Skilled Nursing Services will be provided in your home for your loved ones by an RN or LPN for the following medical complexities: CVL, TPN, CPAP, BIPAP, Ventilator Dependent, Trach Dependent, G-Tube Dependent, Oxygen Dependent, Medication Administration, Wound Care, Colostomy and Ileostomy Care, Neurological Disorders, Cerebral Palsy, Diabetes, Seizure Disorder, Paralysis, Wheelchair Dependent, Muscular Dystrophy ,Multiple Sclerosis and Lou Gehrig's Disease. Unskilled Support Services will be provided in your home by a Personal Care Assistant or Companion Sitter for the following functional limitations: Services include assistance with toileting, feeding, bathing, transportation services, doctor visits, transfer assistance, exercise, putting on clothes, bill paying, book keeping, memory care, companionship, along with many other activities of daily living.
