My mother has been at this facility a little over two years, I am extremely involved and pop in to visit very sporadically- I must say I have been pleasantly surprised to see that the majority of her care takers seem to generally care for her- my mother is no walk in the park and can be verbally abusive and just plain mean at times- yet staff seems to maintain a professional and friendly demeanor around her- especially Eli, he seems to have a positive outlook and is always willing to lend a helping hand. She was initially admitted for physical therapy following extensive surgery and several strokes- we thought she would be only there for a few weeks but unfortunately it turned in to a much longer stay. The PT director, Debbie was amazing, going out of her way to work with mom to make progress which would lead to her being able to be up and moving now, I do not think we would be as advanced as we are today without Debbie's help! The majority of the CNA's there are friendly and helpful. The food is good, my mom enjoys eating there and often times tells me the food "is really good", and the times I have been there to eat with her in the cafeteria, they did have very tasty meals. My mother developed a bedsore from a hospital stay- stage 4- so it was extremely bad, the wound care nurse there- Jennifer was amazing at caring for it. She would make sure to send me progress reports and track the size, depth and make sure all medications were administered when they should have been- the PA, Christina was phenomenal- she would make sure mom was always comfortable and would periodically review mothers medications and make informed suggestions regarding her care. Christina is a Godsend to this facility for sure!! My mother's assigned nurse, Kusum is invaluable to making sure moms care is successful, she is very attentive and helpful, my mother calls her, daughter-that alone speaks to the relationship she has fostered with her!! Why did I wait over TWO years to write a review.... its because consistency matters!! Any facility can treat your loved one good one or two days.. but for a solid two years my mother has received excellent care. When I have spoken to the facility director, Adrianna, I have always been greeted with a smile and a very responsive and positive attitude, I believe that when there is good leadership there is a positive environment, and that seems to be the case here. I am glad I did my research initially and choose this facility for my mothers care.

My grandma been living here for years and the staff is great and sweet , make sure she’s good at all times I highly recommend!!

This is the worst skilled nursing facility I have ever been to. It is dirty disgusting and the staff is extremely rude. I had to wait over three hours for a cup of ice water. I do not recommend this place at all.

My wife currently resides in rehab-treatment at Park Bend [for the third time in five years]. - On each and every occasion our experience has been highly satisfactory both in terms of professionalism and responsiveness of the her improved physical condition on release. The facilities, service provided - and especially the professionalism, courtesy and friendliness of the staff - are all outstanding in my opinion [as a former Family Eldercare board member].

My mother-in-law (MIL) has lived at Park Bend for the past 7 years. The management and staff here really care about the residents. When the hospital medical team determined she couldn't go back to her assisted living facility after she began falling, I was certain my MIL would hate skilled nursing - but she LOVES it, I think because of all of the attention she gets. She loves going to PT and to the dining room for live music and chair exercises - and coffee. They always keep a pot on! She's not into arts and crafts or governance but they have that as well as a Resident Council. Oh, and she loves getting her hair done in the hair salon and her nails painted on Manicure Monday. The staff are friendly. She has favorite staff members but likes them all. They know me when they see me visiting and always greet me with a smile. I have yet to meet a staff member I disliked. The staff are conscientious. If my MIL so much as sneezes, they are on the phone with a status report. I am invited to every Care Meeting, which involves the entire care team for my MIL. The business office is also extremely helpful in navigating the regulatory maze of Medicare/Medicaid. I don't know what I would do without their help! I haven't eaten the food but my MIL has never complained about it. She is on a pureed diet now and doesn't even complain about that - so it must be seasoned appropriately. The facility is clean and about as cheery as a nursing home can be. It is well-appointed and has a library, puzzle, TV, and family visiting area. There is a bus that will take residents to their off-site specialty medical care, stay with them (if a family member can't be there), and then bring them back. The facility provides general practice geriatric medical care on site, and the podiatrist and wound care physicians make regular house calls. I will not kid you when I say that skilled nursing is expensive but from a value perspective, I am not sure you can beat Park Bend. The most important thing, of course, is care. The care is excellent.

This facility had significant issues during my mom's February 2018 stay. The food was not tasty, there were not enough choices and it was served in the room (not in a dining room). The nurses congregated at their station during the late night hours and were very loud. There was little effort on the part of the staff to help my mom with exercise. Later in her recovery she went to the near-by Buckner Villa. That was a MUCH better facility and I highly recommend Buckner Villa

Would like to say place was not what I expected it to be my mom was not in area that was explained to me and place need to be more sanitized smell of urine when walk through halls,should have more hand sanitizers on the walls in hallway think Purcell is a great brand the one you have doesn't smell strong enough to know if it sanitizes your hands housekeepers don't do a good job my mom bed had trash all under it tenants restrooms don't seem to be sanitized correctly most of all over night employees are very rude they don't acknowledge that you are there and don't greet you to where you should feel welcomed so hope my mom doesn't have to stay there long hope her recovery goes well and that she gets the rehabilitation that she needs and don't know if I would recommend this facility. Thank you for letting me give my review and hope facility corrects the things mentioned.

I have to say that the CNA'S & Nurses bust their behinds to care for the patients. Because of them this facility is what it is. Administration is a different thing. They are hard to get a hold of, rude and honestly just don't care about the patients.

My mom was at Park Bend Health Center. We couldn’t bring her pets there; I didn’t like that, but I do like Park Bend Health Center better than the other place. I think they were a little more attentive. You’re available to get coffee anytime you want, but in the other place, they cut it off. The room was a shared room, but she was in there alone. It was just adequate. Food was good.

I really think that Park bend is a really good place, I have my mom there and they did a great job. Therapy and Nursing department were very involved in my mom's health. The place smells good; it does not smell like a typical Nursing Home.
