How to Apply for Home Health Care Financial Assistance in New Hampshire
Seniors or their legal representatives can apply for home health care waiver and grant programs via their Medicaid caseworker. New Hampshire seniors who don’t currently have Medicaid can apply online through the NHEasy website or by phone at (844) 275-3447. Applications can also be printed and mailed to the nearest DHHS office.
