and the use of medication and pain management. Social Worker: Assesses social needs of the patient and family and provides psychological and bereavement counseling.
