Put It on a Card

Legal documents, such as an advance healthcare directive, won't do much good if doctors, family, and other people can't find it in an emergency. Encourage your parents to keep key information -- where the documents are, the name and phone number of their healthcare agent and attorney -- on a card in their wallet or purse.

Definition of Healthcare Agent

Your parent's health care agent -- the individual named in a durable power of attorney for health care -- is legally empowered to make decisions about your parent's medical care if your parent can't speak for himself because he’s unconscious or incapacitated. Also called a health care proxy.
